Thank you for this video. I grew up with this disease and I managed to become a journeyman Carpenter. Now that it has disabled me, and it is becoming so much worse - i want to campaign for occupational safety standards and education for others that build; especially with regard to other vectors that spread this. Thank you for work and advocacy!!

Lorraine, some people have asked me questions about security of identifiable information and that is why they have not yet done this. I myself used alternate information for birthday, social security so I could fill this out. My questions would be who has access to the identifiable stuff, is it encrypted in the server. Additionally for me it was difficult to fill out because I have been symptom free for 9 years without any medications and this seemed to assume that I was still on treatment.
