The thing about jjk powersystem is it actually pretty beginner friendly if you just got into it but Gege definitely cooked to make it way deeper when you dive into it. Idk how but that one eyed cat is a mad chef, I remembered reading the Gojo and Sukuna fight and being pleasantly surprised that Sukuna was using the info he gained when he was in Yuji body, touching Gojo in his domain to avoid the sure hit effect. I was like "Gege really added in that condition way back then to have it be used now?"

The detail that sticks out to me is when Kashimo X-rays Sukuna in their fight and finds that his body, despite having 4 arms and 2 mouths, is in no way hindered by it biologically. Makes it seem like a planned/natural state as opposed to something random/chaotic like the merging of conjoined twins. The theory I like is that Sukuna is similar to Yuji, in that they can both consume cursed objects/spirits since we now also saw Yujis body change.

Sukuna and Tengen's form has similarities when it comes their face. Tengen has 4 eyes, so does Sukuna. Tengen's absorbs other people's bodies to maintain his human form, signalled by his having 4 eyes when he veers off the human form. Sukuna too has all the markers of being more than one person - 4 eyes, 4 arms, 2 mouths. What if in this Kudoku ritual, Sukuna and his twin brother were the last remaining, and only one could survive. And this form is how he survived it, by fusing with his brother, and getting that insane power up. It could likely explain why he thinks Love is useless. Perhaps, when it came down to it, the brother that he loved and he Sukuna, were pitted against each other in this Kudoku ritual, and the only way out was killing the other. And either he or his brother decided to kill the other, hence giving Sukuna's "love is useless" ideology a basis, that when it comes down to it, people sacrifice even those that they love dearly to live on. So while one fusion i.e. tengen is worshipped as a God; the other fusion that is Sukuna is hated and feared as a Devil.

My theory is that Sukuna looks like that because he has been able to craft his body to match his soul. Sukuna has unprecedented power over his own soul, able to laugh at Mahito's attempts to manipulate his and able to heal Yuji from a missing heart by maintaining himself within his innate domain. Why? Sukuna's true form represents everything Sukuna is. Brutish (large arms, shoulders, huge face), hedonistic (extra mouths and arms), apex of sorcery and fighting, and yet, demonic, and cancerous (symbolized by the twisted half of his face). Mahito searched for his true form and finally found it only for him to have suffered too much damage to put it to use, perhaps Sukuna was able to stop him because he did precisely that but in reverse during the Heian era - forcing his body into a form that better reflected what his soul always was.

There's a hole in this theory, and that's that Maki didn't necessarily get a power "boost" when Mai died, she just lost her power nerf. Toji proved that the no-cursed-energy heavenly restriction is just naturally that strong, meaning there's no evidence to imply that Maki is any stronger than she should be. Basically, it isn't that being twins and then having 1 twin die gives you a huge power boost, it's that Maki is a natural prodigy who was held back from that status by the nerf of being a twin, or at least I find that more likely anyway.

I might be missing something but I always thought there was Sakuna the sorcerer and Sakuna the curse. Which would explain his dialogue with jogo and logo not understanding "fuga" as it was him accessing his sorcerer curse technique. I thought Sakuna came back after his death as what we see due to being cursed. Sakuna the curse then meets kenny and agrees to the 20 finger thing. It seems like that was red con by gege because now its kenny who made him a curse finger object where as in the beginning it was sorcerers coming together to seal him as they were not strong enough to kill him out right.

I would say that a curse like manitou was around then and sukuna made the curse give him two arms and two mouths with two more eyes these are really spicific extras to have as a sorcerer so it makes sense that sukuna made a binding vow with a curse similar to mahito and maybe that’s why kenjaku plan needed a technique like idle transfiguration

I don’t think Sukuna would have had such a huge power boost from the death of his twin as I feel like Maki and Mai were a special case even amongst twins, in that the reason why Maki became so much stronger was because of their completely opposing mindsets, meaning she wouldn’t have been held back as much if Mai had also wanted to be a sorcerer. I think another reason is also because heavenly restriction is just that strong, so Maki was already meant to be really powerful, not that she got a massive boost from Mai’s death. If Sukuna and his twin were both meant to be sorcerers, then I think Sukuna’s cursed energy would have just increased due to not having to share it with someone else, and MAYBE get his twin’s cursed technique too, although I doubt it because if that was the case, then surely Maki would have gotten Mai’s technique too
