First episodes usually get created on their own so they can convince the business side to invest money for more episodes. When the show does get continued beyond the first episode then they still have time to make some changes so they can set up a better show. That leads to different or new scenes, characters, etc. In this case, I imagine that someone thought that having a plastic surgeon being famous for breast augmentations wasn't a good ingredient to the show, so they rewrote the role. It is rare that they release the first version, so if you're interested in seeing an example, I noticed there's a release of the unaired pilot episode of Sherlock, another good series. You can compare it to the official first episode and see a clear consequence of such decisions.

It’s probably part of the pilot episode. Pilot episodes are often not part of the actual series.

Why would the guy wear a gold Submariner in South America? He’s gonna get his arm chopped off. Probably why he didn’t make the show. Explorer two is way more appropriate.
