I think the lack of information on prescription medication is due to the lack of time GPs have to hear all the issues, select the right treatment and then they’ve run out if time to explain the correct use of any prescriptions. I’m really glad I found you’re videos just before I was prescribed epiduo as the extent of advice given to me was “don’t come back for at least 3 months” which did nothing to prepare me for the retinization

We need you on Harper’s Bazaar dermatologist video segment. Harper’s Bazaar has a KZbin channel. They periodically have a dermatologist do Q&A. This would be good PR for your brand. They seem to be open for dermatologist contacting them. Give it a try.

My GP didn’t give me any advice when I was prescribed Differin, so thank you for this. What would ‘under control’ look like, when it comes to acne, so I can set my expectations!

I've been using BP and tretinoin every other day for many many years, so I was very surprised to hear that BP i's supposed to be quit once the flare is gone. The thing is that I never stop breaking out. There are good periods, but there are still bad ones. I don't know what to do really. I don't feel like my acne is 100% under control. I use BP all over my face, on top of a moisturizer.

ivanuzzo81 ...I know benzoyl peroxide and Differin can be used together, I’m sure benzoyl peroxide cancels Tretinoin out, maybe that’s why your still breaking out ??... look up Dr Dray on KZbin she has lots of videos on this subject , hope this helps

@@monicaduncan7109 Hey! Thank you for reaching out. I know Dr. Dray and I have just checked her videos on this topic. In "differin q&a with a Dermatologist" she says in fact that the combination is fine. Anyway, I acknowledge the fact that I was probably very discouraged when I wrote that message, most likely in the middle of a purge. I still get those periods from time to time, but overall my acne is at bay. Surely my predisposition is so strong that my skin is never 100% clear. I think that somehow I have to come to terms with that. Needless to say acne, like any desease, can be very taxing mentally. I would like to be care free and not care about skin care, or what I put in my mouth, or how stressed I am or how much sleep I get, but I know that if I neglect all those things my acne would just get worse. It's a disease. Not life-threatening but yet still a disease, which behaves as such.

I wanted to write a huge thank you to you for your videos and so valuable information! I`ve been struggling with rosacea almost all my life and a couple of years ago some acne appeared as well. After binge-watching many many of your videos, I think I finally sorted out my routines. I started using Azelaic acid + Niacinamide and it helped so much with blemishes and also my redness looks much better! I`ve been using it religiously for 2 months now. I`m thinking to start retinoids. I`m a bit scared because previously I had a really bad reaction to it. But now I have a strategy thanks to you! I`m so grateful!

My questions would be: 1. Is there an alternative Anti-Inflammatory to Benzoyl Peroxide? I hate that it stains EVERYTHING in my home from towels to sheets to clothing. 2. I have oily skin and my doctor recommended that I skip moisturiser - so I did. But I have the feeling that this does make it worse. My skin only got shortly better from the actives and now is getting worse again with even more oiliness, some very dry patches and cystic breakouts. THANK YOU SO MUCH FOR YOUR VALUABLE ADVICE! :) PS: I think the answer to the question, why so many people ask about prescriptions is because you get something prescribed by the doctor in an appointment and then you have no chance to speak to the doctor in the time after. You can only make another appointment which, at least with good, popular doctors, can take months. There is something missing where you can ask your questions and get a quick response. I think that's why a lot of "digital doctor" service get so much attention.

THIS VIDEO is what I had needed for the past 2 years when I started my skin care / cystic hormonal acne journey!!! Thank you, thank you, thank you! Make more of these, please! I’m on acne meds both for morning (aczone) and night (Clynd+Tret) and have been cleared almost immediately after starting using them, and have been on maintenance since (sometimes even skipping aczone for a few days, and as of recently alternating my night treatment with your Nightly Serum!) and its so hard to incorporate other products when you are not sure what goes. And let’s face it, I can’t visit my dermatologist every time I decide to start something new.. so this is so helpful! - thank you and I love you and that is all. 😂😍❤️🙏🏼

The tretinoin confusion for me in the USA comes from dermatologists not really understanding how to use it. I know that's hard to believe, but asking basic questions about a routine can result in blank stares. My latest derm doesn't even recommend using sunscreen with tretinoin. I honestly think most people are getting prescribed tretinoin by a doctor (as opposed to getting it shipped from the internet). You're getting questions from people like me whose doctor doesn't understand how to use tretinoin in a simple routine.

