Honestly, I needed this video so badly. I just took the MCAT, and after spending so many hours studying and these past years shadowing and volunteering only to know the statistics, it seems way to extreme at times. I wouldn't want any other dream career in the world, but this helped me rationalize my thoughts a lot. Thank you again!

When I was a freshman in college in 2015, my pre-med advisor told me that I needed a 3.5 GPA and a 508 (74th percentile) on the MCAT to have a solid chance at medical school. Now if you want a solid chance at an MD school you need at least a 3.8 GPA and a 511 (83rd percentile) and a bunch a research with all the other ECs to get in. We need to create new medical schools in this country more quickly, lots of good applicants are being left out.

@@shivanishastri4285 Yes of course! I think either or because if you have more medical students US med schools will put pressure to open up more residency spots because it reflects poorly on them if their students don't match.

I got into a DO school with numbers that med advisors would cringe at. Mark my words, MD schools are going to start losing out on good applicants because they’re hung up over numbers.

I finished my bachelors, took my MCAT, volunteered at hospitals, gained clinical experience working as a scribe and did research in a lab. After following your videos for several months I've finally decided against pursuing a career in medicine. I'm not going to apply to medical school, mostly because I think I'll make a bad doctor and won't be able to provide patients the care they deserve. Thank you for the insightful videos and helping me avoid a mistake that could one day cost the life of a human being.

It's kind of hard to self evaluate in this way. I mean, you know yourself best, but don't necessarily sell yourself short. The 'ideal doctor' personality isn't really reality. There's a huge variety of different personalities in medicine.

Respect the self evaluation but I disagree with this. You've had to be significantly careless in order to cost someone's life. Plus being a doctor is like working in a team so you have co-workers to double check your work.

Peace be to you! Thanks for covering this so thoroughly! Unfortunately, the Physician Assistant/Associate profession is starting to approach this paradigm (high GPA, GRE, etc.). I have seen a huge drop in Black male applicants over the years who hail from areas where they're mist needed ("da hood"). I lived, was trained, and work in these areas even after practicing medicine as a PA for almost 20 years. BTW, I'm glad you mentioned the paucity of Residencies, I didn't know about that; makes a big difference! Thanks again! 🤓❤👨🏾‍⚕️

Honestly this is why I left the path of becoming a doctor. So much competition. Not a super bad thing but you have all these students who are literally out for blood and will stomp on your throat to get that acceptance. I want to help people and if this is the way I get there...screw it. So I decided on becoming a nurse. The path there is so so SO SO different. Everyone is so supportive and friendly. No one wants to stomp on your throat because nurses are in demand and we all WILL get a job whether we graduate from as the top student or the lowest student. If we pass we will get a job regardless. The same cannot be said about med students. And I’ve decided from becoming a nurse I can have the opportunity of becoming a NP which basically is a doctor but a different name.

I also think it depends on the culture of the school. For undergrad and during med school so far, I purposely chose schools where the students, faculty, and mentors did not have that dog-eat-dog mentality. Everyone at my med school is extremely supportive, friendly, and we always try to help each other out.

Chowdhury1000 Agreed (although my school does have a grading system). You can see the difference in cooperation at my school vs. other med schools in the state. We train at one of the major hospitals in the area all throughout our 4 years along with another med school near us. Let’s just say that the students at the other med school always seem drained and sad, whereas we’re much happier. It’s tough, but the environment is totally different.

@Roy_Thousand Same with Anesthesiologists and CRNAs. The problems is that hospitals are hiring more CRNAs that the former due to the high cost.

@@Curlya147 The reason pay is high in north dakota and bumfuck kansas is because of high demand low supply. If you increase the supply by 15k, salary is going down. Its literally basic 101 elementary economics.

Doesn’t matter, salary is not as important than having time balance as a doctor for most. If there are more doctors, less hours would be assigned to each one along with less pay. I think many doctors would take a pay cut for having less hours because of how overworked they are being assigned many patients and not having the opportunity to know each of them well enough.

Excellent video! It’s absolutely insane how competitive medical school is in the US..with allopathic schools accepting an average of only 5-15% with even most Osteopathic schools accepting less than 10% of applicants nowadays. The face of medicine is definitely going to change drastically the next 10 years by the time I’m practicing

You’re right. Something has to give with the residency position shortage crisis. I wrote an article called “a viable solution to our residency crisis” that I think solves the problem without having to beg congress for money. I’d love to hear feedback from this bright group of people about it.

