Peter Gabriel went through a nasty divorce, and relationships with Sinead Oconner and Rosanna Arquette. This is why there is so much agression against the woman and other characters. He was going through some serious PTDS and depression.

This song is all about therapy he went through after the break up of his marriage, it talks about all the horrible things he things as he digs into his life and the support he needs to get through it, the video seems to deal with a domestic between his parents and unhappy childhood memories
