Stephen does keep 'idea" notebooks and when a notebook fills up with notes, he knows he's ready to write that book.

idea notes is not at all the same thing as an outline

is the course content different between 2012 and 2014, 2016 etc? if ive watched his 2016 sessions, is there anything that is not covered there that is done in the earlier sessions?

He pretty much follows the same agenda, but of course, through different examples and different questions you get a slightly different experience.