I think one thing that could be addressed is the way that med school application requirements, both hard and soft, wall the field off from many marginalized people. If you're poor, or face widespread discrimination, it's going to be really hard to get the sorts of scores and experiences that make people competitive, and for reasons that have little to do with how good of a doctor someone would be. If you have to work to support yourself or your family through college, you're probably not going to be able to take time off to volunteer or shadow doctors. You probably won't be able to take time off to study for the MCAT, which means you won't be as likely to score as well as people that can afford to take months off to study like I did. You might to looked over for leadership, or get worse recommendation letters from people who hold implicit or explicit biases against people like you (ie, if you're trans and many people feel uncomfortable around you, or if you're black, or hispanic, or what have you). This is to say little of the tremendous initial costs of entering the field - applications, traveling to interviews, etc. Sure, if you get accepted and make it to practicing, you'll make plenty, but that's money tomorrow instead of rent today. To my mind, that all means that people who come from more marginalized communities, who disproportionately face discrimination and poverty, are less likely to enter medicine, which in turn makes it a lot less likely that they'll be able to turn around and treat low-income and marginalized communities - the ones they came up in - that disproportionately face health disparities today. I think that muddies the water of wanting medical schools to be competitive - sure, we want smart and empathetic people to become doctors, but there are a whole lot of very smart and empathetic people who don't have the resources, time, and energy to pursue medicine because they're already struggling to stay afloat.

Austin Budd I disagree with you here but probably not for the reasons you think. I think this is an example of the checklist mentality. You don’t necessarily have to go shadowing and spend hundreds of hours in a hospital to display your commitment to the medical field. Remember that he said that students should frame their application in a narrative format. Don’t you think that a person from a poor neighborhood, which has a high demand for physicians, is the perfect applicant for a medical school? Their narrative could be that they grew up in the projects and saw first hand how rapidly diseases would spread through the community and decided to do something about. They could show their leadership by talking about how they formed a small group of friends that went around the neighborhood and picked up trash around their houses and recycled. They could talk about how they counseled a smoker or a heroin addict to get them to quit even if their efforts failed. They could even speak about the needs of their community and how, by being a member of that community, are uniquely capable of solving those needs. Thinking that you have to shadow, get the best grades, and meet prominent doctors is the only way to get in is the thing that’s holding a lot of people back.

I agree so much with this!!!! We can’t take time off from life because we are already struggling. We try hard still and then comparing our stats to people who had the time makes us become insecure and not apply at all.

The worst part is that the lack of preparedness begins far earlier than college. It starts in grade school and trying to achieve college readiness while facing the challenges/lack of resources you explained above.

Jo Umero 100% get what you are saying and I think med school admissions committees agree with you. They see applications holistically. That said, the preparedness of underserved students is definitely less. I think they will still be the underdog as they go through medical training, constantly trying to catch up or keep up after having had a worse foundation in the years before med school. I feel like there might need to be a more overall approach to addressing this, starting with elementary, middle, and high school education to help students be ready to succeed in college and beyond. It’s an uphill battle...

If I had the chance to tell my old high school self 6 years ago where I would be today I would have done so and made it easier on myself. Although through soul searching as much as I love to help others in need I changed careers becoming a Doctor. I was a nursing assistant for about 5 years and got to work at the Veteran Affairs hospital in San Francisco. I loved helped them and was inspired to become a doctor because alot of UCSF residents and trainees came and I got to speak with them and network with them. But I knew I had a long way to go. I majored in psychology as well as a background in Information Technology. I enjoyed both a lot but realized even after my BA I still had to return to community college and take the science courses. That along with applying and everything else to me took more time and more loans. I had a debate with myself and had to think hard. I couldn't get into nursing first round because I feared that nursing is already impacted enough and now after planning on becoming a Doctor I began to think and wonder if I really could pursue. I'm low income and already in debt. Been watching other doc videos of the debt they ran in etc. $430k one guy ran in and dude was on the verge of suicide and dropped out of med school. Things like that made me wonder. I didn't want to pay for debt more than I could afford. Sure I could do the military or some way in but never join the military just for a benefit. It should be for different reasons on joining and a calling. But recently over the past month I looked back at my IT experience and sure enough I got a opportunity that I could not pass up and build my career with them. I was able to get hired at Google and now working for them. I wouldn't have thought I could even work or get into Google since it's so competitive and almost everyone wants to work there. But that was my experience on where I am today, I wish the best for all those who continue the path to be a physician. The road is long but is worth it in the end.

I’m glad you made the right choice for yourself. Yes, being a doctor is great, since but it’s a serious decision, especially with the huge debt, that one way or another has to be paid back, and can’t file bankruptcy for. My sister, wanted to be a doctor, but ended up deciding on nursing, since it’s less debt and she can immediately get a job after getting her BSN. Google is great company. My dream is too work at google as a software engineer.

