Why isn't it necessary to complete all the challenges shown in the Assessment after each chronicle? Is it because you can't do the one stating "Enemy stronghold defeated within required time" if you choose to kill every enemy and destroy every other stronghold?

Yeah, some of the challenges aren't realistic to be attempting in an RTA setting. It's probably possible to complete the Chronicle in the 'required amount of time' but it's difficult to judge when it never actually even says where that is--and on some maps, doing so would require absolutely flawless execution that I'm not capable of. In addition, many of the maps require so many perfect victories, which are almost impossible for my katana weapon strats because of a quirk in how the life-draining weapons work (in this case the katana's Murasame)--it slowly drains my health every second and the final hit on an enemy doesn't restore health for some strange reason, so any of my katana-wielding Swordmasters will effectively never get a perfect victory. Again, on some maps meeting the Perfect Victory requirement would require incredibly flawless execution. I have no desire to even attempt such a feat; in particular, there is no sense in attempting a ~5 hour speedrun where I have to reset after an hour, 2 hours, 3 hours or more if I have to give up because I happened to take just a second too long or sustained just a little more damage than is required (another challenge that often appears). And that becomes a cascading defeat--if I take ANY damage in the first battle I fight, I can no longer get a perfect unless I spend time in a citadel recuperating, and that will make it impossible to complete it in the required time, etc. If someone is enough of a god at this game to complete it 100% and fulfill all challenges, I will happily create a unique 100% Hardcore or some such category for them, but I suspect such a thing would actually be TAS-only :)

How did you level up your characters so quickly? I started a new game recently and I'm on chapter 12 but my characters are so weak.

I can only say that it's because I'm fighting every enemy, and using only my 5 custom characters. Experience is very limited in the game, so every battle contributes, especially the battles against the Soulcalibur 'legacy' characters. My levels are much lower in my Any% speedruns because of how many enemies I skip.
