Y'need to remember, they aren't residents. It's a teaching hospital, but they are all qualified doctors. Cameron is an Immunologist, Chase is a Surgeon, and Foreman is a Neurologist. As Wilson says to him in the Pilot, "you've got three overqualified doctors working for you."

Doctor Mike, in case no one ever told you. His team aren't RESIDENTS, they are board certified in their fields and they are in FELLOWSHIP programs at the hospital. They applied for it and House Picked them. One is a Neurosurgeon. One is an ER General Surgeon. One is a Immunologist. House is Board Certified in Nephrology and Infectious Disease and his is running an experimental department of Diagnostic Medicine at Plainsborough Teaching Hospital.

Med students go there to gather practical experience on patients, which is often much more valuable than the book knowledge they'll get in medical school. Students often reside there and work under the command of board-certified doctors (sort of like interns but actually useful).

As a sonographer seeing ultrasounds in tv shows can be sooo painful. He said “look at the right atrium” and pointed to the left atrium.😭 there also wasn’t even a mass in the heart

lol I'm not even in the medical field and I noticed that too. Probably because of the short lecture I got from a cardiologist showing my pulmonary vein (in the _left_ atrium) and the role that area played in the problem I was having. And yeah, I'm looking at this and I'm saying "that's the LEFT one!" (btw my heart is very happy and healthy these days) (thanks to a good doc!)

@@TheAtheist92 Uh, no. That's not the left ventricle, it's the left atrium. Just look at which direction the valves are coapting (toward the apex, now toward the bottom of the monitor), the pulmonary veins visualized near the bottom of the left atrium, the obvious descending thoracic aorta at the bottom right, etc. You're right, pediatric echos ARE typically oriented upside down on the screen, but here they showed a right-side up cine loop. Also, the apical window is the wrong view to rule out Kawasaki's, they should have looked in short axis for dilated coronary arteries! And if you look closely at 08:10, you can kinda see that they are looking at what could possibly be a small mass in the right atrium on the septum itself.

I actually got crohn's disease diagnosed when I was 3 years old because they did a colonoscopy because of bloody diapers. My parents noticed that there was blood, rushed my to the hospital, they said it was food poisoning, rushed me to another hospital where the colonoscopy was done. They actually diagnosed a serious condition because my parents went full House md. mode lol

House jumping to crazy conclusions is mostly because he's designed to be Sherlock Holmes but as a doctor ( He even has his BFF Wilson/Watson and lives at 221B Baker Street ). If you've watched BBC Sherlock or read the books, Sherlock does some absolutely insane deductions and has random knowledge such as Snake bites and obcure details about venoms and plants and where they grow and all that, just like House. You're really meant to just roll with it.

Yes but, if you've read Sherlock Holmes, a lot of those deductions only seemed random and insane because you, the reader, weren't told certain information until the end of the case that Sherlock noticed at some point previously in the story, or just randomly knew. At least with House we're not kept out of the loop until the case is solved. I *hated* that about Sherlock Holmes stories.

2:49 when I was in highschool health class our school resource officer came in to give us the whole talk about alcohol and the symptoms of high blood alcohol content. One of the symptoms is a nystagmus, so he called on a random kid to come up to the front so he could do the whole "keep your eyes on the tip of this pen" thing. By chance, he close the only kid in class that was born with a nystagmus.

More than likely that was an editing/prop slip up. It happens all the time, especially in medical shows. The prop department will flip a scan or film on accident, then it's fixed in the next scene/shot.

One of the big things with ECD is that not only is it a 1/1,000,000 disease (which tends to affect men more than women) but diagnosing it is incredibly difficult and the symptoms can appear at different intervals and intensities between different individuals. Most people with the condition don't automatically fit the description and by the time doctors are able to put two-and-two together, most patients will have died from the condition since there is also no standard treatment to suppress it. Furthermore, while on paper its "slow growing", the severity of the condition is wildly different between individuals. Some patients die within 32 months, others die within days and new research has shown that patients may not even have symptoms which further complicates the diagnosis and results in them dying faster. According to reports, roughly 60% of patients suffering from ECD ultimately die from the condition. Those that do survive the condition essentially have to live the rest of their lives going through harsh treatments to stay alive since there is no cure. So the sad realty is that while they found out what was killing the boy by the end of the episode, the boy is going to live the rest of his life receiving treatments to suppress the condition, and there is a high likelihood that he won't reach adulthood since the survival rate also varies based on the individual and the severity of the condition.

The way House's team works is that he chooses cases. Then has his team do all the procedures because he doesn't trust Nurses or other doctors outside of his team. Except for Wilson or Cuddy in cases of Cancer or special patients. Also Foreman is a Neurosurgeon, Chase is a IC specialist and surgeon. Cameron is an Immunologist. Later on in the show house's team changes and he also has a Plastic Surgeon, a Specialist in Radiology, specialist in sports medicine, and some interns. The Diagnostics department is funded by the hospital to treat the most special cases, that's why house is always going after zebras instead of horses. As he'd say: if it was easy, those other idiots could have diagnosed this patient.

