Thank you for the info! I've got a question: In the second step it says to identify stakeholders, but the RACI chart isn't directed to team members to assign those roles? Or is it focused on stakeholders?

Stakeholders have a stake. You may choose to exclude team members from your stakeholder list and treat them differently. That would be fine. But other stakeholders need to be informed or consulted.

How do you decide who should be consulted and who should just be informed?

In simple terms, ask yourself 'do I care what they think?' If their opinion matters, or they may have important insights, you need to consult them. If it has no impact, you just need to inform them.

Yes, absolutely. But they should be sure to share the load appropriately and not simply make themselves accountable for every task!