I just want to say Thank you so much, your knowledge and willingness to share it has probably saved my face tbh. We dont have derms where i live and the one that is located in the nearnest city has a waiting period of almost 6 months and is almost impossible to see, you’d have to be in real bad shape if it’s not cancer related. I basically have to do my research myself and then ask my GP for a specific prescription. I know it sounds risky at best but I really have relied on online professionals such as your channel to get my acne in check.

This is off topic but, I was at your office in London this past week to purchase some of your products but the person at the front desk said the products could only be sold to patients. She eventually sold some to me. I wanted to purchase more but she said no because she would not have more to sell to other people. It seem a bit strange since, I think, the purpose to selling a product is to sell as much as you can to the customer who wants the product. I hope I don’t sound rude. Just thought you should know. I love your face lotion and sunscreen. I just feel bad that I could purchase some while I was in London

I don't have acne any more really. A few acne scars. But when i use the below retinol my skin becomes very dry and flaky, so not sure whether i should continue every other day or do a weekly treatment. My AM routine AlumierMD Purifying Gel Cleanser allabout eyes Clinique Biologique Recherche’s Lotions P50 Colloidal silver water Hydraclrite AlumierMD moisturiser Skinoren Ceramidin DrJart EltaMD SPF 46 broad spectrum My PM routine Elemis pro-collagen rose cleansing balm ( yes, I double cleanse and it works for me) AlumierMD Purifying Gel Cleanser Medik8 Retinol Eye TR - what are your thoughts on retinol eye creams?! Then every other day Retinol Resurfacing Serum 1.0 AlumierMD (I transitioned from Differin cream 0.1%) On the other days I mix a pump of Sarah Chapman Overnight Facial Serum with Clarins blue orchid face treatment oil for dehydrated skin I would really appreciate your advise! Many thanks K x

Hi Dr. Sam! I hope you're doing well. Your videos are highly informative and I appreciate it so much. I just want to ask how I can gradually remove Benzoyl Peroxide (BP) from my regimen and use it sparingly. I am currently using BP+Clindamycin topical treatment in the morning and Tretinoin at night time to treat my moderate to severe acne. I am also using Cerave Moisturizing Lotion AM/PM. I have cleared some of my acne now and I only have a few left. I am afraid to remove BP from my treatment thinking that my acne might worsen again. Also, I want to incorporate Niacinamide+Zinc from The Ordinary to my regimen because my skin is extremely oily. Looking forward for your reply Doctor, thank you so much in advance. Have an amazing day! -Prince

I hope you see this, please reply if you do 🙏🏻 I've been using my own mix of adapelene 0.1% and bpo 5% in the evenings, it's only been 2 weeks but my skin seems to be tolerating it well. I have already noticed less cystic breakouts and they come to a head and go away faster Another major concern is comedones, I really want to introduce azelaic acid (skinoren) into my morning routine I currently use the inkey lists salicylic acid cleanser in the mornings Is it OK to still use it when using the azelaic acid or should I use my gentle plain cleanser that I use in the evening? Thank you so much for your videos, I've been binge watching and I've learned so much Hopefully I can finally clear my skin up for good this time 🤞🏻

What do you recommend when benzoyl peroxide is the only topical that keeps a patient’s acne at bay? I’ve been using Epiduo for 3 years and every time I stop, I get a scarring breakout. I have tried both Tretinoin and adapalene alone and neither control my acne. Would you recommend accutane in this instance?

pigwidgeonfan i have exactly the same problem. No matter what i use: tretinoin, retinol, adapalene, azelaic acid, bha, niacyniamide....i do still have new break outs. The only time my skin calms down a bit is when i use benzoyl peroxide or antibiotics 😔 and i hate bp so much, because of hiperpigmentation, prooxide properties, making my skin regenerating slower and bleaching things.

I got two free samples given to me: One says "Pure Hyaluronic Acid Serum" and the other one says "Bio Brightening C Serum". Should I wash my face before putting one of them on? I've heard a dermatologist say she doesn't wash her body except for her armpits and private area (and I think face, though I'm not sure), because washing compromises the acid mantle and causes epidermal water loss via evaporation. A lot of washes mess with the pH, so she just doesn't wash. Maybe she just washes her lips and eye area and nostrils?

