Apparently SI joint pain is rare because I've been to a dozen doctors and chiropractors they and never said anything about posibilies of my SI joint. So I had to finally self-diagnose myself that it was my SI joint is where my pain is coming from😢

A lot of people with SI joint dysfunction have EDS or hypermobility. This is the root cause of the most painful cause and sadly aside from braces there isn't much that can help. Perhaps knowing how to put your pelvic hip bones back into place is the most helpful.

@@DavidBertossi I am dx w hEDS and have SI joint issues, the reason she’s saying that is because ours tend to sublux/get out of place and then try to stay that way. Although I have found relief through certain exercises and building up the glutes medius muscles, the exercises spoken of in this video would undoubtedly exacerbate my problems and make things way way worse.

Look up Whelton methods. He has a method that he designed to fix the si joint and get it back into place. He’s in Tampa fl I went to him from Chicago and it was totally worth it.

Excellent video, very informative! I have a question for you if I may : What's the point of identifying the SI joint as the source of pain, if it doesn't add anything specific to the treatment? The treatment plan that you presented is non- specific exercise that doesn't differ all that much from many other ones that me and many other rehab people would use for on many other non-specific pain scenarios. I understand that's it's important for the patient to understand what the likely sources of his/her pain experience are(and the nuances of it), but is there anything beyond that?

It's pretty much impossible to move the joint by itself so you have to use various different movements in order to hopefully help the SI. That's my guess anyway.

From an Osteopathic perspective, whenever I encounter a patient who presents with SIJ pain and specifically points to the joint (inferior and medial to the Posterior superior illiac spine), I want to know if the Laslet pain provocation cluster tests illict their familar symptoms. 1. If it doesnt then I need to consider if the contributor to their pain is a referral pattern from the nerves in their lumbar spine affecting the SIJ. 2. If it does then i turn my attention to things like whether the patient has excessive or reduced force closure (research has identified a correlation). People with hypermobility or post partum for example, tend to have reduced force closure . There are tests to identify this and they usually respond well to pelvic stabilistation exercises with some shown in the video. The pelvic exercises would be specific for this corhort (bridges, side planks, abductions etc). Those with excessive force closure would not benefit from pelvic stabilisation exercises but a more general mobility program as they tend to have alot of co-contraction and protective guarding of lumbar and pelvic muscles which is a contributor to the increased mechanical load on this joint causing symptoms. So in conclusion it is worth knowing whether the source of pain is coming from the SIJ joint itself such as stimulation of nerve endings within the ligaments of the SIJ or a somatic referral from the lumbar spine. This in turn will inform my treatment approach and the explanation I give to the patient. Sorry for long explanation but its quite complex.

@@hillsongirl1 1) How do we know what ''hypermobility'' is, if mobility is specific to the patient + we dont have any measure before the onset of pain? How do we assess that clinically? The SI joint barely moves. 2) What is ''pelvic stabilization'' ? How can a pelvis be unstable, without any fracture, and what parts of it? 3)How do we determine that a pelvis is unstable and even say that the patients pain is due an ''unstable pelvis''? 4)What parts of the pelvis do these exercises stabilize? Can we say the same thing about all the other parts of the body that these exercises engage? It doesn't sound complex, just nebulous with many arbitrary definitions and concepts.

@@michaelangelo1417 as someone diagnosed with hEDS if somebody has hyper mobility in their sacroiliac joint, they are very likely to have hyper mobility, at least in one other area of the body and that is not hard to find if you know what to look for. Are they “double jointed“ in their fingers or hands? are they “double jointed“ in their shoulders, elbows, knees, etc. etc? that could be an indication that that is what is happening in the SI joint if they do not have global hyper mobility

Yes, that was helpful. I'm 70+ and injured SI joint by picking up a concrete planter - 3 yr ago. Unbelievable pain from this. Back, hip,thigh, shin ankle. I've been aggravating it by being on my feet too much. I ordered a pelvic/si joint rehab/exercise book and ligament supplement powder and hopefull the pain will subside some so I can actually do the exercises. I used to do squats which are so helpful in strenghtenening and glad to see you recommend. Do you recommend an Orthopedic or Rheumatologist for this specific problems? Lax ligaments. Most docs don't seem to know what to do for ligaments. I'm so frustrated and not ready to continue lying on this infrared pad forever. Thanks so much! p

Sorry for your discomfort. Have you looked into prolotherapy for ligaments? I think they only do it in the US though.

@@hillsongirl1 Thank you, Louise! U R reading my mind! I don't want steroid injected in my already compromised ligaments, SI joint, etc. but the doctor that did Prolo in this area passed away a few years ago. However, I'm prob going to the pain clinic for eval and they should know all about and be able to provide Prolo...right? I'm praying they do. I really am discouraged about it all. My xrays showed digenerating disc disease , which is common to aging backs but nothing clear on the SI. I think I'm going to go to a DC I've learned about that is very familiar with SI. I'm so tired of not really knowing what to do but I'm trying. I read a book on Prolo a few years ago and I really liked the philosophy and the results many have had. Thank you for the encouragement. You mentioned nothing whatsoever about hypomobilty, where your si joint gets stuck. I’ve had every test possible. My pain is definitely coming from the si joint. After trying numerous different therapies, It has come to the point that I need a si joint fusion ( endoscopic) I constantly have a functional short leg, that is caused by hypomobile si joint. Infact, it’s so bad, I can get adjusted, my legs are even, I can walk out of the office, step off a curb wrong, & I have a functional short leg/unstable pelvis again. With a hypomobile si joint, your pain can mimic sciatica a lot of the times along with very painful Si joint pain.

Great info thanks!! I'm suffering from si pain most likely due to an artificial disc that wasnt centered correctly. I'm thinking my mechanical structure is off a bit, I also had fusion and I know si issues are a common side effect. I'm considering surgery, I would like to know if the si can heal itself without surgery, considering my mechanical issues. A few injections I've had done seems to have confirmed si dysfunction. I appreciate any help very much thank you!

Yes. Many issues in the SIJ region do very well with physical therapy that tends to put a focus on things like appropriate exercise progressions and lifestyle modifications. We'd recommend "interviewing" physical therapists in your region who seem to align with that approach and then choose one that seems to be the right fit.

Absolutely nothing has helped mine and I'm close to being crippled to my bed.. cant I ask an orthopedic to fuse it? Been having pain the last 5 yrs and it's getting. So bad I can feel it poping in and out. I also have ehlers danlos.

I also have Eds, have you tried the “1 quick exercise for SI joint pain” by precision movement here on KZbin? It has dramatically improved my symptoms as well as some videos by athlean x talking about waking up the glutes it’s a video where he shows for simple exercises to work on the glute medius and those two things combined have really helped me

Does anyone know what sudden, INTENSE, stabbing, pain in the SI area, especially when transitioning from sitting or bending to a standing position could be? It kind of feels like somethings slipping out of place or maybe being pinched. I’m talking a 9 on a 1-10 scale of pain. It goes away in maybe 10 seconds though. I can’t sit long or it gets uncomfortable and I eventually get that stabbing pain even if sitting. I’m only really comfortable lying down.

@@daypixie Well the middle of the glute is where the SI joint is, so yes, afterwards it feels like I did too many squats, like that kind of muscle pain, you know?, like you worked out, and sometimes it extends out towards my hip too. When I describe it to my doctor I tell him it feels like somethings misaligned and popping out of joint but he keeps insisting it’s just the muscle, so we’ll see I guess. He gave me physical therapy exercises to do and if that doesn’t work he said we’ll do X-rays and see what’s going on. Does that sound like what you’re experiencing?
