Great video. I am myself an ophthalmologist, with over 20 years of experience. But I started retinal lasering recently. And I churned the internet for some doubts I had but in vain. Your video cleared those points. The eye piece setting didn't work on zero. I had to dial it to minus 5 at times to get good view. It baffled me before watching this video! I had no idea about what any of this was? I had two pretty large tears. One bigger than the other. I went to Moorfeild eye hospital and had laser treatment. The pain for me was like a Cluster headache behind each eye. But knowing that procedure would help me keep my sight it’s worth going through this pain, because it will eventually stop. I think being ignorant to what this whole experience was, helped… I am very grateful for the skill. The doctor had on doing my laser…

hi. thanks for your comment. the curtain is most likely from the blood in the vitreous. it will take weeks to be washed out. however, if it changes or gets bigger: go back to your doctor. the flashes are coming from the vitreous strand physically pulling on the retina. the laser doesn't have any influence on that. it only prevents that pulling-motion from causing a bigger tear

Just had this done today. The Dr says it shouldn’t hurt but after a while I found i felt the laser, caused a very mild dull ache. Afterwards, eye was sore, felt like a headache.

Anyone watching this, how to tell you may have this issue, you can see floaters in your vision, they look like frizzy hair or black dots, "in my case". Do not ignore it, get checked, because the brain learns to ignore them, until you bring awareness to it. When you go for your eye test, they can do additional eye health tests, where they check the back of your eye, which is highly recommended. They cannot see anything with just a sight test.

Can the surgery cause damage to nerves in the eye? I have had this surgery a year ago (for a big tear) and since then, I have photophobia. The ophtalmologist did not see anything wrong in my eye but the light sensitivity started a week after my surgery and has caused terrible migraines. I cannot go out without my sunglasses on and I cannot drive at night. I cannot use computers and halogen lights cause terrible pain. I want to find answers to this mysterious pain I am in.

that is the hardest part: a) just shoot in the fog: usually you can get some shots in if the patient looks in the direction of the teat, you tilt the lens and shoot without a clear image of the retina b) indent the sclera c) try to get as close to the ora as possible with a wide strip of spots d) do some cryo

I had this done for a retinal tear back in the early days of laser. More than 30 years ago now. The repair didn't negatively affect my vision at all, thankfully. (I had a horseshoe tear on the periphery of the retina, and it's still holding just fine.) However, I've gotta undergo cataract surgery soon and am so worried about my thin retina detaching or tearing again. I hate to press my luck

You have to do your cataract surgery at some point anyways. Yes, there is an increased risk of retinal detachment after the cataract surgery. But it's fairly low if your tear has been stable for 30 years. I wouldn't worry too much about the surgery. Just make sure they check up on your retina (dilated fundus exam) 1 week, 1 month and 3 month after surgery

I done this laser surgery for my small retina detachment. My detached retina did not progress for quite sometime like abt around few months i think so my dr say try laser first. I hope it can stop my retina detachement from progressing. I have a question about what you are saying at 4:30. I've never heard about that solution before (adjsting the eye pieces to compensate for accomodation), but I struggle with finding the rigth focus during retinal laser treatment. I might just be tired at the moment but I don't really understand why it would be beneficial. Could you please explain the logic/physiology behind it?

the machine is calibrated so that the slitlamp and the laser have the same focus for an emetropic, non-accommodated doctor. young doctors accommodate a lot in front of these machines, during accommodation the refractive power of your lens increases. this means you make yourself "artificially myope" to correct that you need to put a minus lens in front of the eye. otherwise you have 1 of 2 situations: the laser is working but the retinalimage is blurry for you or the retinal image is blurry but the laser is only working with super high energy because it's not focused on the retina.

I have a question. I had lasik surgery 3 years ago and a month ago I was diagnosed with lattice degeneration. If in the future my lattice degeneration progress into retinal break/tear, does this laser treatment affect my lasik flap or do I have to get my flap lifted before getting the laser?

Hi. Short answer: No. Long answer: The only thing that those 2 topics have in common is your eye and the word laser. It is a completely different type of laser (on the retina we burn whereas on during the LASIK they cut and dissolved) and (at least for an ophthalmologist) the two parts if the eye are as far apart as it gets. The only way I have to interact with your cornea to do the laser is when I put the lens on it. Hope this helps

don't have to avoid weight training at all. ....what you need to avoid is fast movement of the eye: reading, looking out of the side window of a car/train, anything that causes vibrations (biking, powerplate, ....) I tell my patients not to do those things for 3 weeks

definitely don't refreain from workouts. has nothing to do with it. it is debatable whether precautionary laser in lattice is a good idea. i would probably not and just get regular checkups. however, for medical advice you need to ask an eye doctor who has examined your retina.

hi...probably depends on the country and hospital. there are some tricks you need to know but all in all it's a pretty simple skill. in our clinic the residents learn it in the second year. of all the surgeries we do, this is definitely one of the most simple ones. you do need some pracitce tho...

the laser is done to keep the holes from getting bigger or turning into retinal detachment. it does not treat any of the symptoms like blurryness, black spots or flashes. These are caused by the vitreous and will likely fade away over time.

go for the laser. you can't see the scar yourself. you want to avoid a retinal detachment because that you are definitely going to notice

PLEASE REPLY🙏. I have only one eye. I have floaters and I have now holes in retina. I am really worried about doing this! How is it going to affect and is there any risk? After Laser will be something that can make my vision bad?

