In my 20's I had cystic acne caused by very low thyroid. This was finally diagnosed and treated correctly by my psychiatrist! After seeing a GP and a dermatologist, who didn't take my concerns very seriously. The cystic acne cleared right up once my thyroid was treated. I took birth control pills most of my adult life which really helped with regular old acne (And Paula's Choice BHA too!) Thanks Dr.Dray for bringing up all these topics that we don't often know are affecting our skin's health. You are THE best dermatologist!!!

@@memoonasaeed4408 I was on Cytomel (generic is Liothyronine) for 25 years until I went through menopause and my thyroid went a bit haywire. Now I am on a combo of Cytomel as well as Synthyroid (generic is Levothyroxine). It takes awhile to get the thyroid readjusted so don't give up. My Endocrinologist was my doctor for the thyroid adjustment.

I was a Guinea pig when it comes to trying out different diets to control my hormonal acne. I did juicing, milk-free, soy-free, and sugar free. Hell, I even went to the opposite extreme and did the carnivore diet for 11 months! I was desperate. But honestly NOTHING worked. The only thing that worked for me was spironolactone, go figure

I suffered from cystic acne, and POCS for decades. I went to my doc and asked if he’d check my hormone levels because of my painful acne. I just knew my hormone levels were off. He actually told me “even if I check your hormones, and they’re imbalanced, there’s nothing I can do for it.” I was so pissed I got up and stormed out of his office, and never went back! I now take Spironolactone, Apostrophe prescribed me, I also use topical Tretinoin, and my acne is completely gone. My face has never ever been this clear. I had to be my own advocate for my healthcare, because most doctors will not listen to patients these days. Spironolactone has been a life saver for me. My face has never been this clear in my 48 years on this earth! Also I’ve noticed I don’t have as much hair shedding in my brushes or the shower drain. I also don’t have to shave my legs as often. It’s weird how it will allow more hair growth on the head, but less hair on the body. That’s weird. Why is that? I haven’t had any side effects from Spironolactone whatsoever. I highly highly recommend it for women suffering from acne. If only my previous, arrogant doctor would’ve been smart enough to prescribe me Spironolactone 😡🙄

I've cyst acne and I even went to doctor but he just gave me few creams, face wash & ointments but still my face is same with most filled acnes & now I found solution for my prblm bc of ur comment & so I wanna know I'm 19 now so can I use it? & does using spironolactone can affect any contraception (or will it lead to infertility) in future? Or is it safe??

I was diagnosed with PCOS when I was 13, and my dermatologist was the doctor who saw the red flags because of intractable acne. Even multiple trials of Accutane were unsuccessful, and she sent me for more work up when labs showed the excess of androgens (along with the hirsutism that was readily apparent, amenorrhea, etc.). Grateful to her for following the symptoms and referring me to pediatric endocrinology.

I would love a video on which hormone’s & how stress can effect and cause hyperpigmentation- not just oestrogen or thyroid hormones. Adrenal fatigue- how low cortisol triggers a cascade which ultimately can result in hyperpigmentation . Or how Nutritional deficiency can be associated with it too. Thank you so incredibly much again for all your work and videos!

I began dealing with hormonal acne around the age of 20 out of nowhere, and have been battling it on and off ever since. I had luck a few times with topical treatments, and sometimes it just magically going away for a small period of time, but nonetheless it would always come back. I got tested for PCOS which came back negative, and nothing else hormonal was ever found to be a possible culprit for me, although I have always struggled with a few vitamin deficiencies and unrelated mental health disorders. I’m currently on birth control (Yaz) for it and also for Premenstrual Dysphoric Disorder, and it seems to be helping a lot, at least significantly better than anything else I’ve ever tried. Because I struggle with PMDD and never knew it until a year ago, or that the disorder even existed, I do overall believe I’ve always had undiagnosed hormonal issues.

