@MarkShinnick 5 mg once per week. I wish it was 6 mg but I only get 60 1mg pills every 12 weeks. I'm thinking of going 6 mg for 10 weeks and off cycle for 2 weeks. Or I can 6 one week, then 4 plus gfj the next week, and so on.

Thanks Matt for a detailed analysis of the research. What is your opinions on Calcium AKG which also appears to reduce Mtor activity but not to the same degree as Rapamycin

What is the easiest way to get 6 mg rapa for off label use? I doubt I can get it from Kaiser. I am 83. A vegan.

Do you think rapamycin might also improve immune systems that are dysfunctional for reasons other than age (such as vaccine injury or autoimmune disorders) in a manner similar to how it affects the aged immune system? As an investigator/researcher, but not a scientist or medical professional, I find the workings of the immune system and all its different components entirely baffling. It would be great if you can shed some light on these things.

I am curious about the dose that were given to these mice. I was familiar with the study but didn’t know that the dose was 4 mg per kg of weight. That seems to me to be an enormous dose. This seems to be even more than that used in transplant patients as an immune suppressant. My understanding is that in your dog aging project study that you are giving dogs 0.1 - 0.15 mg per kg. That is so much lower. Maybe i am missing something but can you speak to this?

That is very informative. For another 20 to 30 years, I believe there is more information related to Rapamycin for longevity treatment.

I've often heard in such studies that dose is dependent on weight. I have not heard this for humans when prescribing. At least when I've been prescribed stuff.

Hey Matt - more a question for Hevolution but would appreciate your opinion on this. If as a field we truly believe in the geroscience hypothesis, that aging biology is conserved - and indeed the field's foundations are built upon the idea of "conserved mechanisms of aging" - why is there little push for serious RCTs testing rapamycin? The reality is that an off-label drug will only be funded by 1) Non-profits or 2) Govt, and despite your and A4LI's efforts the latter won't be of much help any time soon. Nir Barzilai already showed us that you can design a 'longevity' trial without exorbitant sample sizes and sufficient power, due to event rates in an older population being high when considered as a panel of multimorbidity endpoints. After all, if rapamycin fails in a TAME-like RCT, what else do we have - in so far as we believe that robust preclinical data for longevity would be predictive of human outcomes? If rapamycin is unable to produce tangible clinical outcomes in humans then we ought to know sooner rather than later. While the geroscience hypothesis is far more a theory than a hypothesis in animals, in humans it very much is still a 'hypothesis'. Questioning and testing the geroscience hypothesis is arguably our most important obligation as geroscientists, especially when we have yet to prove our 'worth' to society. The obvious asymmetric upside is - if rapamycin is shown to slow human aging, the status of geroscience changes forever, no doubt about it. Ironically, it feels that Hevolution leadership is aging, conservative, and unwilling to take calculated risks to move the field forward in a big way. I think that instead of just trying to convince people that we deserve more funding (which ofc, is necessary and important), it's also time for the field to put some skin in the game and show that we truly believe in the geroscience hypothesis.

What's the relationship of that Zheng paper, do you think, to the old wives' tale that you should feed a cold, but starve a fever (e.g. flu)... which--she didn't say--would increase autophagy and suppress mTOR signaling?

Dear Sir, I am 47 year old man, for 12 years I took prednisone and azathioprine because of autoimmune hepatitis. Now for fifteen years I have not taken them because I have achieved remission. My ALT and AST fluctuate between 20 and 30. I would like to take rapamycin for so-called "longevity". Have you had such a clinical case of autoimmune hepatitis and taking rapamycin for longevity?. I do not take any medication, except for vitamin D3. Can I aggravate the disease, or is it possible or regulate my immune system to decrease my ALT and AST below 20 ? Thank you very much in advance. My 10 year old Saint Bernard (senior) received an immunotherapy (CAR-T) treatment for osteosarcoma 9 months ago. I am currently giving her 0.1 mgs of RAPA per kg of weight (6 mgs) every 4 days with the hope that it will help her immune system keep fighting the cancer. It’s a bit of bio-hacking but at her age, and with her diagnosis, I’m not sure that i have much to lose by trying. I’d be curious if you have any thoughts on this. I also understand if you do not.

GlyNAC looks to boost glutathione (antioxidant) levels. While glutathione might be involved in mTOR signaling it's not a direct inhibitor.

Was it immune boosting or immune suppression? If the effect was to suppress cytokine storms then the benefit might have been immune suppressive.
