when you don't know the cause of your chest pain
A 35-year-old woman develops sudden, excruciating chest pain, which occurred while going for a run, and resolved with rest. She's never had this chest pain before, and the first thing she wants to know is,
"AM I HAVING A HEART ATTACK?"
You can usually figure out what's causing chest pain by asking the right questions, but sometimes further testing is needed. Of course, whatever you see on the internet, including this video, is no substitute for seeking medical attention in real life. With that said, by the end of this video, you'll know my approach to diagnosing chest pain, especially since MANY clues can give away the diagnosis.
So let's get at it.
Chest pain can originate from all organs in the chest. (heart, lungs, esophagus, nerves, muscles, bones, and joints)
The 3 most serious lung diseases that cause chest pain include
1) Pulmonary embolism, meaning blood clots in the lungs,
2) Pneumothorax, where a lung deflates like a flat tire, and
3) Pneumonia.

And let's not forget about the gastrointestinal tract. Problems with the esophagus can cause chest pain, including GERD (Gastroesophageal Reflux Disease), aka Acid Reflux. Acid Reflux is the most common Non-Cardiac cause of chest pain.
Then you have the very common musculoskeletal causes of chest pain, like costochondritis. Chest pain often induces anxiety, but the reverse is true anxiety, and emotions are often the causes of chest pain….is it the chicken or the egg?
In order to sort out the cause, we have to answer some key questions and understand the context, Like the age of the patient and past medical history. We need to know specifics about the chest pain. It's the character, duration, and exact location.
What makes the pain worse?
What makes it better?
For example, is it better or worse when sitting up or lying flat?
How about when you take in deep breaths?
And, of course, what are the associated symptoms?
Like shortness of breath?
Cough?
Back pain?
Fever?
All very important. Chest pain that originates from the heart can almost always be boiled down to 1 of 5 things: A heart attack, angina, arrhythmias, aortic stenosis, and pericarditis.
Angina refers to chest pain that occurs when the heart muscle isn't getting enough blood flow. It's a feeling often described as chest heaviness, tightness, squeezing, crushing, constricting, or sometimes burning in the center of the chest or a little bit left of the breastbone. The pain sometimes radiates to the shoulder and arms, typically on the left side, and can also radiate to the neck and jaw, and upper abdomen.
Stable angina lasts less than 5 minutes. Unstable angina, which is the worse angina, lasts 5-10 minutes, sometimes longer.
When it comes to a heart attack or myocardial infarction, this is caused by a plaque in the coronary artery that breaks off, travels downstream, and lodges somewhere, causing a sudden blockage of blood flow to a particular area of the heart. It's overall similar to the chest pain with angina, but usually more intense pain, and more likely to feel short of breath, nauseous, and have profuse sweating. The pain builds up over minutes and lasts at least 30 minutes.
Do women have different symptoms from men?
Not really. Women might be less likely to have chest pain when having a heart attack. And they are slightly more likely to have severe fatigue as a presenting symptom.
Pericarditis inflammation of the sac around the heart. Which can be caused by many things, including viruses, including COVID. Pericarditis causes severe, sharp, stabbing chest pain that is worse when taking deep breaths and coughing. The other unique feature about this chest pain is that it's worse when lying down and better when leaning forward. The pain doesn't really away until it gets properly treated.

While I'm going to save this video because I think the overall information is helpful. I'd like to point out that the main reasons the symptoms of uterus owners or honestly any person that isn't a cis white guy are "different" when it comes to anything where pain is a common symptom is rooted in the fact that we're all socialized to suffer in silence. And because of that, we're so used to having to just push through pain, we often have such high pain tolerances that we've essentially told our brains to ignore it as much as possible. Truly no offense, but the only complaint I have with videos here, is that you (Dr. Mike) have a history of ignoring this and even victim blaming when woman don't report things and it skews research. That's the fault of the medical field more than the patients. And I really really really wish you'd address with your own biases at some point and stop being so dismissive when talking differences in symptoms between genders. I hold you to a higher standard because you have shown that you already meet a high standard, and I'm hopeful you still want to do even better. If I seem critical, it's only because I believe you have earned my time to make a critique. I am not telling you anything more than what my profession gave me expertise in. Before I retired due to disability, I was a mental health professional and social worker in child and family services with a focus on treating PTSD caused by abuse and CBT to help abusers stop their cycles of violence. A big part of the latter was raising the awareness of the "suffer in silence" injuries society inflicted and they made worse. I have no idea if you'll even get this, but my entire professional and post professional volunteer work has been combating elements of this, even before I knew that that's what I was doing. It took over a dozen years for me to identify this and put it into words that could help others understand it, and I just hope it expands your understanding of why there are so many perceived yet erroneous differences between cis white men and the rest of the world. I also hope that helps you stop the dismissive wording and shoulder shrugs that signals anyone that experiences these differences that their experiences don't matter, aren't real, and aren't worth your time to address. Please stop inadvertantly telling people you don't care, when I'm pretty dang certain you do.