A lot of problems that cause unhealthy hormones is due to diet that was my main cause of why I developed the worst case of cystic acne ever when I was in my early twenties and I wasn't menstruating, I was diagnosed polycystic ovarian syndrome when the real root cause of my acne hormone problems and no menstruation was not eating enough therefore not getting enough healthy fats not getting enough proteins , I feel let down by the medical system because instead of trying to figure out what was causing my acne and amenorrhea I was only prescribed birth control instead of trying to get to the root cause which is why I have osteoporosis today, having unhealthy hormones can also cause osteoporosis it's very important that we look into our diet cut out all unhealthy foods focus on healthy fats protein and carbs and manage stress because I was also suffering from PTSD when I was in my early twenties I'm sure that also contributed to my severe cystic acne

@@parinazaz4044 I'm atheist but I understand your kind words are coming from your heart, we definitely have to love ourselves and rely on the people that love and care about us, love ourselves enough to seek help investigate do research if doctors are not helping us well We can seek natural treatments or find other doctors who will listen and take the time to treat us and look for the causes not just put bandages on the symptoms

Actually, polycystic ovarian syndrome is treated by healthier eating habits and regular exercise. Other things like hormones and laser can be used to help improve symptoms, but they do not treat the syndrome. Doctors should have told you that.

I hope you don't expect Dr. Dray to be a substitute for making your doctors pay attention to your concerns. You must be acutely attuned to your body and keep your own accurate records of when and how hard symptoms hit you, and you must present these at every doctor's appointment. I dealt with PCOS for all of my "fertile" years, and while the diagnosis was general and all-encompassing, the treatments most certainly were not. I suggest you insist on referral to an endocrinologist or a gynecologist at the very least, and if you go gyn, do NOT go OB-gyn. You want a doctor who will focus on making you as symptom-free as possible, and an OB-gyn has been trained to focus on fertility and will counsel you to be tough and willing to put up with pain and a little hair on your chin, because hey, the ultimate goal in their minds is always to preserve fertility. So don't go OB-gyn. You'll be on a rollercoaster of emotions and mental anguish, as you go through many types of birth control pills for the hormonal benefits, only to end up with IUDs because the hormones of pregnancy can destroy whatever progress you may have made. IUD insertion and removal is never done under anesthesia or sedation but is very painful. You could end up on depo-provera, which thankfully stops your long and painfully debilitating periods for awhile, but then you may need to go to an ER for pain control when the required occasional withdrawal from depo-pro causes crippling cramps, insomnia and lightheadedness. Or you could start to get male pattern baldness or hair around your nipples as a side effect due to testosterone overdose. I was so happy when menopause came; it turns out that two of the drugs I'd tried during active PCOS times are now prescribed for Type 2 diabetes and hypertension, likely caused or at least exacerbated as a result of the hormones I was on during PCOS treatment. It does not go away, and you will live with aftereffects for the rest of your life. So getting a specialist on board now is crucial to any success. Please find a good specialist.

I was diagnosed with PCOS about 4 years ago. I've been on Spironolactone since. My cystic acne disappeared right away. I was also suffering from excess facial hair and hair on my neck. That is no longer a concern. Laser hair removal took care of the old hair and there's been little to no new growth since. I started with a 25mg a day dose but am now on 50 mg a day. Spironolactone has been a godsend for me.

@@elifarslaner7364 I've been using the TRIA at home laser. It takes diligence and patience but it's convenient because I can do it whenever I want, as many times as I want. 95% of the hair on my arms legs and underarms is gone. The facial hair is mostly gone as well but from what I've read it never completely all goes away because it's more susceptible to hormones. I've had the tria for nearly 4 years and still use it for maintenance. If you've got the patience for it, it's excellent.

Spiro gave me extreme dizziness & headaches. I drink a ton of water, I don’t drink juice, soda etc. all I ever got told was to drink more water.. which for me was frustrating because I drink a ton. Wish it worked for me. I’m becoming depressed over not finding something that works.

I’ve had hormonal acne since I was 19 and was recently diagnosed with POI. I was wondering if you could discuss any correlation with POI and hormonal acne. Most videos I come across discuss high testosterone, but I very rarely hear about low estrogen causing acne. Thanks Dr. Dray!