3:30 the way you gotta imagine it is that house always jumps to crazy conclusions, but 95% of the time they don't pan out. those instances aren't shown in the show though because they're not interesting, so we as the viewer get led to believe that he has some sort of spidey sense. Erdheim-Chester Disease (ECD) is a rare non-Langerhans cell histiocytosis, which means that it is a condition where certain types of immune cells, called histiocytes, become abnormal and accumulate in various organs of the body. The exact cause of ECD is not fully understood, but it is believed to be related to genetic mutations that lead to the abnormal growth and accumulation of histiocytes. The exact cause is unknown, but it is believed to be caused by genetic mutations that affect the development and differentiation of histiocytes. Some studies have linked ECD to mutations in the BRAF and MAP2K1 genes, which play a role in cell growth and division. However, not all cases of ECD have been found to have these specific mutations. ECD can affect multiple organs of the body, including bones, skin, lungs, kidneys, and brain. The symptoms of ECD can vary depending on the organs and tissues affected, but they commonly include bone pain, fatigue, weight loss, and neurological symptoms such as ataxia (lack of coordination), nystagmus (involuntary eye movement), and abnormal proprioception (loss of awareness of body position). Other symptoms may include skin rash, diabetes insipidus, and kidney dysfunction. The rapid progression of ECD is not common, but it can occur, its not unexpected. The disease can progress rapidly if there is a sudden and significant increase in the number of abnormal histiocytes in the body, leading to organ damage and failure. This can happen due to various reasons, such as an infection, inflammation, or an underlying condition that triggers the abnormal histiocyte growth. In Austen's case, the presence of a small mass at the base of the brain suggests that the disease might have started in the brain and then spread to other organs, leading to the rapid progression of the disease and also it was present at the heart which is also another critical organ leading to the rapid progression. The infiltrative nature of ECD histiocytes can cause significant tissue damage and dysfunction, leading to rapid deterioration of organ function. In addition, ECD can lead to the formation of fibrous tissue, which can further damage affected organs and tissues. The diagnosis of ECD is often challenging as the disease can mimic other conditions, and the symptoms can vary widely. ECD is a rare disease that affects the body's ability to produce and use certain types of white blood cells called histiocytes. Histiocytes are an important part of the immune system and help protect the body from infections and other harmful substances. In ECD, there is an abnormal overgrowth and accumulation of histiocytes in various tissues and organs throughout the body, including the bone marrow, lungs, heart, brain, and kidneys. This overgrowth and accumulation can cause tissue damage and dysfunction, leading to a range of symptoms and complications. One of the most common symptoms of ECD is bone pain, which can occur due to the infiltration of histiocytes into the bone marrow. Other symptoms can include fatigue, weight loss, fever, night sweats, and gastrointestinal problems.That was why Chase misdiagnosed it as a gastrointestinal problem. The diagnosis is usually based on a combination of clinical findings, imaging studies, and histological analysis of tissue samples. In Austen's case, the diagnosis of ECD was confirmed by the presence of CD68 positive cells in the biopsy sample of the heart, which is a marker of abnormal histiocytes. Treatment for ECD depends on the extent and severity of the disease and the organs involved. In some cases, no treatment may be required, while in others, aggressive treatment such as chemotherapy, immunotherapy, or radiation therapy may be necessary. Unfortunately, the prognosis for ECD is generally poor, and the disease can be fatal, as seen in the case of the 73-year-old woman mentioned by House. Early diagnosis and prompt treatment are essential in improving the outcome for patients with ECD. If you ask about how exactly the histiocytes cause damage tô the organs, Histiocytes are a type of immune cells that are responsible for phagocytosing and eliminating foreign substances and pathogens in the body. In addition to their role in immune defense, histiocytes are also involved in tissue homeostasis, inflammation, and wound healing. In Erdheim-Chester disease, histiocytes become overactive and begin to accumulate in different organs and tissues throughout the body. The excessive histiocyte accumulation causes damage to the affected organs by disrupting their normal structure and function. The exact mechanism of how histiocytes cause damage in ECD is not fully understood, but it is thought to involve several different pathways. One proposed mechanism is that the accumulation of histiocytes leads to the formation of nodules or masses, which can compress and damage surrounding tissues. Additionally, histiocytes can release pro-inflammatory cytokines and other signaling molecules, which can cause inflammation and tissue damage. In terms of the symptoms of ECD, ataxia (impaired coordination) is thought to occur due to the involvement of histiocytes in the brain and nervous system. It is believed that the accumulation of histiocytes in the cerebellum, a part of the brain responsible for motor coordination, disrupts the normal functioning of this region, leading to ataxia. The exact mechanism by which histiocytes cause neurological symptoms in ECD is not fully understood, but it is thought to involve inflammation and disruption of the normal cellular signaling pathways in the brain. Rob Berra exactly. A defibrillator is to shock an abnormal heartbeat back into rhythm. If you don’t have a heartbeat, a defibrillator isn’t going to help. However, once that wonderful person giving you chest compressions hopefully gets your heartbeat back, a defibrillator could get your heartbeat back to normal if it was too slow or fast.

