Dr. Malik thank you for this excellent presentation. I have had elevated SHBG ever since my allogeneic peripheral blood HSCT for the AML leukemia I was diagnosed with in March 2014. The oncologists/hematologists didn't seem to know, or didn't want to explain to me, the finer details of SHBG. Thank you for the description of the complex balances that perhaps reflect the conditions that affect SHBG.

Please don't misread my comment, the SHBG level is very high in my case after the SCT (has nothing to do with AML leukemia), I was told by one oncologist/hematologist that it was my liver causing the higher SHBG levels, a PA endocrinologist said that it was due to the chronic GVHD, it might be a gene expression effect. I don't know but this presentation added to my understanding of this situation with me and SHBG biochemistry. I have found that there are a multitude of gene expressions, body biochemical expressions not well understood. It seems at this time and my observations, that although the HSCT to cure blood cancers has been going on for the past 45 years at City of Hope in Duarte CA. The "medical industry" still doesn't have a sure, beneficial for the patient, method to "tame" the chronic GVHD. What I have experienced and seen is that they discover a certain biochemical pathway that participates in allowing chronic GVHD, then they go through the clinical trials to bring this "molecule" to market. The blocking of a specific biochemical pathway I would surmise might cause undesired effects in the system as a whole and disrupts the homeostasis of the blood components. Thus the side effects are numerous, the medicine might increase the possible survival and mitigate chronic GVHD, but at a cost of "quality of life". The side effects of these drugs can open up the possibility of opportunistic infections, and in parallel lower the blood components like red blood cells, platelets, and white blood cells. Personally I think that there should be extensive research (clinical trials) using umbilical cord derived mesenchymal stem cells (uMSC) and uMSC exosomes to develop consistent dosing regimes and protocols. Perhaps to use as regular maintenance to mitigate and "tame" the chronic GVHD, as well as rejuvenate the vascular system, tissue, and organs that have been adversely affected by the chemotherapy, chronic GVHD, and immune modulating drugs exposure. @nunyabeezwax6758

I loose my 5 years due to vitamin D deficiency, I consult with multiple doctors because I was suffering severe fatigue, mode swing, unable to learn new things but no one ask me for Vitamin D test. Last month one of Doctor advised me for D test and it was 17 only, testosterone was at 190 and B12 was also at 230. I am 41, after this I start using D+K (5000 IU D3 and 180 mcg K2)along with omega 3( 1100 EPA 1100 DHA) and believe me my life start changing, now I can study continuously. I had low T, I started taking boron, and my T shot through the roof. There might not be many studies on it, but for my personal case, it works.

@@SmartestDumbGuy I take 9mg. I would probably start on a low dose and taper up, so you get used to it. I started out too high, and I got super aggressive.

49 Year Old Male with typical low T symptoms. My GP told me that he would only be concerned if my Testosterone levels were practically zero. I had to push HARD to get bloodwork, he did not want to do it. When the results came, my Total T was actually pretty good. I used the numbers provided to calculate my own Free T, which was at around 1.6%. My SHBG is in the upper end of the normal range, the GP classes it as normal. because technically it is, but I know that's the reason for my issues. I have manged to significantly improve my Total T over an 18 month period by supplementing Magnesium, Zinc, Boron and Copper. I use individual products for each, well researched and am pleased with those improvements. My total T is now much better than it was when I was in my early thirties. I hope that increasing my Protein levels might help out on the SHBG and thus the Free T.

Don't be a Boron. Your T is normal. There is no advantage to putting minerals in your pee. Save the money, invest and live better.

As long as your values are within "normal" range, doctors won't care. But normal doesn't necessarily mean optimal. Sometimes you just have to be your own doctor and find what works for you.

Thank-you for the info! One thing to note - your link to the web calculater includes the ending parenthesis, ")", which causes a "404 Not Found" error. If you remove that ending parenthesis from the link itself, then the reference works fine. I hope this helps!

