How would you know if the inside of your feet lifting off the ground is an issue with balance of the tripod feet or a stance issue like not going wide enough before you perform the squat

You can simply try different feet widths / angles in warm ups. The tripod has to stay in contact for the entirety of the squat

Not really anything to worry about. Make sure you don’t lose any of the 3 main points of contact and we shouldn’t have any issues!