I was diagnosed with a “slow growing” blood cancer yet had AML. Basically I had myelodysplastic syndrome, and by the time they found my AML I already had 15% of “blasts”. Sometimes a red flag for leukaemia is a patient who has 0 white blood cells. In my case, my white cell count was 40 which is why it took doctors ages to decide on giving me antibiotics or whether I should have a biopsy. Some people can have one blood test and be diagnosed with blood cancer whereas some people need weeks of their blood counts monitored. I had trilineage dysplasia and I know my case still baffles a lot of medical professionals. My MDS developed slowly in the beginning but once it reached a certain point it suddenly accelerated and I had to be YEETED into treatment. I had a pretty poor prognosis and tested high risk for relapse so I had no choice but to have a bone marrow transplant. I also feel like my age had something to do with my delay in diagnosis. I was 17 and both MDS and AML are considered “old mans cancers” so it’s not something a haematologist will actively seek in a 17 year old female.

I think Mike keeps missing the concept behind the team in House. First and foremost, House's team are NOT residents. Every single one of them is a certified doctor. House, of course, is a diagnostician, and before that a nephrologist, Chase is an intensivist and surgeon, Cameron is an immunologist, Foreman is a neurologist, and while not technically on his team, Wilson is an oncologist, and Cuddy was an endocrinologist. Second, the team's entire purpose is diagnostic medicine. The reason the diagnoses are like this in the series is because his team's specialty is rare and complex medical conditions. Many of the patients in the series come to House after going from clinic, to specialist, to other specialist, who've all run standard tests and procedures and weren't able to find a diagnosis. And I know Mike is a good and accomplished doctor, and House is not a real person, but the fact that Mike constantly questions how House would know something when he doesn't is a little bit annoying. The character is obviously supposed to be older than Mike is, and he's been in the field for way longer, and diagnostic medicine and knowing rare medical conditions are his entire job.

After listening to a squeaking driver's seat for a few days, one evening I pulled into my garage, noticed the squeak again, went to my workbench, picked up a can of silicon spray and applied it to the trunk weather gasket. To be fair sometimes diseases or conditions remiss for no reason whatsoever. Doc Schmidt has a short/video on this. There are many times in which patients with abnormal symptoms and severe conditions will inexplicably recover to perfect Health and there is nothing the hospital can do but let the patient go if there’s nothing wrong. This is particularly frustrating and concerning for people with extremely rare or under studied conditions like Erdheim-Chester or Tourette’s or extremely aggressive cancers. These things can appear for a few weeks and then inexplicably remiss at anytime. This can be frustrating not only for patients and doctors to come to a diagnosis but also can be emotionally frustrating for friends and family. I mean if I were to tell you your mom only has a few months to live and 3 weeks later her aggressive deadly cancer has completely remised, you’d probably be extremely upset and frustrated that you spent 3 weeks preparing to morn a loss just to find out she’s fine now. Of course, this is House so of course this mom was probably just very emotional and unable to logically piece together the point in her anxiety and grief or might genuinely be that stupid. But in reality, she could be somewhat informed about these inexplicable recoveries and cling on to the hope that the older woman managed to recover, though inexplicably, but that her health care team have a generally good idea of why helps vs what doesn’t. However that wouldn’t make compelling television I suppose haha!

It is also spot on about the reason why House can even do this: Cuddy and Wilson enable him. In later episodes it becomes apparent that pretty much _no_ other doctor, or hospital, or department wants anything to do with House. They all consider him a dangerous lunatic, a loose cannon with zero respect for procedure. If it weren't for Cuddy, he would not be able to practice medicine at all. The remaining question is what black magic Cuddy conjures to smoothen out reports, because otherwise, some committee would have gotten wind of House's "colorful" approach, and consequently would have revoked his medical license a long time ago.