I participated in AMA activities and I was surprised that alot of politicians did not know that we need to complete residency to practice. Some advocate to open medical schools and funding but didnt know that we also need to also expand residency funding.

Im interested in hearing Medicine in the old days vs Medicine now. I watched a documentary on Netflix about how the Mayo clinic was started I feel as if the reason why Doctors have to learn so much about the body and work hands on is because of the Mayo brothers as well as trial and error from the ones who cane before us.

I truly believe philosophy (1 year) should be apart of pre-med core classes. It can help create a strong internal foundation and promote healthier introspection.

Lucien Vaughn I agree! I’m doing coursework in philosophy and have got some plans to pursue graduate work in it either alongside or prior to my MD. I’m pretty confident it’ll make me a better physician despite some people’s skepticism.

I like how you laid it out. I definitely agree with the meat of the video about why it's currently so competitive and what the issues are regarding the physician shortage, but I have a differing opinion on whether the application process should be as it is currently and whether it should be so competitive.

i would like you to make a video on the application process, requirements and competition that an international student would face(for scholarships ) in order to get into residency and how students can maximize their chances of securing admission (in US). if you can devote a special video tearing down these aspects for us international students aspiring for USA it would be great help.

In India, med school acceptance rate is 3-5%. Every year around 20,00,000 students appear for the medical entrance test (called NEET) and only 55,000-60,000 get into a med school.

Luckily where I live and study, college education is free, which is Uruguay, and there isn't a max of places. When I started medical school in 2016 around 2000 people began that year, now we're around 700 usually it's the difficulty and pace that makes people quit. At least we have the chance to start, for free.

That's why so many people get their MD degrees outside of US, and then come back to take USMLE. Same time, many foreign med graduates are striving to get a position in US.

It is arbitrary how competitive medical school is. You could open more schools and increase residency positions. Problem solved. However, the shortage does keep wages artificially high for physicians. The shortage is hurting them in the long run with increasing autonomy among NP and PA.

I remember back when I was applying for Med school - all the hoops and loops. Now 5 years qualified I can tell you that the hoops and loops only get bigger when you’re applying for residency 😭

My roommate got 518 on the MCAT and had great extracurriculars but didn't get into any schools. I'm not sure why but I think it's because he applied kinda late in the summer. He's just gonna work as an engineer now. He would've been a great doctor, the system squeezes people out on the basis that there's just not enough spots. I bet the overall matriculation rate would be closer to 60% if there was enough spots for everyone who was qualified.

Could you do a video of just a thorough timeline from initially applying to med school to end of residency? I keep learning about x, y, z recommended timelines or time frames this will happen in undergrad, medical school, and residency, but I cannot visualize the continuous path.

Medicine pays well to some degree of due to the lack of spots available. If you oversaturated the market with doctors of all kinds then the salary would go down for many.

In India only 35,000 approx are selected from 1,500,000 aspirants .... sometimes some student do suside when they are not selected from 2-3 attempt later...😢😢😢😢

Australia also has similar issues re: lack of training opportunities after graduation. At some point there was talk about wanting to increase the number of GPs. Tony Abbott's solution was to open a new medical school in WA (Curtin Medical School) which would be focused on encouraging students to become GPs. This was met with a lot of resistance from the Australian Medical Association as it didn't address the real bottleneck: the lack of GP specialist training opportunities (because contrary to what many people think, GP is a speciality in its own right). I think from a politician's point of view, it is sexier to cut the red ribbon on a shiny new medical school building than to increase funding for a training program that many people don't even realise exists.

@@dancinkayley Dr Syl has a video where he talks about this exact point. Not enough spaces in the specialist training. I think I read that in 1 year they had 8 spots for vascular surgery training. Absolutely insane.

It is even worse in Canada. Only 10-20% get accepted into medicine each year. And tou know what's funny? Canada is very stiff with IMGs and most of the time refuse to give them residencies

This is not only is the US but all over the world a problem! But a place for a medical student cost more than 4 times that of nearly every other student! thats why there are not more places for new students!

Another problem is probably the medical school being a postgrad for premeds model, Why is it enforced? Maybe if you have a MBchB model you'd have more people because it would shorten the duration of study, and also maybe certain specialties should allow people to start in them earlier... prerequisites should be relaxed for people with great scores & ratings. And also open certain specialties to other professions that can do them... like nurses, obviously the entry requirements would need to be adjusted, but I think part of them problem is trying to monopolize/compartmentalize/have an exclusive club of people who can contribute to Healthcare.
