The scapulohumeral rhythm defines the joined movements of the scapula and humeral head. It can be evaluated during active abduction. The scapula and humerus will follow a sequence of movements, which Kai explains with the help of visual cues.

The video shows that in Phase 1 humerus abducts 30 degrees , Phase 2 another 40 degrees, and Phase 3 another 60 degrees. If that is true, how come the total degrees of humeral abduction come up to 120 degrees at the end? Phase 1+2+3 = 30 + 40 + 60 = 130 degrees. What am I missing? Are these numbers just approximations for each phase? It will be great if you can clarify.

Hi i am doing a university project on the scapulohumeral rhythm. I have done some filming of participants performing Abduction of the arm and I am just wondering how you best measure the angle of movement of the scapula when analysing the video. I am using the program of dartfish which has an angle tool. Cheers.

+SterosFysio that's true, on our Instagram post we mention the controversy concerning SH rhythm, it may not necessarily be a causation but can be a clinical feature in a symptomatic population

Do you agree that at a 120-degree abduction of the humerus, the latter is pushing against the acromium and therefore stretching the coracoacromial ligament? Do you think that the passive hang, if performed daily, can help increase the subacromial space with time? I have had bicipital tendinosis due to shoulder impingement for 4 months now. After I attempt to do a passive hang, I feel pain and stiffness when I lower both arms slowly in front of me. The same pain occurs in shoulder flexion. Can you tell why the pain is occuring after a passive hang and which structures are involved? In the passive hang position, as far as I know, no structure should get impinged since the cuff muscles all lie behind the humerus in that position.

The idea of shoulder impingement is highly flawed. There is research showing that people with shoulder pain actually have more inferior glide than people without: www.ncbi.nlm.nih.gov/pubmed/25103132 On top of that, most rotator cuff tears are seen on the underpart of the tendon and not on the upper part where it is said to impinge between the roof of the acromion and the humeral head. 'Impingement' is therefore nowadays called subacromial shoulder pain or just shoulder pain as it is pain coming from the rotator cuff and/or the bursa. What tendons need to recover is load that is appropriate to the tendons reactivity. So you should train your rotator cuff under the guidance of a physio.

Hlllo ..phySio tuitor .some clinician say during humeral abduction the sc end the clavicle slides inferiourly ..... kindly clear ...becoz in treatment they give inferior glide to clavicle in sternal end to improve elevation

There is movement to be observed at those joints during Abduction of the shoulder. Haven't heard of the described SC mobilisation. Also wouldn't look at the SC joint as the main limiting factor in reduced Abduction

Hi Guys! Sorry just confused there slightly with the clavicle rotation. In phase three you mention 30-50 posteriorly and 15 elevation. However you then mention it is only 5-8degrees? Also the total clavicle elevation from all phases equals to 35 degrees- but then mention 20 degrees at the ac joint and 40 deg at the sc joint - which equals 60 deg. Sorry for this painful question LOL, thanks

Hi Brittany. The 5-8° is the relative rotation to the acromion because of scapular rotation. 30-50° is the total amount of rotation measured along the long axis of the clavicle

What about upward rotation of the scapula ? or what do you mean exactly by lateral rotation of the scapula ?

't is al een tijdje geleden maar volgens mij was dat gebaseerd op Magee. Echter wordt de verhouding van 2:1 beïnvloedt door heel veel dingen zoals geslacht, gewicht, sporter/niet- sporter en nog veel meer. In onze online cursus hebben we het uitgebreid erover.

Sir!I have subacromial sub deltoid bursa diffusion shown in sonogram. Since 3months I do pt.my hand can't move back properly,having radiant pain in upper arm.By turning back my hand ,tremendous pain on top of humerus like hinges.which exercise will helo me.pl.guide.I am 62.having cervical &herniated disc...c456.

Strengthen your serratus anterior and stretch your rhomboids. You can do protraction exercises for the former and stretches there are plenty you can google them. 👍🏻

Ur welcome. Good question! Usually the body finds a way to achieve their movement goal, in this case abduction, by recruitment of a different muscle pattern than what would be described as 'ideal' by the literature. So what you see in for example patients with should pain is more recruitment of the upper trapezius in order to compensate a weak/injured rotator cuff. In case of winging, a possibly weak serratus and lower trap might be compensated by more activity by the upper trapezius

I experience shoulder pain in my left shoulder when depressing my shoulders whilst hanging from a bar. A lot of people call this an active hang. Is it because I'm messing up the Scapulohumeral Rythm?

Hey cliff, the rhythm is pretty much identical in Flexion as it is in ABDuction. So it's not necessary to do another video on it Cheers

Is it normal to have a scapulohumeral rhythm of 1:1? When I move my arm, my scapula always moves with it. It’s really hard to move my arm up above my head, what’s wrong with me?
