My father, Jason Miller was a pilot for First Air for decades, he knew these pilots personally and he was devastated when he heard what happened. When the Mayday episode came out it was saved in our vcr for about a year before he could watch it and I saw how sad he was after the episode was over.

I watched the full episode and it’s so sad that the First Officer kept trying to warn the captain that they were off-course and wanted to abort landing and re attempt. Such a tragedy that probably could have been avoided. RIP to all the victims :( I guess the only bright side from this incident was that changes were made regarding training and how first officers can take control.

The pilots did receive CRM training but TSB found that the training was outdated (the airline was still using the CRM training package from the 1990s which didn’t include any modern CRM strategies or simulations etc) and also that the training was incomplete (the pilots were only trained for 4 hours when usually it would be a full 2 days).

Yet another accident caused by bad design of Boeing 737. Autopilot goes off without bells and whistles? Really??!? If that's not a design flaw, then what is? Instead of a voice warning saying "autopilot disengaged" (with bells and whistles) it just turns off one little lamp, out of 500 in the cockpit. Fantastic solution. Compass is off by 17 degrees from GPS and the airplane is not reporting that GPS and compass bearings are different? Those two fatal design flaws have crashed this airplane. The captain is not innocent, but he was misled by the instruments. Boeing didn't do their homework. No wonder that all Boeing 737 Max-8 are now grounded. Different aircraft series, but the bad design solutions continue.

He just didn't take a step back and assess what was going on when he heard the concern, which he should've done, but can't be expected to have done if CRM wasn't properly trained. And no, I don't use the term "pilot error", because only gross negligence can truly be considered to be the fault of the pilots.

Not really. The FO warned the captain 18 times that they were off course and needed to replan the strategy but he did not listen until the GPWS warned them, which was too late. You don't ignore another person 18 times...

Due to the poor vision, the pilots couldn't tell where the runway is. When the master alarm sounded, the pilots thought they almost at the runway when really they were off. If the pilots acted quicker and performed a go around, maybe this flight could've been saved.

To be honest, they should have aborted when the co-pilot originally expressed his disagreement with the approach, but the pilot refused. The time between the call to go around and the plane crashing was about 4-5 seconds, so this was drawn out for TV (i.e. detailing short times as if they happened longer). The 737-200's JT8D engines also have a tendency to take more time spooling up.

The reason the captain was slow to respond was because that he was confused. He was sure he was lined up the runway, so of course he was slow to respond. Also, we know (from reading the title) that the plane is going to crash, but he didn't. To him, this could just as well have been an instrumental failure

When you hear a GPWS "pull up", the only thing you should ever be doing under any circumstance is to immediately initiate the terrain escape maneuver. That involves increasing thrust to maximum, pitching up to near maximum AOA, retracting any speedbrake if using them, and rolling wings level. Turning is not recommended. Turning decreases climb capability and without seeing the terrain might not help (keep in mind a commercial aircraft takes a full minute to make a 180 degree turn).

Apparently, at the arctic pole, they neglected to frequently recalibrate the magnetic compass. They were NOT where they thought they were!

Well that’s a part of it but not the main reason. They thought they were doing an ILS approach but one of the pilots accidentally bumped the control column changing the autopilot mode. This coupled with the wind pushed the plane further east and caused them to be flying parallel to the runway but far from it. The copilot expressed concern for the landing path which the pilot ignored until the ground proximity warning triggered, but by that time it was too late

@@3chords490 That's aviation. That's how we communicate, its the names of the systems. He is referring firstly to GPWS, Ground Proximity Warning System, thats the "pull up" sound you're hearing. It warns of being too close to the ground. He also refers to TOGA thrust, (Takeoff/Go Around) the throttle position the aircraft needs to take off and climb. Although a 737-200 likely won't have a TOGA setting like a modern A320 or 737-Max.

If you hear a low altitude warning, especially in IFR, without question it's time to immediately perform a go-around and figure things out at a safe altitude. Better to be delayed landing than crashing.

+Bleach Anyone? Well, it wasn't his fault. The autopilot switch modes to manual at a certain point causing it to be roughly 7 miles parallel to the runway. The copilot suggested to go around but the captain didn't care so when the gpws(alarm) goes off, they go full power but it's too late.

Blunt force trauma. The bodies just couldn't handle the sudden deceleration from the sudden impact. Think of it this way; cars in the 1950s and 1960s were quite tough behemoths. Yet when there were crashes, occupants were often found dead, despite the vehicle being in relatively good shape. That's because the shock went through the car and into the occupants. That's why cars have crumple zones, to absorb impacts. Obviously planes can't have crumple zones.

