I was once diagnosed with chondromalacia patallae, caused not by misaligned tracking of the knee cap but by "lazy" (slow) tracking, due to misfiring of the quads caused by a "slack" rectus femoris; at least that's how the physiotherapist explained it to me. The doc felt the roughness under the knee cap by rubbing it firmly from side to side while my leg was flat on the table. Luckily for me, I was pain-free in about week after doing straight leg raises with a few pounds strapped to my foot. Prior to being told about the leg raises, another physio diagnosed patella tendonitis (the pain is usually in the same area) and told me to rest, ice and stretch, but that treatment did nothing after a month. I could be wrong on the following, but: I suspect it's better doing the leg raises while lying down; in other words, starting with the leg lined up with the upper body prior to raising it. Either that, or standing and kicking out from a weighted wall pulley that's close to the floor. I guess that doing the movement this way doesn't tighten the hip flexors too much, whereas standing and kicking up passed 90 degrees might, due to the extra hip flexion. My injury occurred about 25 years ago, and sneaks back in minor way every few years, at which time it quickly goes away after doing the leg raises again. Yes, I probably should do them all the time, but, well, you know...life sometimes gets in the way. :-) In my humble, uneducated opinion, the "regular" chondromalacia that I had gets overlooked too often in favour of misalignment (PTFS), and I'm not sure why.

Many thanks for your informative and entertaining set of videos. A doctor referred me to an NHS physio who got me standing on the leg with the bad knee with my eyes closed - didn't work! Then I went to a chiropractor who wanted me to have expensive treatment for 6 months. All this took over a year. I have tried your test, exercises and taping method and my knee has stopped aching in 3 weeks. I am so grateful, and If any pain does return I now have plenty of tape which I ordered on Amazon.

I saw the title of this video and it sparked a memory and I remember my doctor suggesting that I had patellofemoral syndrome back when I was a kid. The mention of pain going up and down the stairs was spot on. I will keep this video in mind when I see my doctor next time and maybe it will put me on the right track to solving my knee issues.

Thank-you very much. I have a bit too much fat around the knee, so taping it wouldn't work - so I tried another approach with a patella knee strap but with a tightly rolled wad of paper towels (about 1" in diameter) stuffed between the strap and knee on the outer side of the knee cap. This shifted the knee cap over to the inside and solved the problem 100% walking downstairs. The pain didn't go away totally as you say, but it's not nearly as bad. I then pulled a knee compression sleeve over the entire knee to keep the knee strap in place.

Hi Bob & Brad! Great video, thanks! Hoping you guys still check out these comments! My question is, you mentioned most of the time the knee cap may be misaligned on the lateral side of the knee so you want to tape it and pull it towards the medial side. However, how do we know which way the knee cap is actually misaligned? Mine feels like its jutted more on the medial side but I'm hesitant to tape it as I'm just not sure.

I want to know whats the criteria to know WHEN its time to have surgery. My knee buckles at times, I can no longer squat at the knees and come up without pushing down on the floor with my hands to help getting up from a squat position, and I have pain when I bend at the knees. I don't take meds YET for the pain . Luckily it doesn't hurt to walk so I'm still able to golf. I want to know more about the surgery and hear testimonials from people who had this Chondro and how their knees feel AFTER surgery.

How about Testing with the Knee Brace? I am 28 yrs old and Physical labor. My doctor said it's the cartillage under the patella. So I guess it's PFPS. He checked it by pushing the knee cap downwards and asking me to bend. Than there was a locking sensation by which he confirmed its PFPS. Is this right?

Me, too! My PT sessions ran out , leaving me only 1/2 thru my rehab. Only 75% functional. 2x a week. Insurance covered 16 sessions. Thsts it. Too bad. No more, gotta go. :-( I dislocated my patella. I'm a tour guide in NYC so walking & stair climbing is my livelihood. I must COMPLETELY recover. I've watched every video. But my therapist gave me no further instructions. He taught me how to walk UP stairs in 6 weeks. YAY! But what about the other part? LOL Never got to the down stair therapy session. WTH?!!! After continuing my daily exercises, devoted!!! 2hr sessions. I desperately knew something was missing in my rehab. I'm up at 4,5,6am watching videos for answers & solutions. & HOT DOGGIT I knew i could count on my boo thangs * Bob & Brad* to get me together! They're not the most famous therapists on the internet , for nothing!!!! I believe after I try this tape thingy, I'll be on my way too 100% The injury happened 6months ago. As of now, 24 weeks post injury ( no surgery BTW)Now I can do everything , pain free , even sit in a low sofaseat for hours due to my diligent exercises and drills and TENS machine daily. Thank the Lord!!! But I still can't run -at all, go down stairs, and only dance I can do is electric slide. Basic. BUMMER! I'm a trained dancer, and fierce Lindy hopper. I won't try any stunts or any acrobats but I would like to at least dance like a sexy momma in the dance floor, again :-). Also, I see incline walking is needed, as well, to stop my knee from free falling with i step on the decline. I wish i could get a facetime with you guys. You rock! You're an everyday blessing in everyway!!! Thanks again for being "ON IT"!

So if you do the taping and of course any exercises and stretching for this kind of pain, will it eventually go away permanently? Or is the damage under the kneecap going to keep it from getting to 100%? I got very vague info from my doctors on this, didn't really describe how it came to happen etc, and pretty much told me you can do exercises but will just have to deal with it. Not particularly something you want to hear, especially when it can get very painful... (That and the fact that they did not do the scope for my knee, so this is more of, what they think I have)

Sara i went to the doctor the 17 of january, last month bcs of that (excuse me) fucking knee pain, and the doctor told me that he thinks that i have “patellar tendonitis” or Jumper’s knee. He told me to come back if the pain hasn’t gone away in a month, so i was there today, and i’m gonna get an x-ray tomorrow, and i’m maybe going to an specialist (orthopedic surgeon). I’m in pain every day, and i hate it, bcs i can’t do the things that i love, playing soccer. I went to soccer practice last night, and i got so much pain in my knee, and it felt like someone stuck a thick needle right over my knee cap. 🙄🙄🙄

I sense pain in the inner side (medial side) of the leg of my left leg patella. It's only when my muscles are tightened (generally while walking). Its only while the angle made by the leg is tipically 10-20 degrees... Can I get some info of such case.. please?

