Back when kirito and everyone went to get Excalibur in season 2, Yui talked about how SAO and ALO had that cardinal system as their core and thats why they kept getting unscripted events. Also, yes the outside people know about the taboo index, but they don't know about quinella specifically nor the reason why the artificial souls, or fluctlights, can't break the taboo index, so they don't know about the seal/code 871 in their right eye

To answer the questions you have is a bit difficult without potentially spoiling future episodes, but Quinella is not commonly known to the main Rath staff, they are aware about the Taboo Index, which is why Kirito was originally brought into the project, the main staff don't know how the Taboo Index came to be, and Kirito was only put in the Underworld to repair his Fluctlight.

Kirito has memories of his irl life but he doesn't have memories of being in the Underworld because his memories of the time he spent post coma were erased everytime he came out of the Underworld.

What doesn't make sense to ME personally, is how the population grew in the first place. If everything is virtual, and in a sense "not real" how can two separate fluct lights CREATE another fluct light? If they had to use the fluct lights of new born babies in the first place, that means that they couldn't artificially CREATE a fluct light. I don't know it just doesn't make sense to me.
