I am new grad working at Nursing Home, They are charting Progress notes using Matrixcare software. I couldn't figure out what form they are using such as SOAP or SBAR. Finally, I figure out by watching your video. It is F-Dar format.

hey Sarah, What does Patient oriented x 3 means? I am a little bit confuse

+Jose Mercado Great question. It means that the patient knows who they are (can state their name), where their at (location), time and event (the year and who is the current president etc.) Sometimes it is referred to as oriented x 4....depending on how your were taught. I hope that helps!

At my nursing school they only have "DAR" would the F be included the D ?

Some nursing schools teach it differently. The F stands for focus...which is the purpose of why you are writing the note. You could include it in the data if you felt it was necessary. Thanks for watching the video :)
