Sir, what is important is whether a disease is controlled by single (major) gene or many genes(quantitative). When close marriages happens lethal recessive genes occures eliminate the individual in major gene controlled diseases or it depends on type of disease. Eg. Haemorrhage in royal queen victoria family where brother sister marriage happened over generations. So also sickle cell anaemia. When disease is controlled by many genes it takes many generations of closed marriages (we say inbreeding) to express disease and these diseases have been filtered during thousands of human generations. To avoid inbreeding of r close marriages our fore father's have adopted a system called ಕೊಲೆ, ಗೋತ್ರ, surnames to maintain the diversity. That is a Akki surname girl should not marry with a boy with surname a Akki. So also Shetty in south canara, Dhillon in Punjab. Any way arranged marriages are less these days and happens beyond geographical distances, casts, religion which has taken care of inborn diseases. Even then it occurs then it is due to genetic similarity or recessivity of genes for particularthat particular disease. Most diseases or genes expressed due to internal cell environment or what food you ingest by practice. You cannot do any selection in humans due to social conditions but we do it in cattle and animals. Who will go for prenatal selection of couples by early tests and what happens to love marriages ad you did.

Diseases due to chromosomal abnormalities happens due to irregular disjuntion of chromosomal cells of egg or sperm in meosis but trisomy or monosomy sperm fail to fertilize an egg and nature has taken care of to select only healthy sperm. On the other hand sedentary egg can harbour trisomy during meosis but can retain for fertilization. Even then selection of egg cell also take place during its release to fallopian tube. By chance egg carrying abnormal chromosome is released it can still fertilize with normal sperm but leads to inborn diseases. However, such faetus can be aborted but frequency is very rare.