We use to watch House every week with med students ( I was an epidemiology student). MD and Epi students would meet at someone's apartment (10-15 of us), we'd cook a big meal bring some beer do a little studying but all of us would have to present a diagnosis we thought it was. Could consult books, internet, friends, etc. but our choice had to be in by the end of the second commercial break and we couldn't duplicate someone else's diagnosis. A super fun way to get some studying in during a time crunch and becoming familiar with both common and these super uncommon things there was on House and learn how to work that internet search. You never wanted to be the person who missed the random episode that had the simplest response. We were all almost always wrong. Highly recommend playing that game. mad

Hey Doctor Mike, I love your channel so much and House is one of my favourite shows, I definitely want to see more House reactions👌 Both being said, just wanted to let you know the reason the doctors under House run most of the labs and do tests with patients (and all the other eccentric things he asks of them) is because they’re in a very specific department specifically created by House for rare diagnostic problems other doctors can’t figure out. I believe usually the doctors he chooses for his team have to have some sort of background in crazy medical and personal lives.

This story was acclaimed for its accurate depiction of (this is going to be spelled wrong) Erdheim-Chester except for the timeline, which was compressed for dramatic reasons, and of course House's unclinical process. There was a real doctor who diagnosed a real case of Erdheim-Chester after only having heard of the disease from watching House, thinking it sounded vaguely similar to a disease that had him stumped, and looking up the legitimate details of the disease. And yes, the real doctor took many months to eliminate all the obvious other things it could be. It's a slow disease.

@@dumbass3843 I think you are being wilfully dense about the show. A lot of your criticisms are explained in the show. This show is a an adapatation of Sherlock Holmes but in a medical context. 1. Why is House so rude? Doctors are not supposed to behave that way. 1. Yes doctors are not supposed to be rude but who says House is a usual doctor who does everything the way things are supposed to be done. His character is written to be an arrogant mean jerk who is not interested in treating patients but solving complex mysteries. Who does that remind you of? Sherlock Holmes. 2. Why does House look for Zebras (or the most out there unlikely explanation) instead of a common sense very likely common explanation? 2. Because it is the premise of the show that House only gets those cases which are out of the norm. In the show, all the cases that House gets have already gone through multiple doctors. Which means all the likliest options have already been ruled out by all the previous doctors. So the likely answer might be common cold but guess what...common cold has already been ruled out by previous doctors. If all the likliest possibilities have already been ruled out, then whatever remains, no matter how improbable, must be the truth. Who does that remind u of? Sherlock Holmes. Didnt Sherlock also deal with unlikely murder mysteries that would, more often than not, never happen in real life... 3. Why does he break into people's houses? That is so inappropriate. 3. Because he is an inappropriate person according to unconventional morality. He doesnt respect peoples privacy (yes you should). He also believes that people are fundamentally liars (call me a cynic but that is true). He often finds out information which the patients wouldnt tell anyone out of sheer embarassment or maybe revealing that information would expose them to criminal liability. 4. Why does he know random facts? 4. He is a genius. He knows random facts for the same reason Sherlock knew random facts which most people wouldnt know. It is easy to criticise something without knowing proper context.

Hey @DoctorMike, fellow MD here :-) just wanted to add something - I know chest compressions are first line for any kind of CPR, but I think what they wanted to portray here is the so-called Three-Shock-Strategy. They didn't do it exactly as advised, but basically when a patient is already connected to a manual defibrillator when going into cardiac arrest, giving 3 consecutive shocks without chest compressions in-between is actually recommended by the ERC (european rescuscitation council). In this video of course he wasn't connected, and they used hard panels, so that wasn't right - but had he been connected, this procedure wouldn't actually have been so wrong! ;-)

JIll C that’s absolutely true, however, house jumps to conclusions when, as a doctor, you shouldn’t be treating what it could be, but what it is most likely to be. If someone goes into the doctor and their throat hurts and has white spots, a normal doctor will treat for strep most likely. Not assume it’s a rare disease that just happens so extremely rarely. That causes too much stress for the patient and it’s also silly to play to the unlikely odds before you even consider the likely odds. He doesn’t follow a logical, linear process of elimination.

But again, if you pay attention houses epiphanies usually end by saying "Chase, Forman or Cameron was right from the start", usually saying they're idea was dumb "At the time". But just watch it and now that I've said it, you'll notice. Also chase didn't take over houses practice because he's cool. lol

im an ASCP board certified phlebotomist, and im starting radiology school in 22 days (i was in nursing and switched over), it BUGS me so bad wen they make it seem like the doctors draw all the blood specimens, do all the lab testing, and all the radiology procedures...i feel you mike

They give local anaesthetic so the kid wouldn't actually feel it. Most kids scream at vaccines because needles are scary and they haven't developed communication skills to explain their feelings in any other way than screaming yet. Totally plausible in both cases.

@@mydogeatspuke don't forget that vaccines are more painfull than most other injected medicine (there are lots of metals in vaccines i fi remember correct. im ight be wrong so don't quote me on this)

@@RussianJan1tor I mean pain is pretty subjective. Some people find tiny needles incredibly painful, some have trouble with larger needles, and some aren't bothered at all. This includes infants. Some injections do burn or go deeper into the muscle and leave longer lasting discomfort but it's all down to the individual.