I have been to ER twice because I thought I was having heart attack and it turned out to be anxiety. But I do not regret being on the safe side, going and getting all the tests!

My sister was diagnosed with anxiety in an ER but she died of an undiagnosed heart condition at 43. Unless they’ve run all the tests to determine no cause, don’t give up on finding a physical answer. It could save your life.

I was told I had anxiety when I had multiple pulmonary emboli. That ER doctor nearly killed me as I trusted her and continued to walk around short of breath for weeks until I went into crisis and ended up at a different hospital.

Only after all the necessary tests were done on me that my doctor finally diagnosed me with anxiety. But, before that, I had an ECG, treadmill stress test and wore a Holter monitor overnight which all came back normal.

Dr. Mike, in the 13 years between 2004 and 2017, I went through 3 cardiologists, 3 endocrinologists, 2 gastroenterologists, all trying to find out what was causing my symptoms. During that time, I was sent to the E/R with suspected heart attack *9 times*-none of them were CI events. Not a one of those 8+ MDs could figure out a cause for my consistent, labile hypertension, spells of tachycardia (it felt odd, doing a Valsalva maneuver in the shower), chest pains (c/ left arm involvement), thoracic pains, and flashing spells of allergic-like reactions. My BP went (without external stimulus) from being above AHA normal to jumping crazily to 190/150, 145/122 (the scariest), 180/130, and such, then back down after a few minutes. It was often hard to sleep because of the pounding of each heartbeat in my ears [Oddly, cardiologists didn't seem concerned by this obvious over-exertion of the heart.] Then, in 2017, I found I could stand for no more than 5 minutes without going numb from the waist down. Imaging showed that I had stenosis at L5, with a mobile L4. During surgery, it was found that it was far worse: I had acute stenosis, with the normal, c. 1 inch opening of the spinal canal at L5 down to nearly a quarter-inch, essentially strangulating my spinal cord. (With L5's hold on it, and L4 mobile, a fall, push or such could have severely damaged, or even severed the cord there at L5.) The surgery scheduled for 3.5 hours stretched into 8. (The condition was bad enough, and my surgeon direct enough, that insurance never questioned anything he prescribed.) Awakening from anesthesia, I found all my symptoms had disappeared. The surgeon explained that my spinal cord had been sending out continuous pain signals, although beneath conscious level, that were driving my adrenals crazy-I had spent most of those 13+ years in the midst of a fight-or-flight episode. My BP is now consistently 120/76 (some mornings it's 118/68, not bad when you're 67). The worst part of my condition was those symptoms were too often so quickly transitory that the normal methods now in use for diagnosis never caught the sudden spike in adrenal hormones, especially since they were quickly quashed by the body, with suppressive or counter-reactions. The lesson is never to give up when you know something is wrong. Those of us who have pets know that, since pets won't tell us, we have to pay attention to their signals to discern what veterinarians in the Southern US sometimes call a "JAR" condition: that pet "just ain't right." *This is not an invitation to fall into hypochondria, but just to remind you to **_learn to recognize when something "odd" is "really odd" rather than just "normal odd."_** Then, don't give up.*

My BIL has stenosis of his spine but further up in his cervical spine. In early mid Feb of 2020, he came home from work and nearly fell taking off his shoes. The previous days before, he also had a hard time holding things. My sister said, "that's it, we're going to the ER". My sister at the time was about a week away from giving birth. After numerous tests over the course of almost a whole day, a doctor noticed he had stenosis and was one wrong move from being paralyzed. He was rushed to the OR within 15 minutes of the diagnosis. The next day, he was walking normal. We think that he may have injured himself during his days at FedEx and packing trucks. But it's crazy that he was so close to being paralyzed and my sister giving birth so soon.