What if you never make it to the blemishes starting to subside phase?? I react quite badly to prescription topicals, even finacea and even after really persevering leave me with red angry irritated skin with the acne mixed in even with minimal use. And internal medication on,y allays the systems. They always come straight back immediately after being discharged. This has been a 20 year journey under the guise of many derms. Is it best to just accept that this is my skin rather than try to keep fixing it.xx

I have a Kyleena IUD in to help me with my pain due to endometriosis (tried the contraceptive pill and had great skin but it didn't help with pain that much and had side effects)... however, the side effect I'm experiencing with the IUD is the worst yet. Over the course of the last year, my acne has gone from bad to worse. I now have acne all over my body as well and don't know what to do. I've started on tretinoin on my face only and I think that I'm in the "purging phase" but am not sure if its causing a contact dermatitis as well.

Thanks for the acne series videos. I’m 40 and only getting my adult acne, which started in my 20s, under control now by following your routine. My spots were almost all on my cheeks, where I also have a lot of redness and post inflammatory hyperpigmentation. The rest of my face has always been okay. Please can you advise why I have so many problems with my cheeks instead of the jawline as would be usual? This really puzzles me! Also, my nose and chin are ridiculously oily despite using mild cleansers etc. My foundation will literally melt off in 2 hours unless I use long wear foundation and powder, which I am now trying to avoid. Please do you have any suggestions on how to control the oiliness as it’s a major nightmare for me! Thanks. (Fellow NI girl here).

Hi Dr Sam another great video! My question has to do with incorporating Dr Sam’s serum into my nighttime routine. I’ve been using Tretinoin for almost two years now (daily at night) and my skin has well tolerated it. I do find my cheeks are prone to dryness afterwards so I’m thinking of alternating between Tretinoin and the serum every other night, given the serum may be ‘gentler’ to my skin. Would like to hear about your thoughts on this!

Retinoids reduce sebaceous glands but as we age, our skin naturally loses sebum and gets dry but paradoxically retinoid is anti ageing... I do understand the fact that it increases skin turnover but it also reduces sebum...Is that gonna affect dry skin in the long run as it is a well known fact that dry skin tends to age faster than oily skin due to less sebum

Or if can I use benzoyl peroxide (alone) in the morning and then tretinoin (alone) in the evening? Or if I should use one of them first (ex benzoyl peroxide), then use the other one (ex tretinoin) when my acnes get ij control? And of that's the case, for how long must I take benzoyl before i switched to tretinoin? I'm really confused about it, hope you can help me. Thank you so much.

Hi Dr Sam.. Ive just found this series and i've just restarted trying to get a handle on my skin. I have cystic acne, thankful that it doesn't tunnel. I also have blackhead, get pustules, and believe i have rosacea. I don't get acne in one place more than others, its all over from hairline down to the underside of my jaw. Even on the space just in front of my ear, behind my ear and in my ear. Ive started titrating with the 2% granactive retinoid from the ordinary as well as using PC 2% salicylic acid. My overall skin seems brighter im still breaking out. Im using cosrx hyaluronic acid toner, ordinary niacinimide & zinc, inkey list peptide moisturizer and inkey ceramide night treatment, mineral sunscreen daily(w reapplication). Im 8 weeks in. Hi, Arantxa. I think any exfoliation might be better in the morning. At least that's what I do, especially in the shower when getting ready for work. But, let's see what Dr Sam says.

These videos are so helpful!!! I use tretinoin in the evening and azelaic acid in the morning. I usually dampen a cotton pad in the morning to lightly take away any residue of the retin-A and then apply the Skinoren. I am wondering if I need to do this light cleansing before the Skinoren or not. Also - can I apply moisturizer on top of skinoren???? THANK YOU SO MUCH!!!!!!!

@@shania.janine hi. I wash my face with a mild cleanser in the morning and apply skinoren and then do makeup or not. Evening cleanse face and apply retin a. This has been great but spirinolactone was the game changer in quelling breakouts

Any thoughts on long term tretinoin use? I’ve been using it for almost ten years (several nights a week) and it really keep my skin smooth and clear. I told cosmetic injector this, and she cringed. She wants me to slowly back off of using it so that I don’t get thin “plastic” skin. I tried this, but my skin freaked out and started breaking out again. What do you recommend? I’d love to keep using tretinoin since I know it works for me, but I don’t want to do long term damage to my skin.
@ninalou4629

Are you sure your injector is not trying to make more money off you? If you stop using tretinoin slowly but surely you will start getting wrinkles and will need more cosmetic work done. If your skin is doing great after 10 years of use theres no reason to stop there are people who have been using for far longer as long as youre using sunscreen everyday youre golden.

