Edit : The Good Doctor, season 6 episode 16 Words and spelling Paragraph of the short's episode : Shaun (young autistic accused surgeon not talking) and his colleague came across a car crash. His colleague took care of a woman that was still in the car, while Shaun found the driver further in the woods after getting ejected in the crash. He saw his hand needed amputation on the spot or his life would be in danger. He took the victim back to the car with a backboard and performed the amputation while the ambulance was arriving. The plaintif (amputated victim) is going against Shaun, claiming he didn't need to be amputated, putting Shaun's practicing license in jeopardy. Shaun decides to set on the young attorney here to help him, as she has some severe OCD (the 3 "excuse me"...). Shaun wants to dissect the amputated hand, she tells him not to as, if he finds out he was wrong, they'll have to communicate it to the other party. Basically, the doctor here first confirmed that vasospasm (involuntary contraction of the muscle/artery/veins) always happens with the whole limb at once, an occluded vessel means something exterior or interior (blood clot...) is blocking the artery/vein. The radial artery (last pic) is one of the two arm arteries, the second one is the ulnar artery, but by him affirming it didn't vasospasm, he confirmed that it couldn't "go back to normal" without external action (Shaun amputating the hand). Court goes in Shaun's favour and judges him not guilty. After this, the attorney that spoke here and him go to check the hand, he indeed found the occlusion, confirming his decision was the right one.

Vasospasm can occur in isolated vessels with blunt trauma for example. The dominant blood supply to the hand in most people is the ulnar artery which is why the radial is used to get arterial blood gases and arterial lines. A minority of patients will need the radial for adequate hand perfusion, particularly the first and second fingers.

A lot of people complain that he's "not a good representation of it". But as someone who loves watching older movies and cartoons, Sean is probably one of the best depictions we've ever had. Before this is was demonic possession and "slow" sidekicks there for comedic relief. It's a drama series so everything is going to be exaggerated to some degree, especially the protagonist. Nobody in dramas acts like a "normal" person because then you don't have the totally good guy/totally bad guy dynamic Hollywood is still cripplingly dependent on. Before shows like this, name one show or movie with an autistic or autistic coded protagonist who wasn't saving/changing the world or suffering. Before this the only time someone autistic was depicted positively in media is when they were revolutionizes, trail-blazers, people who altered the course of history. And they were rightly praised for their accomplishments but it's all people knew. Sean is a doctor. That's it. He's the most "just a guy" we've ever had.

Wow. Am i going to be the first paragraph guy here? Simplest way to put this .... Traumatic vasospasm she confirms does not happen in isolated blood vessels, but rather the entire limb (or in other words not just one vessel but all vessels/arteries in the affected limb). The radial artery is in the arm.....but so is the ulnar artery (they basically run on opposite sides of each other. So when he confirms the radial artery did not spasm, she basically is saying, well then that by default means the ulnar artery did not either (based off the theory that the entire arms vessels should be affected by vasospasm, and not just one)

@@KateLynnRaven So from my understanding, if used a different example: it's basically like there's a bunch of tubes going through a pipe, the flow goes A to B in some, and B to A in some directions. Let's say that there's a crack in the pipe. All the tubes inside would get affected, instead of just the ones going from A to B. So that would be like Vasospasm, since it affects the whole area rather than an isolated "tube" (vessel).

Background to the short : Shaun (young autistic accused surgeon not talking) and his colleague came across a car crash. His colleague took care of a woman in the car while Shaun found the driver further in the woods after getting ejected in the crash. He saw his hand needed amputation on the spot or his like would be in danger. He took the victim back to the car with a backboard and performed the amputation while the ambulance was arriving. The plaintif (amputated victim) is going against Shaun, claiming he didn't need to be amputated, putting Shaun's practicing license in jeopardy. Shaun sets on the young attorney here to help him, as she has some severe OCD (the 3 "excuse me"...). Shaun wants to dissect the amputated hand, she tells him not to as, if he finds out he was wrong, they'll have to communicate it to the other party. Basically, the doctor here first confirmed that vasospasm (involuntary contraction of the muscle/artery/veins) always happens with the whole limb at once, an occluded vessel means something exterior is blocking the artery/vein. The radial artery (last pic) is one of the two arm arteries, the second one is the Radial artery, by him confirming it didn't vasospasm, so couldn't "go back to normal" without external action (Shaun amputating the hand). Court goes in Shaun's favour and judges him not guilty. After this, the attorney that spoke here and him go to check the hand, he indeed found the occlusion, confirming his decision was the right one. :)
