It's always important to take the negative with the positive when it comes to criticism and I think you handled both very well. In my own work, which is absolutely not a hugely important thing - Records and Information Governance - I am often having to accept wildly opposite accounts of myself. One day, I am given a glowing review, thanked, offered words like 'wonderful' and 'awesome,' but on another day, if I missed something or made a mistake, I can feel the disappointment through the Teams call. Striking that balance between the two is difficult but necessary.

In my opinion, the reason why some of the patients may have felt rushed is due to the way to speak; you speak very elequently with relatively no pauses (ums and erms). You know exactly what the next words you are going to say are and speak at a slightly faster rate (not like that streamer XQC, maybe more like 1.05 to 1.1 x speed 😂). Also if you have been wearing facemasks in your workplace I find from experience that it is much harder to read a person's expression and therefore intention, so maybe some of them misinterpreted your expression as boredom or something

I understood your perspective until the end, suggesting patients having a rating could help the provider know if they're a "nightmare patient". I kind of follow why you might think it helpful, but I disagree that it's the same type of service relationship as a customer/restaurant or passenger/driver, and here's why: generally, people are seeing a doctor because they are at a point where they feel they need to. They are likely suffering from an issue that is causing them some level of distress, and may have been dealing with it for enough time to feel desperate and perhaps scared. Many people also have to take time off work, or maybe arrange childcare, etc. that can cost them time and money, and have likely waited months for an appointment. This results in the patient already starting at a place of vulnerability. For dermatology issues, I think this is especially true due to the outward nature of symptoms that can cause significant physical, psychological and self-esteem issues. Some medical providers are not as considerate or friendly as you, and will truly have poor bedside manner, act visibly rushed, or make the patient feel belittled. Patients can also get "bounced around" between doctors/specialists, and are generally not informed of expectations beforehand (i.e. how long the appointment typically lasts, scope of care/what information is relevant for the provider). All of this can be a recipe for a patient either coming across as a "nightmare patient" to a provider (or set of providers), or walking away frustrated and then leaving a bad review on a site that doesn't require the patient to be specific or even vaguely informative. Instead, what I think could help doctors best is if review sites: 1. create a completely separate rating between the DOCTOR and the OFFICE - fact is, sometimes a doctor's staff does not act in ways that reflect well on the doctor or their service. As a patient, I would want to see the doctor's rating separate from the office/staff, and I would want to leave separate reviews, as well. I don't think it's fair for me to rate the doctor's wait time or availability poorly when I've seen the staff chatting away and not answering phones, as an example. 2. Require a WHY to the ratings/categories for example. If the review is just "this doctor sucks", I think it should be disallowed for being unhelpful to both the doctor and potential patient. It's more helpful to see "doctor made me feel rushed BECAUSE they (gave one word answers, stood/paced around, talked quickly, dismissed my questions, etc.)" than just a 1-star review or vague complaint.

Giving very Eileen episode with her file being shared on Seinfeld 😂 but agree - communication and experiences are a two-way street! I don’t think anyone in customer service should have to be particularly friendly to people who are being very rude for no reason (I mean, I was anyway but think it gave me a warped sense of healthy boundaries for a while in my personal life…)

What daily moisturizer do you recommend for combination type skin? I have many small black heads on my nose (even though I use a physical exfoliator and sometimes manually remove them by squeezing my nose's blackheads). I get acne on my cheeks, chin and forehead, upper back. Where I have acne it is either red (Post-Inflammatory Erythema) or white (fat I think) at the tip of the acne bubble. I am a 16, my skin is very elastic and with great color where I do not have acne. My father never had acne and never uses anything on his skin and has impeccable skin. But my mother had severe acne when young and had acne until her 40's. I sometimes have the tendency to pop until I see blood, I use a cleanser and oil after popping and 3 days later I have a scar but no acne where I popped. My skin heals very fast but it gets acne very easily, I have many small scars that slowly fade away on my cheeks from where I popped in the past. I am currently using a cleanser, physical exfoliator and oil. My acne is always dry and I accidentally pop while paying attention to something and then gets infected with more acne around it, especially when I am not home to clean the wound right away, on my cheeks especially. I have no rough uneven skin, only acne with some (Post-Inflammatory Erythema) and healthy elastic pretty skin where I have no acne. What daily moisturizer do you recommend for combination type skin? :)

Hey, could you please make a video of if tretinoin causes liver damage and birth defects? I recently started using tretinoin because of all the hype. But Idk i read in a few place’s about these side effects that I didn’t see any dermatologist mention on their videos. I was hoping you can talk about it. Also, because a lot of people have been advertising tretinoin to the masses and it has become easy to get our hands on tretinoin through platforms like nurx, and stuff. So please make a video about it if there’s anything like that. Thank you. ❤

Could you please tell me, for man with beard (I don't have like a big thick beard), should we apply all skincare products over it or skip it? I always apply cleanser, moisturizer, retinol, everything over it but some people i see in videos skip it but what if I shave my beard? I want all my skin to look the same and healthy, I don't want to shave my beard and the skin underneath is different. I've been looking for an answer to this question and nobody talks about it besides on Reddit which I don't trust 100%.

Any comments on the masking mention in the beginning? It can make it very difficult for vulnerable patients or people with high risk family members (or anyone in the pandemic, now that we know reinfection can increase chance of long covid, and that heart issues soar) to go to medical appointments when masking is done half-heartedly in medical settings.

10 minutes is hardly enough time to evaluate anything. Diet, lifestyle, and environment has alot to do with skin health and problems and that takes time to discuss. Go see a naturopath and they will spend 30 to 60 minutes and charge less money and treat the whole person.

Hi Dr. S..please dont think me rude, but if you came in to see me and only stayed 10 mins I would never return to see you, especially if it was my first appt! Patients wait months to see a Dr and 10 mins isnt enough time for you to get to know them and their concerns. I must be able to think my Dr is concerned with my health and make a connection with them. I know you guys are extremely busy and time is money. The best reviews are from word of mouth that come from patients that feel their Dr truly is concerned about them!

No offence taken! I totally appreciate some patients feel that way, but honestly, with NYC dermatology I’d be surprised if there’s anyone able to guarantee more than 10 minutes per appointment. Definitely some options still out there, so it’s best to be transparent about what is feasible!