AWC Not to mention no safety belts( or just waist ones) no back neck brace and the fact that most people didn't wear safety belts even when they were available ( same today)

Taha Sidd this is a copy of my comment: On 25 March 2014, the TSB issued their final report on the crash.The conclusion of the investigation was that the aircraft missed the runway centerline signal during the autopilot-managed turn to final approach, but an inadvertent movement of the control column caused the autopilot to switch modes. As a result, the autopilot did not continue turning to re-intercept the ILS signal and instead levelled out. Due to being oversaturated with work in preparing to land, neither pilot noticed the change in the autopilot mode. The aircraft had a malfunctioning compass providing incorrect readings to the flight instruments (although the onboard GPS and ILS were providing correct information), causing the pilot to believe the aircraft was flying more toward the west and would cross the runway centerline again and re-acquire the signal. In fact the aircraft was flying roughly parallel to the runway and a wind from the southwest was pushing it even further to the east. The first officer raised the issue of an incorrect heading and suggested a go-around multiple times, but the captain ignored his suggestions. An attempt to make a go-around was initiated when the ground proximity alarm sounded, but there was insufficient time to gain enough altitude before impact. The aircraft was operating as a charter flight from Yellowknife Airport, Northwest Territories to Resolute Bay Airport, Nunavut. It was carrying four crew and 11 passengers. The aircraft crashed on approach to Resolute Bay, killing 12 people. The weather at the time was reported to be poor. The Canadian Forces were conducting Operation Nanook 2011 nearby, which was about to simulate an air disaster in the Resolute Bay area and Royal Canadian Air Force firefighters were among the first to respond and reach the crash site locating three survivors.The fire chief, designated Red Leader, was informed that there were 15 people on board the aircraft. Red Leader reported that 3 survivors had been located and the search for others was being organized in accordance with rescue procedures. The other crew and passengers were found to have died on impact.The operation was suspended and those taking part assisted in the rescue efforts.One surviving passenger was admitted to the Qikiqtani General Hospital in Iqaluit, Nunavut for one day while awaiting medivac transfer to Ottawa. The other two surviving passengers were moved from Iqaluit to the Ottawa Hospital in Ottawa, Ontario. Additional information: First Air Flight 6560 was a charter flight which crashed near Resolute, Nunavut, Canada, on 20 August 2011. Of the 15 people on board, 12 were killed, and three were injured but survived. The aircraft involved, a First Air passenger-cargo convertible (combi) Boeing 737-200, was flying within Canada, from Yellowknife Airport, Northwest Territories, to Resolute Bay Airport, Nunavut. It crashed approximately 2 km (1.2 mi) east of the Resolute Bay Airport runway. The crash was investigated by the Transportation Safety Board of Canada.On 5 January 2012 a preliminary report was issued stating that the TSB listed the accident as a controlled flight into terrain (CFIT). The report stated that two seconds prior to impact the crew began a go-around. At that time the crew had completed the landing checklist, the flaps were at 40, the aircraft was travelling at 157 kn (291 km/h; 181 mph) and the landing gear was down and locked. Both engines were in operation and producing power at the time. The aircraft had been following an ILS approach due to poor visibility, and post-crash investigation found the ILS system to be operating normally. In fact another aircraft landed 20 minutes after the crash of Flight 6560 using an ILS approach. The report called on First Air and Canadian regulators to standardize Crew Resource Management and improve training to prevent the failure of communications between pilots which ultimately caused the crash. The aircraft involved was a Boeing 737-200 registration C-GNWN.[2] Ship 414 (serial number 21067) first flew on 2 May 1975 and was delivered to Wien Air Alaska on 14 May 1975. The three survivors, a 48-year-old man, 23-year-old woman and 7-year-old girl, were medivaced to Iqaluit.The last radio call was received at 12:40 pm CDT (17:40 UTC),at which time the aircraft was, according to First Air, about 8 km (5.0 mi) from Resolute Bay Airport, and the aircraft crashed some 10 minutes later. There are conflicting reports about the time of the accident. According to First Air and Transport Canada's Civil Aviation Daily Occurrence Reporting System the crash occurred at 12:40 pm CDT (17:40 UTC).However, according to the Transportation Safety Board of Canada the crash happened an hour earlier at 11:42 am CDT (16:42 UTC) and 1 nautical mile (1.9 km; 1.2 mi) east of the airport.