Dang! I'm impressed you were able to get action as soon as you did. Too many doctors take the maxim "when you hear hoof beats, think horses, not zebras" as an admonition not to look for zebras even if horses don't really fit. Occam's razor does more harm than good in troubleshooting because it practically invites complacency. When we jump to a conclusion we have one chance of being right and unlimited chances of being wrong.

I would say 90% of our chest pain complaints in urgent care were due to anxiety. Costochondritis and gastric reflux were extremely common too.

After complaining of chest pain, my wife was recently diagnosed with costochondritis and it seems to come and go, but never completely go away. Sometimes she won't feel it for a week or more, then sometimes daily for a week ore more. Docs just say take NSAID and try not to overdo it. Wild.

I was first diagnosed with costochondritis 10 years ago. Mine comes and goes too. The pain usually isn’t too bad, but ibuprofen helps when it gets too bothersome.

As a 1st responder (police), I went on several "possible heart attack" calls over the years that turned out to be gall bladder attacks. In fact, one of the EMTs at my first such call (it was at a Pizza Hut) referred to it as a "café heart attack." In both cases it sure looked symptomatically like a heart issue to me and those surrounding those patients!

A significant difference is in the lack of shortness of breath with exertion. All the women on my wife's side of the family have needed gall bladder surgery by age 30, so it is not an uncommon problem. No fun at all, though, typically with much more severe pain than a heart attack. If heart attacks hurt that much I think people would take them more seriously.

Thanks for bringing this up. I had a similar thing happen when I had chest pain attacks with back and shoulder pain at around age 40 and my blood pressure was kind of high. I went to see a gastroenterologist first because my symptoms sounded like they might be a gall bladder problem. That doctor told me I "didn't look sick enough" to have a gall bladder problem and I wasn't jaundiced. He did an endoscopy instead because I was worried I might have celiacs, as my mom had been diagnosed with that. He said that was all normal and it must be anxiety. I went to a cardiologist and had a stress test done because I wanted to rule out that it could be caused by my heart and they said they couldn't find anything that would cause pains. They basically told me the pain might improve if I lost weight, so I did. I still had crazy chest pains and it went on for a year and a half until one day I just couldn't take it anymore and I decided to go to the hospital while having an attack. They ruled out my having a heart attack first, then I asked them if they would check to see if I had a gall bladder problem. Turns out, that was the problem the whole time as I had numerous gall stones. I had a surgery to have it removed and I have been much better since. No more chest pain attacks and my blood pressure has gone back down to normal range. Mainly I was just happy that the hospital was able to figure out what was wrong with me and it was such a relief to hear that the chest pains that had been waking me up at night weren't all in my head. One of the guys in the ER said it was pretty common with women my age to have that issue. That was something I wish I had known before because I would have gotten a second opinion on that much earlier had I known.

Thanks Dr Mike, brilliant education! Irrrespective of age chest pain needs to be taken very seriously. I had severe chest pain and I thought it was a heart attach. I'm 40, carnivore and extremely healthy, so I was shocked! It turned out to be costochondritis.😔 Very scary experience.

Thank you for this explanation. As a sort of cariophobic I'm constantly dealing with the fear of having a heart attack. Every chest pain, every heart beat that feels "abnormal" rushes me into a panic attack. Which will make the symptoms worse and gives me more anxiety. Videos like this can teach me more about how to listen to my body and give my mind a rest. I've been to the ER and doctor a couple of times with a panic attack and even though they always tell me my heart is fine after ECG's it will always be a part of me. This started when my 1st child was born. And when I'm alone in bed and the kids are home it gets worse. The fear of dying while the kids are home and mom is not around is the worst. I won't sleep and stay awake till my wife is back home. I don't want to resort to medicines because I don't want to be dependent of them. I took oxazepam once when my panic attack was completely taking control and couldn't function anymore. But don't want to do that every time. I'm now trying to control it through breathing exercises and this works for me. I have more control over the panic attacks and they are not as bad as they used to be. But the fear always remains.

6 years ago I fell down unconscious due to sudden cardiac arrest. I never remember any sign of chest pain both before and after the problem was fixed with a medicine coated stent (PTCA) to my LAD. To this day I never had any cardiac related issues nor any significant chest pain and I am living a very normal life past 58.

