Really liked this episode. It feels like there's such a gap in KZbin and other non-research paper/medical media relating to psychodynamic therapy/theory, personality disorders and neurosis and psychosis. I'm a layperson but this helped click a lot. Thanks for the insights! Also I like the back-and-forth, devil's advocate, and hypothetical scenarios like "if held at ***point" (censoring in case KZbin would delete comment)

Of course there is a gap -- because psychoanalytic theories are untestable.

From the view of the patient, unless the patient is a psychologist, it doesn't matter what it is named. I was a BPD type that was relieved to put a name to my behavior. It gave me hope and strength because I can do something about it. Although I understand every patient is different.

Thanks for the interesting conversation. Whatever I learn about BPD seems to always lead to more questions than answers. What stood out in this conversation was Dr. Ruffalo saying that he believes that bpd is a more psychotic variant of narcissism, while also seemingly acknowledging the more 'quiet' subtype that buck that trend. This stands in contrast to Nancy Mcwilliams' belief that the BPD of the DSM takes most of its qualities from hysteric personalities. I don't have the answers, but I find it quite striking how clinicians have such varied views.

It is interesting to compare different thinkers' conceptualization of BPD. I think Nancy McWilliams' point was that the subtype that the DSM included came from Dr. Gunderson's hospital work with BPD, where he saw more of a histrionic/hysteric patient, but she herself does not characterize BPD in this way, necessarily. I appreciated hearing about Ruffalo's description. It would be neat to hear different experts' conceptualization of BPD lined up next to each other in one episode.

​@@michaelwalker6775 Yeah, I definitely agree with you! I'm pretty new into psychology deep dives so it would be really helpful to see an outline of different models for BPD. That makes sense what you've said about Nancy Mcwilliams' view. From what I've read Elinor Greenberg's description was the one that resonated the most with me the most, perhaps because her language is very accomidating or that I have a lot more experience with the more 'quiet' BPD. But I also found her writing to be deeply empathetic for the interior world of someone with BPD - similar to McWilliams' Diagnostic Manual. If you or anyone else has any suggested reading in the same vein I'd be greatly appreciative.

I listened to the Apple podcast but just looked at this and saw the neat presentation-format, appreciate it! I gained so much from this episode, but what struck out to me the most was Dr. Fu thanking Dr. Ruffalo for coming onto the podcast, implying to me that he likes doing the podcast (taking part ownership) and will keep at it, woo hoo! Excellent interview and excellent guest as always, thank you guys both.

I discerned from three and a half years of reading and online lectures that CPTSD is another name or euphemism for Borderline personality disorder. It is good to hear that spoken aloud here so that I can continue my assessment of my own relationship experience in alignment with the research to make the best assessment going forward. If it roars it’s a lion. If i accept the roar I’m now a head-in-the sand ostrich. Choosing a healthy relationship means the pppl with bpd/cptsd need to be in voluntary treatment or we cannot be in a healthy relationship with them. We must be in treatment if we continue to expect a different outcome from them without treatment. (5:50)

Financial dependency leads to emotional dependency. Trauma based fear of abandonment. Repeated with sudden and severe consequences.

Im diagnosed OCD, Bi polar1, Quiet borderline. If i were to self diagnose, I'd say I'm a skitzotypal PD type. I have never opened up to a mental health professional about the inner cognition of my internal world. I know better than that ! I slide straight past all of that. No early mentions of conversations with God. I protect my inner sanctuary from the outside world. For the 1st 45 years, it was all a well-kept secret. The only thing that interested me was the internal dialogue. I facinated on all things i believed to be magical or spiritual. Synchronistic events were guiding me off course. i was placing myself in danger to share what i believed were spiritual truths. Believing there was no other thing in existence of greater importance. If i saw the No2 whilst mentioning the No2 id place meaning into it. I dont say that i no longer do this, but im no longer letting it control my perceptions. I lose track of time. Im in a daze (according to those observiing me) i dont recognise the daze. I feel that i still have spacial awareness when im "dazed". Im most comfortable on my own. Being around others takes to much energy. Ive noticed myself when i need to go into a public setting, any knid of question or comment im unprepared for gets a cringey kind of response. Kind of like... me walking away repeating what it was i just said that made absolutely no sense at all. I probably didnt even form a correct or complete word. Its embarrassing. However im obviously so detached that i laugh at my own ridiculousness. Im sure I'd appear to be socialised a lot better if i could write my responses as opposed to having to speak randomly at times IN PUBLIC to a stranger, its almost as if i develope an instant stutter. I walk away laughing to myself. At myself. Laughing at what on earth could so & so be thinking ?? This is why i just sit there .... at the Drs office...

How would presenting as NPD be considered “getting better”? That’s not better, it’s just trading one personality disorder for another.

I got better and ended up a lot more narcissistic, but its not the disorder itself...its just the ability to self-inflate as a defence against turning inwards on ourselves like borderlines do.

I’ve been diagnosed with both. Which is a complete contradiction. I think the psychologist that diagnosed me with BPD doesn’t know what she’s doing. She was sloppy in her assessment. Where the psychiatrist that diagnosed me with cPTSD is by far thorough and in a methodical approach. These assessments need to be conducted by a professional that has more clinical experience.

I was married to one for two decades without knowing what bpd was. Unfortunately for me and my two sons she had family enablers who were very wealthy. When her lawyers were finished I was falsely accused of violence and abuse and haven’t seen my two sons for 5 years now because of her illness and me not knowing how to deal with it. My children are victims of her zero empathy. I believe Brad Pit went through similar. Good men who get involved with an evil masked woman with no capacity for empathy and bent on revenge. Coming from cluster b parents myself I can clearly see why I was dumb enough to marry someone who is npd bpd. My poor boys need to know about these things before they make similar mistakes or choices. Unfortunately they won’t talk to me because they’re convinced I left them because I don’t love them or fight for them. Toxic disgusting mother.

PTSD is not psychotic. It's a reasonable response from your body after trauma in order to keep itself safe and alive

You’re oversimplifying the neurobiological complexity of PTSD. Psychotic symptoms can and do arise in PTSD, often as a result of extreme stress and dysregulation in neural circuits. Pathologies like PTSD should not be framed solely through a lens of stigma reduction, as this can distort our understanding of their biological underpinnings and the profound suffering they entail. By minimizing the reality of these disorders, we risk doing more harm than good to those who need comprehensive care and accurate representation. Also, PTSD symptoms aren’t “normal.” They’re typical for the disorder. Nothing that makes you believe your life is in extreme danger when it isn’t is “normal.”

@@anthonyrispo1229 You could define some of the symptoms as psychosis but we all disconnect from reality at times but no one is calling us psychotic. Part of differentiating illness is looking at the cause. PTSD is a protective response so as to never repeat intense trauma. Disassociation, paranoia, hypervigelenlce, etc are 100% normal ways of coping with trauma, whereas, a schizophrenic person is abnormal and so is classified differently.

The manipulators discussing the manipulations of the other manipulators sounds like these smug, smirking and chuckling dudes find this all to be a big game that they hold the key to. Does therapy work? Not really. Why not? Well it's the patients fault, naturally. Not the therapist.

Most personality disorders can only be improved with behavioral therapy. It’s not a chemical imbalance. I think it’s sad that mental illness is only associated w/pathological aspects for which medication is the cure. It is actually the opposite.

Furthermore, if medicating a person is the only solution to treating mental illness it takes away any personal responsibility for the person’s action. It’s kinda like saying “The devil made me do it.”

@ Personality disorders boil down to maladaptive behaviors. It’s definitely not easy and takes a lot of work and personal development. And some people will never be reached but there is hope.
