As someone who’s diagnosed with this, thank you! I’ll be using this to explain my treatment to a family member that doubts the validity of my needs, along with showing my daughters why they need treatment for life. One of my daughters was dx’d a couple years ago, the other more recently. It’s definitely not fair! It almost took my life!

As someone with PA and in a group with tens of thousands of other sufferers, the neurological symptoms often come first . Most doctors will never do a through history with diagnosis because most have a ten minute time slot . , there are also many different labs who use varying degrees of what is “healthy / normal range which means many go undiagnosed . My niece was diagnosed through blood tests her level was 250. Other people have had a level of 130 and been told they don’t have PA even though the antibody test for intrsic factor or gastric factor antibody were positive . Many doctors will only give 3 monthly injections which is not enough , so glad understand the initial loading schedule and the monthly maintenance that is essential .

Well this was a great video!! Im glad I watched before my exam! I was just taught that pernicious anemia just meant being vitamin b12 deficient. I was not told that it was an autoimmune involving intrinsic factor! This made it all so much clearer!!

So if a patient is suffering from pernicious anemia vitamin B12 oral supplements are not an option due to patient's stomach inability of absorbing the vitamin?

Exactly.. it needs to be administered to the blood (or intramuscular in that case) without passing through the GI tract.

Not true. I treat my disease with sublingual high doses of vitamin b-12 and b-6. You can find b-12 sublingual which goes under the tongue and dissolves there don’t try to swallow try to have it all absorb under the tongue. I honestly take about 15,000 mgs of b-12 daily. I also get the 1000 mg injections and iv iron drip.. my stomach is wrecked honestly. So I have to be very careful. A lot of medications I take are through iv, needles, or under the tongue.

Hey, yes. This is exactly what I was saying. Sublingual administration is not the same as oral. It is for bypassing the stomach and liver metabolism. Therefore the body gets its vit. B12 but without gut absorption and conjugation with intrinsic factor in the stomach.

but if you administer vit b12 intramuscularly,by-passing the intestine, how do the body cells absorb vit b12 that isn’t bound to IF?

I think because instead of absorbing B12 in the GI tract to get to the blood, you can now get to the blood through the tissues (Intramuscularly)

@@btudrus its probably easier to get the B12 supplement from injection (especially if its every day and most likely done at home) rather than IV

@jayx3843 Yes, It causes macrocytic megaloblastic anemia. So any type of deficiency caused by vitamin B12 or castle intrinsic factor or folic acid def causes this!

Recently a non medical recommended B12 (but with B1 for absorption) It appears to have made a diff to my energy levels. Ideally a blood test to verify pre and post should have been done, but am not well. Injections sound like one could have a high doze, however google shows side effects include tingling fatigue etc. Was wondering what would be the daily recommended dose if taken orally and if B1 is necessary. Am weary of supplements cause of caking agents :(

If you have pernicious anaemia oral supplements will not be absorbed . Yes you should be off all b12 supplements before blood test or yiu will get a high level of B12, but this will be the b12 in the blood, and not the active B12 for which there is another blood test . A good lab will run a B12 test and if that is in the lower end of normal they will do the Active B12 test which confirms cell level of b12 . B1 might be relieving some of your fatigue. The injections do cause some tingling but that is just the nerves rebuilding . It takes about 7 years to lose your B12 levels , by that time people have lost the ability to walk , before paralysis (which as I said can take years ro develop people will have tingly pins and needles in arms and legs , this is due to nerve damage . We have people in our group of Pernicious anaemia sufferers who were diagnosed very late and have irreversible paralysis. If you are not eating enough meat , you might just need to take a pill as B12 doesn’t come from plants . Beef liver has the highest source of B12 or Dried liver pills can increase B12 if you don’t like liver

The later is MUCH more bioavailable, even if the body has a problem absorbing anorganic iron. " It takes about 7 years to lose your B12 levels" It depends on your genetics. Some may have become problem much earlier.
