Thank you for your great content, Dr. Rami! :) Where would you start the process of cognitive restructuring when it feels like there's a constant flow of negative thoughts in certain situations?

The first step would always be thought awareness. So writing down or thinking about all of the constant flow of negative thoughts - if there are a lot of negative thoughts, writing them down can actually work to slow down that process and allow a person to think more clearly about what they're thinking. Hope that makes sense.

Where do you go from thought awareness then? It feels like it's easy to get stuck on awareness itself. Challenging the thoughts on paper or in mind repeatingly until they're slowly substituted by more realistic thinking?

That's exactly how to do it. Thought challenging is initially done using thought records to slow down the thinking process and getting used to how to challenge thoughts. Once a person gets good at the skill, the thought challenging then can be done in a person's head, without needing to work through a thought record.

I'd like to ask a question about my current self-awareness. I acknowledge that I may have unrecognized flaws, including aspects of my thinking and overall behavior that might negatively impact my mental well-being. I'm unsure if my actions are detrimental to my mental state or if I've developed a destructive mindset influenced by my surroundings or external factors. My main goal is to address and fix all issues. Are there any assessments I can take to gain a better understanding of myself and work towards improving these aspects of my life?

Thank you so much for this very helpful info you share so generously. Do you have any further info on health anxiety? I keep feeding fear by believing physical sensations to be a sign of underlying illness. The ‘all clear’ from the doc doesn’t help at all since they themselves say there is no such thing. Underlying causes may take years to show up on labs.

Hi Keith - if you mean thought challenging, then yes, that can be helpful for addressing anxiety, in addition to behavioral approaches like tolerating uncertainty, exposure to feared situations and behavioral experiments.

So anxiety is not chemical imbalance in brain?

Very good question Ivan. I don't like the "chemical imbalance in the brain" explanation for anxiety and depression because it relies heavily on a medical model of mental health that is simply not supported by data. It also leaves a person with no other options other than to use medications, because how else can you change a chemical imbalance in your brain. The way I like to think about it is that life causes chemical imbalances in your brain. When you're happy, your neurochemistry changes. When you're sad, your neurochemistry changes. And there are multiple ways of changing that neurochemistry. And what we know is that behavior changes and thought changes can have an impact on neurochemistry.
