I see lots and lots of failing first and second maxillary molars due to lacking MB2 treatment, Periapical radiolucency almost always right there on the MB root. They say greater than 90%+ of max first molars have them

Technically that's not completely true as most of MB2 canals are type 2 canals or y shape canals which even if you don't find them they will be sealed off from both sides of the canal so the nerve endings or the bacteria will not cause a major problem. And just keep in mind that they found that high percentage of MB2 canal a little bit more recently with the inventions of magnification devices and before that doctors used to do a lot of endo treatments with no significant complications or failure .

To my knowledge, I have lost 1 tooth in 40 years of practice following endodontic treatment, and that tooth had a M-D fracture extending into the furcation. I know that sounds impossible, but, truly I do not have issues post endo using the RealWorldEndo method, cleaning and irrigating the canals thoroughly with the Scout file, #10 Hedstrom files, followed by rotary files.. Many times in those 40 years I have not found the MB2 canal. My understanding is that canal often joins with the MB canal. Take your practice to the Top Tier. What’s your final solution or concentration of sodium hypochlorite?

3:1 dilution of water (3) and sodium hypochlorite (1). I do not "inject" the solution into the canal, but drip the solution into the pulp chamber and let the files carry the solution into the canals. This is the way Endodontist giving courses at my former teaching center in Dallas recommended placing the solution. Some of the Endodontist used 100% sodium hypochlorite and some used solutions of other liquids, including only local anesthesia irrigation. You want to clean and irrigate the canals, but you do not want a sodium hypochlorite accident. I do not claim to be an endodontic authority, but 100% sodium hypochlorite irrigation solution worries me because of the potential for a sodium hypochlorite accident. I have had excellent results with the 3:1 water (3):sodium hypochlorite (1) irrigation solution for 40 years. I do not want to have a big problem with any dental procedure, and a sodium hypochlorite accident could be a big problem. Take your practice to the Top Tier. I was instructed by several endodontists that "direct access" to the canals was critical. I have never done it any other way. I would hate to think about trying to perform endo. without direct access to canals. Some dentists/endodontists might be able to perform good endo. without direct access, but I am not one of those people. Good endo. is difficult enough already. I do not want to complicate it even more without direct access. I do not want to create a "huge" access opening, but large enough for direct access to the canals. "Direct" means the files/drills are entering the canals from directly above the canal, not from the side or at an angle. Take your practice to the Top Tier. Sir Im having right maxillary sinus issues even after my endoscopic sinus surgery. Today i went and had a root canal on molar number 2 on the right because it was sensitive to cold the dentist says you might have inflammation which could affect your maxillary swelling but you don’t have a infected tooth. We went along with a root canal on the right number 2 molar my question is would this root canal help the same as extracting the molar to determine if in fact it is causing inflammation of my maxillary sinus

I would not extract the tooth. I would not think an inflamed upper molar tooth would cause sinus issues. Sometimes endodontics is the answer for a tooth that is extremely sensitive to thermal stimulation.

If you just domed like me because of your faculty technique try to teach your self a more rapid easier technique like crowndown or single file technique or maybe later when you have more skill with endo try to combine different techniques together and made your own special technique Just remember in dentistry we only care about the goals or the significance of what we do not the way how we do it .

8.5%. I am not an expert on chemistry. That is the % NaOCl I have used for a long time, diluted with water, but other %s probably work just as well so long as you file the canal well with a Hedtrom #10 hand file initially, prior to using rotary files. If the canal is filed and irrigated well prior to using rotary files, it is not a huge issue if a rotary file separates in the canal. I love rotary files, but you must be careful not to let them "bind" or they can separate. This is especially true in shall, curved canals. It should not happen often, but if you perform enough endo. with rotary files, it is going to happen occasionally. Hand file and irrigate thoroughly first! Take your practice to the Top Tier. I know a dentist that make Endo for every tooth that he will make a crown for it even if tooth has vital healthy pulp and when I asked him he told me that he do that to avoid sensitivity after placing crown .. I think this is so aggressive but I want to know What is your opinion ? and what cases I can do that ?

I do not know what that shaper is. I do the endo just like I show it in the videos. Usually, the simpler the procedure the better. Take your practice to the Top Tier. 1 failure in 40 years? That’s better than every single Endodontist I ever met ever in my career so therefore I call BS. MB2 matters.

I agree, it sounds impossible, but as stated, "as far as I know," I know of only one failure and that tooth was cracked through the furcation. As the chief of the endo department told us, "You can fill the canal with horse manure and be successful if you clean and irrigate thoroughly." I am not advocating poor filling of the canals, but that is what he said. I never think about an endodontic procedure failing when using the RealWorld Endo rotary system. Now, I am not trying to be a hero. If the tooth does not have a good prognosis, I am probably not going to perform endo. In those cases, I would probably recommend extraction and an implant. Endodontist are undoubtedly performing endo on many teeth with poor prognosis. Take your practice to the Top Tier. I do not know what you are referring to. I have not used mercury amalgam in 35 years.