I'm in my upper 40s and ever since I started taking vitamin D3+K2, zinc, and magnesium about 5 years, I feel great! I just got my bloodwork from the doc and all of my levels are optimum. Before that I was obese, low energy, and no sex drive but now that has changed. Also my PSA levels are normal too! Now I have to make sure I don't get stimulated during the day so I don't have an embarrassing reaction LOL. Also I eat a lot of crucericurous vegetables too which is supposed to suppress estrogen and lower SHBG which I didn't know until today! Thanks for the video as I now have confirmation that I was doing the right thing all along 🙂

wow thats gerat to hear who mentioned the dosage to you? Do you just take it as it says on the bottle or were you advised by an professional? The recommodation is often too low and basically for ppl who are not in need for it but we all know as older as we get we do need more to feel fit: I would love to hear back from you!

@@6sinnecoach I have done a lot of research over the years and found that 5000UI of vitamin D3 is optimal at least for me as I am darker skinned and live in a higher latitude. My last blood test showed me at 37ng/ml! I take this along with 100mcg K2, 30mg chelated zinc, and 400mg of magnesium. I take every morning except I take the magnesium at night to help me sleep.

Why can’t geriatric physicians explain testosterone as well as you just did. I am 67 years old and I’ve had LT for a long time. I have been treated off and on for the problem for at least 10 years now and have never had it explained to be as well as you have done in this video. I now have some more things to talk to my doctor about. Thank you.

boron and stinging nettle root extract will lower it you have to cycle boron if taking a higher dose (towards 10mg) or after a while it will increase estrogen

The answer to every question is yes. However, I would not touch Finasteride. In rare cases it causes mood and sexual health problems that do not stop even when you stop the medication. It’s rare, but worth considering. But yes, TRT will worsen hairloss if you are prone to it and Finasteride will protect you from that. If you are not very prone to hairloss, dandruff shampoo can work as well (that’s what I use, totally reverses the effects of TRT on my hair).

age 44 my SHBG is high (60) & prolactin high all thyroid levels normal. Free T bordeline low. Tried boron & tonkat ali but still can't get it lower.

I think the increased protein and fiber actually decrease the overall need for insulin levels. Fiber can actually stabilize blood sugars requiring less SHBG to manage the body. The net testosterone effect is more important than the insulin levels. Type 2 diabetes has significantly high insulin levels and the body's effectiveness and it is more or less insulin resistance. For every hormone there is a receptor and it is just as important to understand the relationship between the receptors as it is the hormones because the importance is not high levels of testosterone but the connection between testosterone and the receptors that it connects with so it is more important to not have high insulin levels because they make the connection more difficult to happen. The fiber and protein may actually make the testosterone more efficient than just high levels. The key is efficient connections and high levels are easy to detect but the receptors and connections are not and why a lot of diseases that are unknown at present have not the proper treatment because it is a disease of the receptors more so than a disease of the hormones.

I have a very complicated one Im 38, my free test came back well over the max, my shbg came back well under the minimum... now i cant put on muscle at all really, i feel like i have many of the symptoms of low testosterone.... My triglycerides are over the max just about and 2 of my i think white blood cells are above the limit... akso ive never taken steroids in my life

Hey Doc could you make a video talking about reverse kegels? I think that there is a lot of information about Kegels, but reverse kegeling is often neglected, and from what I read, it's crucial to do both in order to achieve a balanced pelvic floor.

Morphology, FSH, LH, Testosterone, Abumin, SHBG, Prolactin and Estradiol (estrogen). Knowing Albumin, SHBG and Testosterone levels will allow to calculate Free Testosterone and Bioavailable Testosterone levels using online calculator. It's more accurate than just Free test done separately and less expensive. You can add TSH, FT3 & FT4 to check for any Thyroid issues. Worth checking cholesterol (LDL, HDL and total) as they are testosterone precursors. Last but not least: PSA (prostate). TRT doesn't cause prostate cancer, but might not be recommended if there are any existing prostate issues.

