So my HH agency came up with new policy that medicare is not allowing any abbreviations in notes anymore & all notes has to be in laymen language for patient to understand if requests copy of his medical records at any time, & that HH agencies that don’t follow, will be cited during audits! Any resources that negates the above?? Thanks I.A.

this lowercase pt meaning patient has no official source. No medical dictionary has this in it nor does APTA endorsed it. I wonder if this is only "common" in the US as I am from EU country and I never heard anyone saying pt for patient.
