But how do you keep it clean when it’s 3 feet underground connected to the main sewer line?

The smart thing to do is put it indoors where the building drain is rarely that deep beneath the concrete slab. If you do have a deep situation, then you have to construct a manhole not much different than what you see in a public roadway.

Jamaica is very hilly. You should always have gravity flow to septic tanks for goodness sake.