The risk of losing vision due to a retinal detachment is much bigger so I would definitely do it based on the limited information i have

I also got this laser surgery for retina in my left eye but I forgot to tell my doctor that I have dry eyes so i’m very concerned right now because my left eye kinda hurts and feels dry and also please let me know if it’s alright to get laser surgery with dry eyes or else i will keep regretting this.

I had laser barrage for holes in my retina and my right eye had two procedures , no problems. The left eye about two weeks after laser has a shimmering spot. Scintillating Scotoma. Please advise, I’m having trouble coping with this along with hundreds of floaters

@@bdj1126 If that is the case, I wouldn't worry about it too much. If a trained ophthalmologist has done a dilated fundus exam, looked at the peripheral and central retina an not found any changes, it will most likely go away in a couple of weeks. The floaters can be super annoying but are harmless and you won't notice them anymore, once you give your brain the chance to learn that they are not dangerous. the flickering light can have multiple reasons, all of which your ophthalmologist would be able to see.

I had Smile relex surgery 3 years ago. I recently got told I had 3 holes and weak spots in my right eye and 2 holes and a couple of weak spots in my left. so I got retina laser surgery and my right eye can’t seem to focus when I read things close up. Is that a permanent problem?

this has nothing to do with the laser. as we get older we lose the ability to read things up close. it's such a slow process that most people don't realize it until they have a reason to test it. so very often it is something like this that makes us think about our eyes and we start testing their function and realize: it can't accommodate as well as it used to

@@DShun35 The side effect of the retina laser can be some dryness. That wears off after a few days. Not seeing clearly up close is not an effect of the laser and will not go away.

Hi thanks for the video! I have a question, since the retina is at the back of the eye, is the laser going to cause any harm to the cornea or to any front part of the eye, since it is going through to reach the retina? Or is the damage reduced due to a different absorption coefficient of the cornea? Thanks :)

good question. it doesn't seem to do that. and here's why: the laser energy is highly focused. you'll see that when you try to perform your first retinal laser. just a couple of microns out of focus and nothing happens. so, you'd have to be very bad to be 22 millimeters off. and hit the cornea

@@learnabouteyes thanks for the reply! how dangerous is it if the focus is not set properly? Say if it is aimed too deep beyond the retina. Will the patient suffer any eye problems? What is your usual ritual to obtain the length of the eye? Do you do an ultrasound A-scan?

The reality is the RS will lie to you and tell you no, but that’s not true. For example if a eyelash or dark matter such as a small piece of mascara gets under the contact lens used by the RS the laser will focus on that and destroy your cornea - they won’t warn you about that or other life threatening complications from the retinal laser

I had this last week for lattice degeneration , i also have a few black floaters occasionally and have had the neon orange string that I see at night a few times since surgery, a week ago. ( i see it less x than before ) I had experienced the Neon orange string sighting for a bout a month prior and thought it was weird. It to me wasnt what I would describe as a flash. It was more the appearance in the dark of a neon thread in a sort of horse shoe or circular image for a few seconds. Sharing this for anyone who also was wondering what the Flash is.( isnt) . QUESTION Learn about eyes my question why did the laser hurt less in some firings than others?? Is it the specific area he fired onto is more sensitive? or the power of the laser was turned up/down .. ? It got quite painful, ( I don't know why I wasn't given a real painkiller?? Also my vision has degraded in this eye prior to laser dimmer , blurry in spots, I am really worried and going for a second opinion.

hi, power can be turned up and down but most often the differences in pain from shot to shot are based on how close the area is to the ciliar nerves. unfortunately there is no way around it. it is pretty uncommon to give patients pain killers for retinal lasers, but some people are more senstitive to that type of pain and take some paracetamol 1 hour before the laser.

@@zenmaiden1 and thanks to the doctor: when I got my extensive retinal surgery done by the USAF by an Air Force surgeon I knew it was unique but as I am looking at cataract surgery I am looking around about laser surgery since my eyes are fragile. HE USED BELLADONA DROPS for both surgeries which were extensive. I had NO PAIN and he completely reattached the retinas separate years.

Do opthalmologists actually know how this feels to the patients? I doubt it. Its the most stressful and incredibly painful thing I have ever experienced . Also the opthalmologist wiped the lens on his pants before he applied it to my eye and then got angry when I remarked on the cleanliness of doing that. I will never go back.

Thanks for the video! I had two small (1mm) tears repaired four days ago and still see occasional flashes, especially at night (dark room). Is this normal? Will this go away? Or when does this point to there being another tear?

the flashes come from the vitreous pulling on the retina. the laser doesn't change that - it only prevents retinal tears from getting bigger. eventually the flashes stop, when then whole vitreous has detached. can take weeks-months. in this time regular followups with dilated eye exams are required. new tears often go along with additional symptoms like more floaters

the laser isn't causing any floaters. it's the underlying disease (posterior vitreous detachment) that is causing you the floaters. they won't go away but they are also harmless as long as you don't have any more tears in your retina.

@@learnabouteyes but before the laser operation I didn't see this block of floaters(7 or 8 circles), I just started to see them after the laser photocoagulation. if the doctor did mistake in the setting of laser device(energy and spot size), could it lead to floaters? for example when the energy stronger than 400 mw, will it burn the vessels and then these burnt vessels go into the vitreous ??? Could this be the reason? Or if the laser beam goes into the hole, not around the hole, could this lead to problem? I need please answer, thank you.
