hello, for a full thickness rotator cuff tear, would you always refer to orthopaedic surgery?

depens on the muscle. my father had a complete tear on his subscapularis muscle, it healed conservatively very well and he is back in action.

This is what the guidelines recommend: referring can be considered in the presence of a full-thickness tear, significant pain, disability, and/or muscle weakness. They advise a low threshold to refer to a surgeon. However, this does not mean they will get operated on- often this does not happen, in older individuals for example. Ideally, they'll come back to you for a conservative approach.
