The internal deformation (i.e. PL/EA, TL/GJ, M*theta/EI, etc.) is the deformation due to internal loads. Those internal loads are caused by the real external loads. The real displacement is the external displacement that you are trying to calculate.

@Dante Falcone Internal loading is caused by external loading (shear, axial, moments) acting on a system. To find internal loading you need to first find the external support reactions using equations of equilibrium and then make section cuts