Every hormonal lab I’ve ever had has been day 18-21. Except that my cycle has NEVER been 28 days. I took progesterone for years and had negative side effects and it took ME figuring it out, and I was working with a functional doctor. Doctors just don’t talk about the things you’re talking about!

I was growing lumps under my neck, tired and sleepy and at first getting pregnant problems. I went to an endocrinologist after being told by a nurse who was practicing as a Gp/ doctor I had a slightly low thyroid but she wasn’t going to treat it right now. My growths got larger, my hair was coming out and I wanted to eat and sleep. My sister said go to an endocrinologist. As soon as I did I had to have surgery to remove the lumps to see if the thyroid growths were cancerous. I didn’t have cancer but I barely had any thyroid. I was placed on Synthroid and after several weeks I began to feel better. I became more energetic and I ate less and the goiters in my neck never grew back. My hair started growing back and the Levothyroxine improved so many things. I still have some thyroid left but not enough to substain me so I will take Levothyroxine or Synthroid for life. I’ve had two small increases. Never trust or stay with someone if they appear to be afraid to treat something that is missing or needed in your body.

Wow that was so educational thank you! I have 2 adult daughters 20 and 24. The 20 yr old is on the final dose of Accutane and her cystic acne was painful physical and mentally. She’s also is sensitive to birth control and it helped a little. My other daughter strangely started lactating slightly and tests revealed a small pituitary tumour. Her prolactin levels were fine but thank you for mentioning it should be done at the right time of month. I’ll ask her about this.

I got hormonal acne at 35 from extreme life stress and the nodules were huge and painful. The only thing that works is spironolactone, but at very high doses. I take 200mg religiously and my acne is completely gone. Like, nothing. My confidence has soared. I experimented with lower doses over the past several years but this is the only thing that works. It’s not covered by insurance so is a bit pricey but it’s a small price to pay for clear skin.

In high school I developed dark course hair on my stomach and I had my blood work checked but apparently it came back as normal. Maybe some people are just hairier. I always had oily skin, hormonal acne, have hairy arms and I have a high sex drive. My dermatologist thought there might be something hormonal also because I had gained weight. But the tests came back normal.

Exact same!! Several drs and many years of tests and failed medication have all said it's because I'm from the Iberian peninsula, which is Spaniard and Portuguese (Mediterranean). Nothing they can do to combat my acne because most of it from my genetic hairiness irritating my pores. I also get terrible bikini bumps when I shave... Not even spironolactone had helped UGH

I have been suffering wild moderate acne on jawline and chin since I am a teen...I have a good life style and diet, no particular stress. Recently had hormones blood work, nothing came back wrong, no pcos. Retinoids (differin/epiduo) have helped me a lot but still break out often on those areas, however the spots are smaller and go away quicker and the skin has improved very much.

I got off birth control after being on it for 15 years and I now have the worst cystic hormonal acne I’ve ever had. Now I’m pregnant, and it’s only gotten worse. I’m hoping it gets better. My face hurts ☹️

I'm in a very similar case, was on the pill for 10 years, came off it, am now pregnant and very bad acne on my chin and jawline. Nothing works! I have tried soooo many products. Visibly the issue is coming from inside. Did you find anything that helped?

I have suffered from hormonal acne since my teens and have taken accutane multiple times. My doctor had me try Spironolactone and thankfully it keeps my skin acne free.

I'm tired of "holistic health experts" on social media diagnosing everything as a hormonal imbalance. And then telling people how to "cure it" with their stupid diets and workout routine cycles and herbal supplements. I'm tired of it, and tired of them getting away with it even though there is NO basis for what they're saying.

I've been shedding hair, have very stubborn spots around my breasts, back and chest. I've noticed my body hair is also coarser and darker. I really hope I can be seen and taken seriously for this. I might have to pay for home tests as the NHS in the UK is extremely stretched and won't normally test for this 😥

OMG, I have these symptoms, I have non-scarring androgenetic alopecia and i have hair growing on my chin and upper breasts. I've been to my doctors 4 times and they just tell me im fine and they cant find anything wrong with me. I was told by dermatologist that I have low Vitamin D levels, I was told by my PCP its nothing to worry about. Also im black.