I am sure she isn’t because she is a personal friend of mine and does my injections for free. There is a lot of information out there about long-term tretinoin use thinning the skin. I’ve been told this by multiple medical professionals.

Hi Dr Sam. Thank you for this series, I've been able to take a huge amount of info and suggestions away from it. Just a question about some of the products recommended such as the LRP effeclar duo and the LRP anthelios shaka sunscreen, they both seem to have comedogenic ingredients in them. Should I be concerned about this or will the benefits of the other ingredients outweigh the possible pitfalls of the comedogenics? I've been using the Anthelios XL for a couple of years thanks to your suggestion, but since they've reformulated to make the Shaka version, I've found myself breaking out.

I don’t mean any disrespect, but I’m afraid telling people to stop the benzoyl peroxide as a preventative measure once their acne is under control is going to cause a lot of people a lot grief when their inflammatory acne comes back. If they replace benzoyl peroxide with the azelaic acid it won’t be an issue I guess because the acid is supposed to be comparable to BP, but if you stop the BP and don’t add something with similar properties, you just lost one of the things that helped clear your skin. Unless your skin and the reasons behind your acne suddenly change, then you need to keep using the products that successfully cleared your skin indefinitely as a preventative measure. Of course, for some the acne might remain under control without the BP, but that must mean they didn’t need it in the first place (or they replaced it with a product with similar properties). Again, I don’t mean any disrespect, but I’ve had acne for 25 years and I know the anguish that comes with clearing your skin and then having a really bad breakout for whatever reason. In my case, I use a BHA, BP and treat my hormonal imbalance to keep my skin clear. If I stop any of these, the acne comes back to different degrees depending on which product I stop. Thus, I need all three as preventative measures every day.

Misce Llaneous i do agree with your every single word. 21 year long acne sufferer in here 😔 no matter what i use aside, when i want a quite clear skin i need benzoyl peroxide or antibiotics. Everytime i switch from bp to skinoren or effaclar duo...i start to have more and more breakouts. I suppose that means bp is the most important thing in my regimen 😕 and i truly hate this ingredient

What if a retinol or the ingredients accompanying it cause problems when sitting on your skin overnight? Would the retinol still be effective if you left the product on for a certain period of time(contact therapy) and then rinsed off later?

Hi Dr Sam, what should I use to fade hyperpigmemtation? I feel like I've pretty much cleared my acne but I've got so many red spots on my face left from previous pimples that I still feel like not a lot has changed. I am currently using azaleic acid and niacimimide, as well as the ordinary granctive retinoid. i'd also like to incorporate daily exfoliation (I use the ordinary aha bha peel twice a week already). What do you recommend?
@kendallstark4302

Are you using an SPF 50 in the morning? Too much sun exposure could make those marks darker. Also, as she said, patience is paramount, since it could possibly take six months to eliminate them. PS: how do you find The Ordinary's AHA/BHA mask? I'm thinking of getting that, or their glycolic acid toner.

@@kendallstark4302 Yes, I am! I pretty much have to, I am extremely fair. The peeling is great, i find it leaves my skin looking brighter and cleaner. I'd definitely recommend it, considering it never irritated my skin and I do have sensitive skin, and I never used a chemical peel before that. The only thing, it tends to sting a little if you put it on already irritated or dry skin

@@dianahabtegaber7165 Thanks for the recommendation! I'll definitely try that peel. Like you, I'm also very fair (and a redhead with blue eyes) so I need to wear sunscreen basically always. I've always worn it in the warmer months, but now I'm doing it every day, rain or shine. I have a tiny bit of what I think is hyperpigmentation, so now I just want to eliminate what I have and also prevent further damage. Good luck on your journey! 💗

Hi Dr. Sam -- my case is tough. I have PCOS and began taking 100 mg of spironolactone for hormonal acne on my chin a year ago. At first it worked wonders, and I was clear for months. Then the acne came back, almost to the point that it looked the same as before! I upped my dose to 150 mg, but I'm still not clear. I've also been taking a generic of Ortho Tri-Cyclen this entire time (have been on it for 6 years) and apply a generous amount of benzoyl peroxide to the affected area every night. All my skincare is no-nonsense and fragrance free, I used SPF religiously, and I eat a pretty healthy plant-based diet and get plenty of exercise. I was so discouraged when the spiro stopped working after I thought I had finally found my miracle cure. Have you had patients who suddenly stopped responding to spiro? What would you recommend?
