What Does The Pes Anserine Bursa Do?
As mentioned above, the pes anserine bursa normally functions as a pulley to help reduce friction between surfaces.
As you can see, your knee has a lot of movement available in forward and backward motions. The pes anserine bursa is in the perfect location to help tolerate these significant forward and backward actions.
Under normal circumstances, the pes anserine bursa is not irritated or swollen since the compressive forces placed upon it are within its tolerance limits. Yet as we have all discovered at the beginning of every spring/summer, sometimes we go beyond our body’s tolerance.
What am I talking about referencing?
Sunburns!
Our bursa, just like our skin, has a tolerance that can change as you gradually expose your body to challenges.
In the cases of sunburns, your skin has been exposed too quickly to the sun’s rays and has a rapid and painful response.
With pes anserine bursitis, rapid exposure to running, jumping, HITs training, or walking can be enough to create a production of excessive fluid and inflammation of the bursa.
Inner knee pain will rapidly follow.
A rapid increase in physical activity is not the only cause of pes anserine bursitis. There are plenty of seasoned runners and triathletes who suffer from this type of overuse knee injury.
In these situations, a trained eye can help you see if your body lacks function in other regions of your body that are producing overuse of the knee and the bursa.
Consider the Mobility, Stability, and Strength Model when looking at your overuse knee injury.
Have you had any recent aches/ pains on the OTHER leg?
Plantar fascia pain
Knee pain
Groin
Lower back
IT Band
Perhaps you have had a chronic ache on the other leg that you’ve managed for years with a foam roller or stretching?
When discussing overuse injuries that are not from activity spikes, you have to look at it another way.
What is not working (dysfunctional and not painful) to make my pes anserine hurt?
Using a simple squat movement assessment (in-person or Zoom), we have found countless athletes placing MORE weight on their injured knee than on their “good one.”
Does Arthritis Cause Pes Anserine Bursitis?
People with arthritis of the knee and hip can experience pes anserine bursitis, but not all do?
Allow me to draw your attention to another KZbin video I created to prove this point.
This video demonstrates a non-surgical therapy called skin rolling. Skin rolling reduces “arthritis” knee pain with about 1/2 of the people who try it.
Read all of the comments below the video, and you’ll see I’m not making this up.
If you have been told your arthritis is the cause of your knee pain, yet rolling skin works, you may be in for a pleasant surprise. You can get better rapidly, as you can see. Now you need to reverse engineer HOW you developed this painful situation by addressing your dysfunctional/ non-painful areas.

I was given a diagnosis of torn meniscus after a disastrous aerobics class in 1990, when the pain continued for many months. I was not given physio, just a direct ref to orthopaedic surgery. They did the op, and I had no physio after (admin error) and I continued with knee pain for over 35 years. Any physio I had concentrating on the joint itself, even though I felt it was all soft tissue injury. At one point around 2017 my knee was XRayed and he found OA and blamed it on that, despite me having the same pain from day one post surgery in 1990. Eventually, I used an ultrasound device (actipatch) for a few months and this reduced my pain considerably. This treatment really is supposed to work for shallow injuries rather than deep in the joint. Now when pain returns, it is in the PES area, as before, and as I have psoriatic disease which has so far been responsible for many enthesitis events I realise this is probably just another of those events, and probably always was from day one. As my cartilage op proved to address only a tiny tear, I was lead to believe that I had "made up" my pain and later had fibromyalgia added to my list of "problems". Now, whe

continued... I did have an MRI, andwas told it was "mild" effusion. If this is mild, I can't imagine what severe swelling would feel like.

February I injured my medial ankle. Now i'm healed. I started running. I haven't run in a very long time. After running 1 mile this area hurts!! I've been resting for 5 days and it still hurts :(

They are similar but it comes down to each persons ability and tolerances. For example, we would give some people with a meniscus injury box squats on day one, while someone who is deconditioned quad rocks since weight bearing is too hard. The difference is not as much in the diagnosis as the individual. That all comes down to assessment. In our office we assess, not guess, at what to give. Did any of you find a solution or diagnosis? Im suffering greatly 6mths on and feels like its getting worse...cant walk for longer than 5mins without pain starting

@@pault477 I had steroid injection. It was my own finding through videos. I feel much better, before the injection , I had pain when I walked, I am walking more now. I had knee replacement one year ago. Pain was not going away, only anti inflammatory drugs would work so I figured, it was some kind of inflammation. Surgeon had no idea, he kept saying that structurally everything was good. I was so frustrated.

@@jeeli786 yes - the surgeons are lacking in diagnosing problems after surgery. They usually recommend PT which in some cases help but the injections seem to help the most. Had injections on both sides - that helped - now need one on top of knee. The TKR knee hurts more than the other knee

Great video.. a refresher for me. I am actually concerned that i may have a meniscus tear. I thought I have Pes A., but the swelling is taking time to go away...more intra by the Pes. A and very little by the sup. lat. knee. I will make an appt with doctor this coming week. I am able to walk fine, but squatting deep or crossing my L leg over my right I feel it on the sup lat knee. Anyway is has been about 1 1/2 week. The ice helps combined with ES. But you are so right , I can see this being misdiagnose. So I will make sure. I just gave you a thumb up and subscribed. HI Sebastian, thanks for the tip. That is very interesting. Although I have to say my knee has been feeling pretty good w/ regards to walking. Maybe some pain if I bend my knee too quickly or deep squats. I'm feeling a little more confident that a little more rest, ice , and stretching/strengthening will be the remedy at this point. But I am interested in your videos and did subscribed. have a great mother's day with the family :)

Great video! I didn’t realize the meniscus injury could drop down like that. My pain is in the apes Anserine area, is very tender to the touch in a tiny spot, and is only triggered when lifting leg (activating hip flexors) To me, this points to Pes Anserine. Especially since my previous meniscus tears didn’t have pain in this motion. Thoughts?

My doctor told me maybe I have Meniskus . MRI was not so clear but the spot I have the pain is like Bursitis problem

And how do you know that you have exactly Pes Anserine? Are there any special tests for this? My dad has pain in both knees in these places, but how can I be sure that it is Pes Anserine and not something else .... for example, Osteonecrosis of the knee

Hi I don’t think it is swelling, but hurts when I walk, I been doing the exercises but it only reliefs the pain for thirty mins to an hour and when I walk again it hurts the medial of my knee...

Hi, i am doing alot of one leg standing, and balancing, wall squat with a medicine ball in between my legs, also bridge exercises and also single leg bridge holds. All of these help the pain for about 30-45 mins and then the pain comes back

Also whenever i am sitting, whenever i go to extend my leg (knee extension) my knee would always make a loud pop / click noise like cracking your knuckle (it doesnt hurt) but bit strange it would make a noise like that

a valgus stress test will show a MCL ligament injury vs a pes anserinus tendonitis. You cant feel meniscal pain, meniscus has minimal nerves

There are various tests that increase pain with meniscus problems in rotation and flexion. LCLs often show gapping on normal people without pain. The pes A is often a spinal referral problem with poor hip stability and presents as local tenderness to touch on exit point of saphonous nerve.