Your right, just a slight pressure under the sternum indicated 99% blockage of LAD for me.

@@iFriendofYours There is a device called AED (automatic external defibrillator). The machine check the heart condition and gives the right amount of electric shock automatically. The emergency medical staff used it at the airport itself and on the way to the hospital. Even at the hospital they used it 4 times to bring back the sinus rhythm (normal heart beat). I woke up 4 days later in the hospital after the stent was already fixed. Only a week later when I came out of the hospital they gave me the Discharge summary which explained what happened in detail. That's how I knew it. Also it took me more than a month to do normal work like walking and getting back to my job.

@@iFriendofYours Yes. Indeed I was lucky to be at the world's busiest transit airport (Dubai) and the best airlines Emirates. Dubai hospitals is just at the end of the terminal 3 where I was treated. It's a government hospital but the treatment is on par with the best hospitals in the world. Later my cardiologists told me that Dubai hospitals did a fantastic job of fixing the stent. I still have the procedure recorded on a CD. The hospital charged me only for the cost of the stent and a few medicines and all the other charges were free. I almost forgot that I have a stent because I have zero heart related issues even after 6 years. The chances of survival in this kind of problem (sudden cardiac arrest) is just 5%. 95% of the people don't make it.

Back when I was 40 and going through fertility treatments, I had a heart attack that was caused by a blood clot. It felt like I was experiencing the same kind of muscle burn you get from an intense work out--but in my chest. It got so intense I broke out in a sweat and then threw up, after which it seemed to subside a bit. They did a blood test at the ER that showed I'd had a heart attack. The heart catheter showed no obstruction--hence the blood clot theory. Scariest experience I've ever had!

Fascinating! Replace 35 with 39 and the description of the woman matches me perfectly (crushing chest pain during exercise that goes away with rest, and seems to be slowly getting worse). Other than an EKG, the doc hasn't done any further testing, saying that it's probably nothing serious...

My youngest sister was almost 40 and almost died before they realised that she had a genetic condition with her heart. Even in high school they were doing some kind of breathing thing in science class and she couldn't make it work and they said she wasn't doing it properly. Two pregnancies and she had to almost die before they realised. Doesn't help that she has a different father, and mother won't admit it, and he had a massive heart attack at 40 as well. All siblings had to get ultrasounds and MRI's. So yes, chase it up as you never know. Funny how it is easier for us to advocate for pets than ourselves.

Took 5 trips to AE at 48 till one of the doctors recognise Angina due to symptoms occuring on exertion. Microvascular angina doesn't show up on normal tests..needs an MRI . Lesson of the story? Keeps going back and see different doctors untill someone recognise your symptoms . End off

Hi Doctor Mike. I'm very glad that you brought up this topic. I always have chest pain whenever I lay on my bed facing downward for more than an hour. I had to slowly and gently turn myself over after that. It felt like my chest was stuck on my bed and it'd rip my lungs and my heart out if I didn't do it slowly. So I always avoid laying on my bed facing downward ever since. My chest is completely fine other than that so I didn't really think much of it and move on. But I did some research once but couldn't find anything helpful. Thought it might be a good idea to share this with you Doctor. By the way, congratulations in advance on reaching 1M subs! 😃

I forget what that is called - in the vernacular it seems to be a variation of a "stitch in the side" - but it is no fun at all. I've only had them maybe a dozen times, almost always when I am face down in bed and it wakes me up but it seems to get better - not go away - if I move rapidly. It hurts like crazy but it is over more quickly. I sure hope somebody has a better idea.

I have been diagnosed with cervical angina, which means the nerves coming from my damaged cervical spinal cord and ending in the chest, cause pain identical to normal angina. It can only be diagnosed properly with an EKG or troponin blood test. I have had a heart attack in the past and now have Congestive heart failure so if I have to go to the ER, a lot of testing has to happen. 2022 is my year for staying out of hospital!

