The 3rd risk is similar to A perfect misunderstanding but the hand signalled actually has flags this time

If that bloke thinks that stopping two yards from the signal on an emergency brake application represents good driving practice then he's got rocks in his head. In my railway if management got word of such a thing it would be treated as a safeworking near miss (and the consequences wouldn't be far removed from what would happen if you actually did go past the signal). Much better is to "see" the next signal at red until you know for sure it's at proceed, and drive to stop as you normally would with service braking.

I was about to say that surely you shouldn't have the need to use an emergency brake application? Of course better than passing the signal, but _needing_ to use the emergency brake to stop implies that you weren't travelling at a speed that would have allowed you to stop at the signal anyway(?) [Edited for clarity]

@@fetchstixRHD I mean assuming it's not something like "Double Yellow - next signal out" (which... would that not be a Cat. B SPAD anyway?), any driver driving under a cautionary single yellow should be preparing to stop anyway. So an emergency brake application shouldn't be needed.

I am not a train driver and am only a railfan, but even I heard that and thought that was not a good protocol at all. The train in that scenario saw a caution aspect and should have slowed down and be prepared to stop regardless.

they must have got special permissions to recreate the SPAD's otherwise thats alot of unintentional SPAD's just caught on camera

The AWS ramp will be set at a distance from the signal depending on line speed, so if it goes off, it's calculated that an emergency brake application will bring the train to a stop at the signal

No one has them, all was done by Spa films, will be on tape in a warehouse somewhere but likely not digitalised for most of them.

5:15 The handsignaller isn't meant to fly the green flag until he knows that the driver has already passed or has been authorised to pass the signal. In all fairness I do agree with Mr Tyreman though!

The first clip/ Scenario 1. If that was me driving the 220/221. How I would have handled that situation is that I would have approached those signals at less than 30mph. As it would have been a lot easier to prepare to stop before that SPAD risk occurred. I would rather be a few minutes late than risk getting a SPAD penalty on my licence. Scenario 2. There was a signal that was giving the driver a warning that the next signal was at danger anyway despite that it failed as the last signal at 2:51 looked like it was showing a single yellow caution to the driver anyway. Had he been paying attention, he should been slowing to a steady speed ready to stop when the next signal came into view regardless of if the signal was working or not. This is what I would have done although I had noticed that the last signal that was shown in the simulator replay omitted the previous "caution" signal. So although it wasn't an exact replica in the simulator. I do get the point of what he is trying to say there. Although as the last signal was on a signal yellow, there would be no need to slam on the emergency brakes anyway as the driver would be able to make a smooth stop. Scenario 3. This is where common sense should kick in. Because despite that what the hand signaller is telling me. If my signal was showing danger. I would not move the train until I got either a caution signal or a clear. So I would have pointed at my signal and shook my head left to right. Also if I had any doubts, I would have called the signaller and asked for a status update and informed him/her of the hand signaller that was giving me a "proceed" flag. That sort of SPAD is easy to avoid if the driver did this. As that is what I would have done in that scenario. Scenario 4. How I would have handled this situation. I would have packed my things away whilst I was still stationary as I could then concentrate on taking the locomotive back to the depot safely and also I would be doing what I should be doing and that is driving safely and looking out for signals. As I would be getting home a lot quicker to see my family that way rather than risking ending up in a box due to an horrific crash.

Scenario 4 (packing up while in motion) was a factor in the Southall rail crash in 1997 (the driver was packing up his things, on an HST where the AWS was isolated, passed a signal at danger and collided with a freight train). 7 people died and 139 were injured.

This is also in part the fact that AWS has a big flaw and that it doesn’t say which is Preliminary Caution, Caution or Danger This is what All Railways must have in their AWS different horns to discern what signal is ahead and also there should be like a little box with lights to signify what signal is ahead as another safety measure should the AWS be faulty or likewise

Depends on the circuimstances, can be derailment, collision or if the driver is lucky, nothing. For the driver though it will mean a proper investigation to understand how and why it had happened. It will be recorded on the driver's file too and an action plane will be created the avoid it in the future. Continous poor performance though can result in the driver being suspended or even dsimissed. Again, it realy depends on the situation, and we are now only talking about category A spads.

It depends on many factors. The worst that can happen is that many people can die like the 1988 Clapham Junction crash when one train passed a signal at danger and smashed into another unit which then smashed into an oncoming unit and caused a massive wreck. Many people were lost that day and many others were injured. On the other hand and if the line is less busy and there is enough space. The only thing that will happen is that the driver could be reprimanded and the incident will be recorded on file and the SPAD will be a mark on their train license. If the driver at hand causes too many SPAD's. Then three outcomes could occur from this. First of all. They could be offered further training where the driver does a SPAD awareness course so that they can avoid doing SPAD's in the future. Second, they could be suspended and then retained from scratch. Thirdly which is more as a last resort. They could lose their train licence and be completely banned from driving trains anywhere.

A May 2016 video by the Florida-based Distant Signal KZbin man claimed that passing a so-called "absolute signal" often got a train crew working for CSX, a major American freight rail company, relieved on the spot or, worse, fired (in the 1st 30 seconds of the video titled "How To Read Signals On The CSX Part 5 - ABS Signals"). It's understandable why given that there is NO excuse whatsoever for a SPAD without prior authorization from rail traffic control.
