You are definitely a gift, Professor Zach Murphy, and of course, your entire team. I graduated a while ago, and I am still coming back to these lectures over and over.

NAFLD is now called metabolic dysfunction-associated steatotic liver disease (MASLD), I think we will need to get used to the new term soon.

Hope you can do a deep dive on the most probable cause/s of insulin resistance (IR). I've heard of too much PUFA's causing structural integrity issues on the cell membrane itself; IR is a natural reaction of the body to too much energy availability via cross-inhibition of both FA and glucose use (Randle Cycle); wrong food proportions in the context of too much carbohydrates. Impacts of sleep, stress, meal timing, intermittent fasting, extended fasting, macros in breaking of fast, exercise, etc.

Thank you🙏. My daughter was dx with T1D at 6 y/o, almost 16 years ago. I never really understood insulin resistance other than our endo. telling us that insulin is unable to process carbs, now I know why! Fast forward to now, she’s graduated from HS, went to Community College, is a Pharmacy Technician, working full time, and has gained ~130 lbs. She has experienced Tachnycardia and has Sleep Apnea, Chrohn’s (currently in remission 🙏🙏). She’s currently taking a GLP1 (haven’t seen a difference ), considering bariatric surgery but she’s so young 😔. But, I don’t see a way to ultimately improving her eating habits. I just can’t get through to her!

Thank you so much for the amazing effort, but the new guidelines state that bariatric surgery is indicated for a BMI of 30-35 when accompanied by metabolic syndrome or other comorbidities, and for a BMI of 35 or higher regardless of comorbidities.

Regarding the clogging up inside of the blood vessels... if you loose a lot of weight, and get into a normal bmi range... will most of the clogging get removed by itself? *or is that "damage" permanent?

Losing weight and achieving a normal BMI can have significant positive effects on your cardiovascular health, but it doesn't fully reverse existing plaque buildup in blood vessels. Here’s a breakdown of how weight loss affects clogged arteries: Plaque Reduction: Weight loss can help reduce the progression of atherosclerosis (the buildup of plaque in arteries), but it doesn't completely remove existing plaque. Plaques can stabilize and become less likely to rupture, which reduces the risk of heart attacks and strokes. Improved Blood Flow: Weight loss often leads to lower cholesterol levels, reduced blood pressure, and better blood sugar control, all of which help prevent further plaque buildup and improve blood flow. Partial Reversal: In some cases, lifestyle changes (like weight loss, a healthy diet, and regular exercise) can lead to a partial reduction in plaque size, especially if combined with medications that lower cholesterol (such as statins). Permanent Damage: If significant damage or hardening of the arteries (from calcification) has occurred, that part of the damage may be permanent, but the risk of further progression is reduced with healthy lifestyle choices. In conclusion, while losing weight can greatly improve your overall cardiovascular health and reduce future risks, it doesn't completely remove existing clogs in the arteries, especially if the damage is extensive.

Pretty easy fix. Animal based diet high in fats, medium protein and low in carbs. Doctors need to start looking at root cause, not just settling for "eat less, move more". Not good enough.

With regards to meds, antihistamines, especially centrally acting, are a massive cause of fat-specific weight gain and insulin resistance. Drugs like olanzapine will cause the most weight gain owing both to potent D2 and H1 antagonistic effects.

I am looking for help due to thyroid removal because of cancer, it has been years and still I am overweight, I do not eat any type of sugar includes any type of sugar and carbs, eat only 1 time daily. I just wish I could do something to help myself. I take Unithroid 112mg daily that’s all the meds I take.

Have you consulted an endocrinologist about this? May be an obvious question, but I don't see anyone else who could help you with a treatment plan, your problem is too specific for any other health professional

This is merely a hypothesis called the carbohydrate insulin model which is of high debate in nutritional research and more often than not its been shown to be disproven through clinical trials and inpatient experimental data with metabolic wards and isocaloric diets.

I think this video should be re-made. There are different theories of the pathophysiology of metabolic syndrome. Many argue excess calories cause hypertrophic adipose tissue to undergo lipolysis, releasing Free fatty acids(FFA) into blood where it enters skeletal muscle (forming DAG which inhibit PKC which inhibit the insulin receptor) and also the high flux of FFA entering the liver up regulates VLDL secretion. The biochemistry defending this has been established in vivo in humans using NMR spectroscopy. Other evidence is the actos medication increases adipose tissues capacity to store FFA, reducing lipolysis and reducing these symptoms, also on the opposite spectrum the disease lipodystrophy reduces adipose tissue capacity to store fat, increasing lipolysis and FFAs entering blood and exacerbates these symptoms. I need to emphasize, im not saying the obesity and FFA theory is dogma, however to say the adipokines are definitively the cause I personally believe is inaccurate and should be emphasized is theoritcal