I have this crazy chest pain a few months ago, I thought it was Covid-19 symptoms, I went to the ER do a ekg. See the heart doctor, went to see an gastroenterology do a few test and last I do a endoscopy, where the doctor do a biopsy and the result was I have , H-pylori… I was treated for with medication where it gets better but if I do coffee in two days in a row, that pain comes back… and now I work in the hospital with a mask on, it’s so crazy

Was in the ER last Thursday. I had chest pain but it was very forward, like my sternum. It radiated to the insides of my breasts. It was worrying and I couldn't figure out why. I bent over to move shoes from the door so EMTs could get in and the pain melted away. But I had sky high BP when the EMTs took it. Went to ER. All tests came out good. No shortness of breath or anything like that. BP went down and I was sent home.

Same symptoms happened to me while running in morning. Went to ER couldn't find anything. Happened a couple times. Dr. In WA diagnosed I was illergic to Caffine. If I have coffee in morning and then exercise it happens. I went off Caffeine Coffee and no more problems.

Thanks for this video Dr. Hansen. In my situation close to a month ago i went to ER due to dizziness (not spinning but rather feeling like you're about to faint), shortness of breath when i try to exert on carrying out a conversation and then you can tell the dizziness could about to flare up. I've been having a little minor of chest pain but come and go. In a good day during this month, if i get lucky i would get just a congested head w/ eyes movement related to that congested head. Otherwise, I'd experience the dizziness, shortness of breath, nausea that i'd have to lay down for at least 30 min to an hour to start recovering. At hospital ER, they did the EKG, CT Angio for the heart, head, neck, and brain and they said no blockages or anything that was out of the norm. Recently, i just did a stress test for the heart, echocardiogram and i'm awaiting for the results. Prior to the stress test i did let the nurses know that i do have a congested head but no dizziness or anything at time. Performed stress test very well without any chest pain or shortness of breath but actually every time i do excercises or brisk walking, it seems to help clearing my congested head. This has been happening daily for almost a month and is not going away. I'm also being scheduled to see my neurologist in a week or so. This is really impacting my daily life, activities and work. Dr. Hansen, if all the tests were ruled out good could this be something else other than a congested heart issues or something related to the heart etc.? I'm not seeking medical advice here but was wondering if anyone might have an idea of what's going on as i do have a feelings that my echocardiogram will come out w/ good results as well.

I have heard that symptoms in women can be different than men's. A female friend in her early 40s had been feeling tired for a few months. She wasn't overweight and exercised regularly. She was jogging at the beach but felt very tired so sat down on a bench and died from a heart attack.

What causes sudden and severe pain in the ribs that lasts only a few seconds? I've got occasional "rib pains" my whole life. I can get a couple a day or go months inbetween. The only last 10-20 second at most but feel like I was stabbed. They don't seem to cause any damage, just anoying as hell. Docs have said "don't worry about it". I'm just curious what these pains are. I've had a PE and afib, these random pains don't feel like either of those.

I felt chest pain twice and it was due to Anxiety. When the doctor told me that I had Anxiety, I asked him...what can cause Anxiety attacks? He asked me if I was under any stress of any kind? He also asked...are you having any problems of any kind? I replied...yes, I am. My husband keeps yelling and screaming at me all the time and he keeps hitting me on the head and my arms. He slaps me on the face and pushes me and so many other things. This is when he said...You need to report your husband to the police because he is abusing you. This is when I realized that my soon to be ex-abuser husband wanted to kill me. I am not with him anymore and I feel better. I am at peace and I know that I don't have to accept this type of abuse from anyone.

Microvascular Dysfunction is a Dx of exclusion and when it does not lead to infarction ( MINOCA ), it is called INOCA. N.B there different underlying causes of MINOCA/INOCA and micarovascular dysfunction is only one of them.

I have chest pain since getting covid. that was over 6 months ago. Funny is when doing exercise it hurts less. I had some heart tests and everything looks ok. Great video, honestly after covid I wouldn't worry that much about chest pain, it could be muscular, but in my case pain was so bad that I thought I was having a heart attack....... keep up the good work Doctor.

That is a fairly common "long covid" (PASC) symptom. The good news is you have a healthy way of quieting it down! Sadly, no effective therapeutics for PASC yet.

Mannnnn I didn’t know nothing about this but i caught covid Twice! Then I caught pericarditis I thought I was having a heart attack at the er but I wasn’t Iva had daily chest pain since then and my cardio dock has checked my heart many times and it’s fine. I did get diagnosed with high blood pressure tho but not crazy high numbers im in the 130-140 over 90-95 range which came be fixed by simple lifestyle changes but its the chest pain that worries me. I really think covid jacked me up big time

I’m so sorry. Jaw pain is a “classic” sign of a heart attack. Some people even have “pain” in their sinuses-but it is actually referred pain from the heart.

