I am an internist and my cousin is a dermatologist. I went to do a rotation with her during medical school (she entered medical school at age 26, I entered at age 38 and we are the same age so she was in practice for a decade before I started training). One patient of hers we saw had diffuse sun damage. I was studying cardiology at that time and I exclaimed when we left the room, this patient has SKIN FAILURE (thinking of a heart failure analogue). My cousin said "YEAH, EXACTLY - I am going to USE that!" Through the years we have thought of the outcome of excess sun exposure as skin failure. Today due to you two I now have a new word to describe the lifetime etiology. "Exposome." I am going to USE that. Thank you!

Maxfield and Shah to answer in more detail as they are the dermatologists, but what I consider to be symptoms of "skin failure" would be diffuse sun damage frequently leading to brown age spots (like seborrheic keratoses) , fragile skin of age with chronic breakdown - the actual medical term for this is "senile purpura", appearance of numerous skin cancers. It is one of those things where "you know it when you see it." This is the kind of patient who is frequently seen by dermatologists and tends to have had significant sun exposure over a lifetime.

Quite the veteran of Mohs procedures for basal cell skin cancer. I appreciate Drs. Maxfield and Shah offering input into skin care and preventive treatment. My own personal exposome has its own resume!

Actually this Vichy probiotic fractions would be good for all of this and also put your favorite cream on top. Tolerance Control cream from Avene is good but la Roche Posay makes a good one too called Toleriane Sensitive Cream.

I've been using it for a long time as part of my tretinoin routine. anyone who uses tretinoin understands why simple formulation is the best option for irritated skin. but it's nothing new, let's mention avene and la roche posay, biotherm, SVR or Uriage. they've had probiotics in cosmetics for a long time. Vichy has too many preservatives and fragrances in cosmetics which irritate my skin ( that's my experience). only a few products are gentle to the skin during tretinoin treatment, including this one. I recommend this one for sensitive or irritated skin especially during treatment with tretinoin.
