There is no concept of first party and second party. The affidavit format is fixed. You can purchase the stamp paper in the individual name

If the trust is registered in Gujarat then the trust deed and audit report is in Gujarati language then it can be uploaded or trust deed in English language is mandatory ?

Only after 3 years you can apply for FCRA. Now you can apply for FCRA prior permission

No, the trust should be in existence for 3 years. If the deed mentions that the trust existed for many years but was registered late, then there is chance that you can apply

Not mandatory to open! You can spend directly from SBI! But you cannot use your general account for fcra purposes.