Thanks for another great video ❤️! I think a fasting insulin blood test would provide much more valuable information regarding PCOS then fasting glucose

Absolutely check fasting insulin level! The majority of American adults are have chronically high insulin levels and are insulin resistant. Few doctors order this informative test, unfortunately, but IMO a fasting insulin test should be included anytime CBC, COMP Metabolic panel, and Hemoglobin A1c test are ordered, at the very least. Also the reference range for the fasting insulin test has a much too high upper reference range. Optimal results are on the very low end of the reference range.

I started Spironolactone since July and it has cleared my hormonal acne. The only negative is how often it makes you pee since it is a diuretic but my skin is clear.

Yeah, Every month around period time, my face would breakout, headache and depressed. I took birth control pills and it all went away. Loved it. Then news came out saying it can cause blood clots, so i quit it. This was early 70's. My name is Karen, Edward is my son. In think its inherited because my daughter goes through this every month. I had two female friends and they never got acne.

Video request: visible veins- topical, surgical, behavioral, and oral therapies (including supplements). I mean VISIBLE, non-varicose veins (e.g. Madonna's hands). I know this is really vascular, not dermatology, but maybe you can chime in??? Everything online is seems to deal with varicose/spider veins (that can be a good video), but I'm talking about visible, large veins related to exercise. I'm not the only one who has them, you have one on your right face, so you know the struggle!

I second this. I never really see anyone talk about visible veins on the face and/or body if it isn’t spider veins or varicose. I am very fair-skinned and have visible veins pretty much everywhere on my body, including many on the face. I know it’s not really a dermatological issue either but nonetheless I’d still be interested in a possible video.

@@db-gb5xi Ok but as far as I know visible veins are not the same as varicose veins. perhaps the treatments are the same but I don't know. The literature is wishy washy on it. I'm concerned with visible, non-varicose veins.

So odd because bloodwork confirmed my only hormonal imbalance is super low testosterone and during the time this started is the only time in my life I have had acne.

ive had hormone tests and my levels are fine, yet the only treatment that my skin responded to WAS hormonal (BC & spiro). do you know what's going on here? ive been dealing with jawline cystic acne for over 10 years and nothing works except birth control and spiro, but i get really bad side effects from both :((( it's so frustrating. the ONLY abnormal results i had was high cortisol and low DHEA. things ive tried: - diet (no difference) - topical treatments (no difference) - birth control (completely clear - but bad side effects like severe depression, headaches, brain fog, etc) - spiro (small reduction - major side effects because i have very low blood pressure already so it was making me ill)

Not a doctor and hope you find the right treatment soon. In the meantime, have you tried drinking peppermint tea? Some women find improvements in their acne drinking a cup or two a night. It definitely isn't a guarantee since it only addresses androgen, but it's worth a try unless you're allergic. I find it really relaxing to drink before bed since it has no caffeine.

Just because your levels are normal by lab standards doesn’t mean they are normal for YOU the labs help see what’s going on but a good doctor knows that if symptoms are present it’s a good sign that something is up. Green tea has always worked really well for me for hormonal acne make sure it’s organic, also dandelion tea which helps the liver detox.

I was on accurate 20-21. Recently I got bad cystic acne on my chin. I keep my skincare super simple and added a retinol. Finally after almost a year my cystic acne is “under control”

Wow I’ve always had irregular periods growing up. I took so many pregnancy tests, I should have bought stock. Facial hair, which gets worse as I get older. But my poor daughter is 19 and has ALOT of facial hair. I think we will both get tested for hormonal balance.