Hi chaps im going to try this. However, is it possible that my patella could be tracking the other way? As in too far to the inside? I ask because i can barely push my patella over to the inside let alone tape it there. Also i feel like ive caused this due to putting pressure through my toes and balls of my feet during squats instead of my heels. This makes me think ive strengthened and tightened the tear drop portion of my quad and neglegted the outer portion. Ive been doing this with over 360lbs on my back. Ive corrected this technique and im hoping im on the right track but would appreciate your advice. My pain is right on top of my patella and hurts most during squat and dead lift but also hurts just getting up out of a chair etc. Thanks for the great video chaps.

I have had surgery for patellar retinaculum lateral relaease about 2 weeks ago. Now my knee bends to a 90 degree angle. Can you suggest me some excercises to bend it more and more? Thank you so much.

What if the knee dislocated to the INSIDE? I dislocated my kneecap 5 times (over the course of 15 years) to the inside, and i had MRIs done and supposedly i have Grade 3/Grade 4 Chondromalacia (no ligament damage or anything else) but i have no pain on my knee AT ALL during everyday activities and im 30 years old. I got up stairs every day and i dont get any pain. I get pain once every month in which i cannot straighten my leg because of the horrible stabbing pain behind my knee but it goes away after a day or 2. I cant run or take sharp turns obviously or else my kneecap can dislocate pretty easily.

I put leukotape on by keeping my leg straight, pushing my kneecap over, and taping from the outer edge down over the inside of my knee like in the video but the second I bend my leg it rips right off. Am I putting it on wrong?

I asked before BUT NO ANSWER ...what about no pain when walking or bending. Or exercising ..severe lightening pain whenever it wants..laying..sitting..sometimes standing.last 30 sec. Or so..sometimes everyday sometimes not...nothing therapists have done to knee has caused pain..no clicking..or catching..only severe quick sharp pain out of nowhere

ok, first I want to say you guys are GREAT. I love your videos. My issue is as follows: 1. I have little to no movement in my patella. 2. fluid in mostly my left knee. 3. I went to a specialist and he used a long needle to draw some of the fluid for testing. I now have a new issue. pain in my left shin. Is it possible the needle did something?

Been looking for a video by you both on Posteria pelvic tilt. I am female,5 ft 8 inches tall and have a flat sacrum, short pelvis and long spine. I am now 82 and after a lifetime of standing straight and walking a lot, I now bend forward from my lower lumbar. I can’t walk any distance anymore. Recently I found a bump in my spine which pushes my spine to the rear. I found I can pull up with my upper back and feel the bump disappear. I can then walk as long as it stays in place. I was wondering if you could do a video on this which I think is posterior pelvic tilt but since it’s a self diagnosis, I wouldn’t bet on it.

Hi, MRI report: full thickness fissuring of LEFT medial patellar articular surface indicative of grade 4 arthritis. The cartilge deficency extends over a distance of 14mm in the axial plane. Clinical symptoms: I am unable to squat beyond 90 degrees causing severe pain. I cant kneel on the floor. Pain is severe while bending knee whilst weight bearing, nil symptoms when bending knee freely. what will happen to me in future? im doing my rehab exercise but im scare

I ran this test on my left knee, but I find that pushing the kneecap to the outside (to my left) actually makes it smoother. Does that just mean I have a swollen Patella? This all started about 2 weeks ago, leading up to my meniscus surgery on the RIGHT knee.

Hello, what does it mean if it cracks only when going up but never when going down stairs? I can also push my knee cap with the web of my hands (towards the shin bone) and it makes a crunch noise but no pain. I find stretching my hip flexors and rectus femoris temporarily reduce the sound. Ive gotten an xray before and my knee seems to be tracking correctly (from side to side) so it may be upward and downwards tracking issue. My PT said i have runners knee but i no longer go. Also, i heard xray cannot determine whether you have runners knee, that is cause i have maltracking up and down and not side to side right? Is there any taping recommendation? Is my knee aligned too high (towards quads) or too low (towards shin) (idk if u can tell from the fact that my knee only cracks going up but not down as i am unskilled) Also the place it is cracking is very close to where the patella and patella tendon (if that can give you any clues onvwhat my problem is...) Any feedback is appreciated very sorry for long comment have a good day and thanks :) (im 16)

Would be good to consider/discuss that we don´t always move/bend our legs in the "perfect scenario", i.e straight line and to the front (which would provide perfect tracking of the patella, in theory at least). Consider soccer movements: tons of out of balance, multiple direction, one leg stands, through a game. Anyway, to anyone reading: stretch calves, hips, hamstrings, quads, ankles and focus on glutes strenght (main culprit, consider how much time we spend sitting).

I have swelling on the inferomedial border of my patella as well as fluid behind the cap, very difficult to extend my leg completely and walk down stairs... Don't recall a specific moment when I hurt it, just started getting sore at a certain moment while rock climbing. Do you think it could be this issue?

Hey guys, I hurt my knee at football practice, and I just felt a small pop, no trauma or anything that I remember. It swelled a littler like a day later, but nothing alarming. I do have pain when I squat, climb/descend stairs, and when I bend the knee while walking. I am wondering if I could ask you guys if you have any idea what it could be before I head to the doctor? Thanks