I'm 51 years old and I thought I was having a heart attack the other day. I went to the ER they did to EKGs to X-rays and took blood and said that I was fine. I think it's due to my hormones the cold weather and anxiety. I'm still very terrified please pray for me ..... Update. : it could be lung related. Im getting it checked out by a pulmonary dr

I saw a study where they said that pain of heart attack equals menstrual pain. That might be the correct reason why "women do not have chest pain with heart attack"; it just does not always register to us as pain, because it is low on our pain scale. What doctors have not taken in yet is that realisation that generally women's pain tolerance is quite high. Normally female bias makes health care professionals under value the female patients pain.

I’ve had stabbing pain in a spot just under my left breast seemingly between the ribs every once in a while and not so often that I thought something was seriously amiss ever since I was little, the first time it happened I had been running, I was told it was a stitch in my side, which of the five options is the most likely thing causing the pain? It has recurred several times and sometimes without any running I don’t remember ever bringing it up with the doctor so I don’t know what exactly it is.

I hear you! Fortunately it is a food sensitivity thing for me and not hard for me to control... I know I am really lucky about that. The particular foods have changed over the last 30 years, from the original orange juice to any acidic liquid to chicken. I have wondered if omeprazol daily to reduce reflux possibility would help but right now nothing is being a problem. For me sipping water between bites of food has helped a lot, but doctors sure haven't!

@@503sld I worked with a guy with something like that sort of achalasia. In his case the sphincter above the stomach was just too small. He had occasional treatments with a balloon that was put into position and suddenly inflated. He said the moment of blinding pain every year or so was kinda worth it. It sounds like your condition is even worse. You have my sympathy.

@@flagmichael Once operated on, its not that bad. It is life changing, but you get use to it. In Achalasia, the LES (lower esophageal sphincter) does not relax. Which means, it simply does not open. Before the surgery, it is hell. Food gets stuck in the tube, comes back up at every meal. At night, there is a huge risk if aspiration since the food makes its way back up towards the tube that goes into the lungs. Your friend could get the same operation that I had, it would help him pas the food into the stomach. There are two procedure a Heller Myotomy and a POEM (Per oral esophageal myotomy). I feel for him, its not pleasant!

And if she kept saying that she did not have anxiety and knew that there was something wrong, they treat her like a difficult person and send her home. If she comes back to ER with the same complaint she is hypochondriac. That's what happens in here if EKG looks normal 😑

With a few exceptions our internal organs tend to be singletons. We have two lungs, two kidneys, but when our singleton internal organs fail our life generally comes to an end. Since that usually happens beyond our reproductive years it does not have a place in natural selection. We all go sometime... but I am very sorry it is so soon in your case.

In March 2022 I had real strong chest pain, it went after going to hospital for 13 hours and it was apparently acid reflex it went now on the 8th sept 2022 it started it again but it’s different from the pain I had before the first time. I’m so scared right now, I’m only 20 years old also I laid down on my right side but the pain was on the left side so I had to get up fast as it was so painful. I’m laying down on my back which I never do.

Can spike proteins cause all the above symptoms? Seems like they can since they block blood flow on all levels and cause severe inflammation where the spike is sticking out of the vascular endothelial cell.

It is not a concern on its own. COVID-19 is now seen as an inflammatory disease rather than a lung disease but the spike proteins alone have little to do with it. As with nearly all inflammatory conditions the immune system is central to the process but the virus itself can seriously damage cells in the circulatory system.

A cat scan shows it. Had a case in the family a few years ago - it looked like a candy stripe on the aorta. Before the scan was complete the tech called the surgeon over to look at it and he began facility prep immediately.

@@JC-bl9bo It really was scary. He could walk for a half a minute or so then his legs wouldn't work. He crawled to our bedroom door and reached up to let the dog in to wake us up. My wife - a retired nurse - was on the phone with 911 and was trying to get a radial pulse... there was none but he was still able to talk. In the ambulance there was unreadably low blood pressure in his arm but he was still coherent and responsive. Now he has a synthetic aorta.