Yes, mine is abnormally high! I just started boron today, which is supposed to help. (I'm female, total T is high normal and free is at the bottom of normal. I also have estrogen dominance. I definitely feel like my T is low.)

@@MsJacob24 I haven't had another blood test yet but I didn't feel it doing anything. It could be that another form of boron would absorb better. I also tried tongkat ali (herb) which felt like it was helping.

Thanks Doc!!!!! I am currently on a high dosage of vitamin D (once a week) due to low levels of this and testosterone in my blood work. I was taking a over the counter vitamin D supplement everyday. I'm hoping this works. I'm also taking zinc and working out more. I work long hours at work so I see the sun when I travel home for about 15 minutes and milk doesn't agree with me except for the unwanted ability to make music from my rear. Trying to increase my testosterone so I can have good work out sessions and a healthy sex life . Again thanks your the greatest and you are always on point when it comes to educating your audience!!! Age can bring irregularity or particularly constipation. Get that taken care of and sleep improves. Improved sleep brings libido back up. Just anecdotal\ of course.

Could it also be that SHBG goes down in correlation with high BMI and Hugh protein intake because men who train hard with weights eat more protein and tend to have a higher BMI due to carrying more lean muscle?

Just to be clear, testosterone doesn't increase in the early mornings exactly. It increases basically right before and during the wake cycle. So if a person is a night owl when they go to bed, which is the morning, they won't have an increased amount of testosterone at that time of day.

So I'm still a little confused. Is it better to have lower or higher SHBG? SHBG binds free testosterone, but at one point I thought I heard that increased SHBG causes increased testosterone.

Dr. Rena, I'm 40 and have low normal total T, it bounces between 400-550 I'm also a traumatic brain injury survivor, and would like to optimize my hormone profile to help in regaining strength and movement. Morning, to early afternoon. It can interrupt melatonin release in the evening. It somewhat imitates " sun clock ", that our bodies are used to.

I have read, no guarantee, that women with decreasing estrogen post menopause, have relatively relatively higher Testosterone ... Which contributes to some women getting recalcitrant pot Bellies like men Without going full HRT, Some kind of light Rebalancing in such women of their estrogen/ testosterone levels would allow them to get rid of their pot Bellies Male or female, pot bellies are central adiposity, visceral fat, which is pathogenic

Still unclear on which exact test to ask for? If you get a testosterone test, it will typically just measure total and free( and free is not accurate unless self calculated?) Also what about the dangers of getting test treatment?

Obesity decreasing HSBG? Studies say the opposite. Also, low carb diet increase SHBG; therefore a ketogenic diet even though might increase total testosterone, this will be unavailable because of the increasing in SHGB. Maybe you should review the video?

Sounds like you want to decrease. What you want is free flowing T. SHBG binds to T, not allowing it to flow freely.

@@AT-jz4gz i Am. Testicle torsion produces a very very strong pain, you could not be writing comfortably from your pc. But you should exclude some kind of infection after that injury.

@@cabecehuevoBut im still in a lot of pain, the pain increases a lot when I’m exercising. The other day I went swimming and I started having severe pain, even if I climb up the stairs. Now I’m even having trouble walking or sitting and the pain has been pretty much constant.

The best protein is animal meat and fat. It is what our bodies are made of. There is a reason there odds no such thing as animal fat or protein unless it’s made through heart and chemical reaction. See it this way. You’re out in the wild. You need fat and protein. You catch a fish out a wild pig, as long as it’s clean, you can even eat it raw. Plants, you need a lab. Animal meat is also the best way to get nutrients while plants have defenses like oxalates and lennins that bind to their nutrients causing problems to our health

Dr. would you be interested in my experience with gout arthritis and testosterone. These words are never uttered in the same sentence at the rheumatologist office but I promise you there is a connection. If testosterone doesn't play a part in gout why isn't it common in women? Sorry this isn't a sexual comment but if you suffer with gout you're screwed.
