A charm exclusive to the Omori route. Abbi's Eye reduces someone's heart by 90% and their juice by 99%. In exchange, they get massive stat boosts, and the fight starts with an Observe-like effect that breaks Perfectheart's second phase.

Not really. This is actually an improved strategy that just happens to remove the one item use. Aside from Annoy (whose item equivalent is the exact same), each skill or follow-up is important and more necessary than any item would be. Basically, older strategies have Hero use Sno-Cone to fully heal Aubrey before she used Last Resort. This strategy instead uses Call Aubrey, because the extra damage dealt does more than the extra damage a full HP Last Resort would do. The intended improvement is to remove the use of Curveball and the one in three chance tied to it, and the extra damage is enough. It just so happens that the whole run is now itemless, hence the title.

Why use appendix for kel? isn't it completely useless?

Yep. I use Appendix as a joke when possible if a given character’s stats don’t matter. You could technically use any equipment on him as long as he’s still faster than Perfectheart.