My 2 sisters and I all got very bad acne on our cheeks and chin when we got into our 20s. My mom had it the same so I think we just have genetically sensitive skin to hormones... bc fixed it for all of us but I'm getting off of it cause of the side effects. Starting tretinoin again at a lower dosage after failing it a year ago with twyneo (skin was in a ridiculous amount of pain)

I’ve had weight gain (20 pounds) over half a year, hyperpigmentation covering the sides of my face despite topical prescription treatment, and surgery for polyps found on my uterus. Huge change in periods- clotting, more pain, heavier. Could any of this be related to hormones? I can’t find a doctor to believe me that I’m simply not overeating- I’m not at all whatsoever. Im exercising too. I feel like something is happening to my body that’s not within my control

I've had acne growing up and still as an adult. Only thing that cleared it up was going on pill birth control and honestly why I am still on the pill. How can I go off and get treated from over-the-counter stuff or even stuff from a dermatologist when its hormonal is how I see it. Medication and such has side effects and since I know how I do on pill birth control, why try to do something new where I don't know how I'll react to it and stuff. Although I've always had regular periods and none of the other issues you mentioned, I don't think my hormone levels are too off as my acne was never as bad as cystic, just always 1-3 pimples on my face, usually on or around the chin.

I had a test show that I was low in free testerone and I started taking DHEA to see if it would help before my dr gave me another prescription and I feel so good! I started out with 10 mg on my dr recommendation and for now I think it working great.

I have all of these symptoms plus a deep voice, back acne and excessive chin hair, but I've had two hormone tests from my GP and the results keep coming back as normal. I'm constantly blotting oil off my face and I'm not sure what to do next.

Thank you so much Dr Dray for this video, I've been suffering from adult acne and hair loss but I've been hesitating of taking contraceptive pills because I'm afraid they will make my hair loss worse, because that's one side effects from some contraceptives. I had my gallbladder removed and ever since then my acne has gotten worse and my face now grows thick black hair . I have tried to talk to my doctor's about this but they think it's normal. I have constant headaches. No one is taking me seriously. I have taken BC but it makes my headaches worse . I feel drained constantly

I am currently on Accutane for my hormonal acne and it was the best decision I've ever made! side effects are not that bad I was on spironolactone before and decided I didn't want to take a pill forever.

Be sure to find a doctor who will actually listen to you and your body. I’ve had doctors tell me that because my cycle is regular that my hormones are fine, but ever since I went on birth control and then off of it my body has not been working the way it used to. My current doctor took into consideration my current problems and my history and was able to determine a hormone imbalance. Doctors are so quick to put you on birth control or dismiss your concerns because the results come back within the ‘normal’ range. But they may not be normal for YOU.

Helpful! I’m experience extreme hair shedding. They seem to be not breaking but coming out from the root. I have lost weight over the past year that could be one component. Another issue is stress. I’m going to try anti dandruff shampoo to see if it helps. I have significant inflammation in my body and skin. Not sure what else to try

I have onset of adult acne- didnt even have any at all in high school etc. I have high DHEA-S which my end says is indicative of PCOS. I have a bunch of other symptoms too which I thought I was peri-menopausal

I’m polycystic and made the mistake of getting a uterine ablation to get rid of repeated cysts and fibroids. I also had cystic acne but cured it with activated charcoal soap. Very little hirsutism. Regular periods until the ablation. Now, at 62, I have no clue how far into menopause I am. Have had one hot flash. Dumb!

Oral Spironolactone will help your cystic acne, with less side effects than accutane. I’ve seen a lot of people online complaining about the awful side effects with Accutane

Diane fixed my acne 30 years ago. I had a mild policystic ovary. 6 months of the oestrogen pill and gone for good. My daughter had the same problem, and much as I insisted the dermalogist he did not want to send her for an echography, which is the fastest way to diagnose the condition. He insisted on keeping my daughter on oral retinoids. An echography is the best test to diagnose PCO

I also took Diane… my experience was horrible 😢 I started to breakout (my skin was clearer before from taking accutane) I felt dizzy and almost every night was throwing up and I felt depressed I stopped the treatment and my skin is improving again although I have the scars 😭 and the best thing is that I don’t feel like distracted or depressed, was weird…

I recently broke out the worst I have. I highly suspect I have pcos. I have been losing hair, jawline cystic acne, and irregular periods. I have an appointment to see a gynecologist and I’m hoping I can get answers as it has been a long journey to feeling like there is no cure. I haven’t tried Spironalactone or BC but I have tried winlevi and currently on doxycycline.
