We gotta share this . This melting pot of shared knowledge is finally elevating dentistry and healthcare to be better and better and finally the best honest way to practice with integrity . I am learning so much on how to truly save my teeth .

Jaz, I thought I was the only one who came up with my so called "inner philosophy" about refusing doing horizontal preps on teeth which don't have enamel all around which is basically most of my crown indications. Glad to hear that someone shares the same opinion.

I feel disheartened, stranded and lost. I know dentists in my area or country, don’t do dentistry with this intensity of care and intent to give teeth the best chance of lasting longer . Ortho extrusion?, saving wisdom teeth by repositioning them or doing a hemisection. Is there a practitioners list for dentists can add their name ?

Sorry but, what kind of bonding does he use, I do only direct composites and have done this for 10 years now, unless the tooth already has a crown. I do complete rehab in composites. I treat other dentists and they refer complex cases including deep lesions to me, I’ve never had a deep margin elevation where the filling does not feel bonded, unless the isolation failed, and then I fix it and try again, And my older fillings have to be drilled out all the way they don’t fall out when the enamel part is gone. But my primary focus when bonding is the enamel because it always work, on this I agree. There is a lot of home grown ideas here, but always funny to hear what other people think. I always say do what works in your hands.

28:47 stating that C-factor is called so because the "inventor" was named Carel that made my day😂😂😂😂 There is even a Wikipedia article on this 😅😅😅😅 it’s coming from ‘configuration’, from cavity configuration…. 😅😂😅😂😅😂

I’ve always considered hygiene when doing DME, but I think I’ve wanted to do the DME and be successful at it more than taking the hygiene aspect into thought. I got my first radicular caries after a perfect endo / dme. I’m definitely starting to think about hygiene the more experienced I get. Love your podcast!

28:49 continued, he makes another interesting statement: 'Davidson studied flat onlays'... The group qround Davidson and Feilzer did not studied "onlays"..., they studied composite materials, and their behaviour in different experimental set-ups... After that he says that because there is a "infinite" C-factor between the tooth and the onlay, the composite will be full of cracks... That would be true only if the onlay would be attached firmly to something, but it's not... it can move in the direction of the tooth. This is actually a problem with inlay preps, where there are 2 opposite tooth walls (vestibular and lingual) and the restoration is locked between them, and it cannot move "freely" in one direction or the other... And this problem is actually mentioned in one of the articles of Davidson et al... But even then the weakest link will give up, and that is the thinner tooth wall or the bonding surface and the cohesive failure in the composite comes next... And that's why Dr Venuti is the greatest sophist dentist ever! 😂 (That being said, I learned a lot of things from him! I just not like his way of potting forward some arguments)

always appreciate a good debate and I thank you for these points - including the C factor issue - I was previously taught by David Gerdolle that C factor was more favourable for a flat onlay, so I was surprised to hear Pasquale say about the infinite C factor - I did not think to raise this during the interview (2 years ago from today!) but the missing piece of information for me is that YES it would be true if the onlay was attached to something else. Thank you for this perspective. Part 2 is very good will upload it tomorrow Doc

What is the advantage of GIC vs RMGIC for you Jaz?

@@kvgolfa GIC is less technique sensitive in instances where moisture control is not ideal :) Due to resin monomers in RMGIC, it is more easily contaminated by moisture. However, it is that much more mechanically sound and wear resistant than GIC. Cheers from Australia, and great work Jaz! Thanks for your timely interjections throughout the vid. most appreciated!

@@kvgolfa thanks @jamespaik5018 - when I replied above, I forgot that it was in the context of dental cements. I am not sure of which cements are RMGIC based but yes, I do use GIC cements such as Fuji + due to moisture control issue James mentioned plus pure chemical bonding. When it comes to restorations eg Class V - if I was picking between GIC and RMGIC I would go for RMGIC superior mechanics in that instance
