Thanks to youtube we get better. Doctors are just feeding themselves and feeding the whole industries involved, shareholders, sponsors, health care system. When politics/economics are involved, we will not get the help we really need to cure all kind of things. I am so glad this podcasts and intervieuws exist and books written by this people.

Your comment happened to be the latest, albeit a yr old, but I want to offer a why. It's my understanding that physicians, in their training, are taught so long as they don't deviate from what they were taught in medical school and residency, they have a defense for malpractice. Unfortunately science has busted multiple aged best practices.

I have been listening to a couple of podcasts by Prof Bikman and this still blows my mind. I had always considered myself a healthy person. In her 40's lean, muscular, rarely infected by viruses and eating lots and lots of fruits, vegetables, nuts, seeds, legumes etc. I didn't have serious health problems until 2 years ago when I moved to Germany. I'm an Asian person and always ate whole food throughout the day in my country. Since moving to Germany, I suffered terrible inflammation, joint pains, brain fog, blurred vision, fatigue, heart palpations and ringing ear. I went to see several doctors. They drew my blood and said everything was okay and that the pains were probably in my head. I was so gutted. The inflammation was the worst that I couldn't sleep. And when I was so sleep-deprived, the inflammation would get even worse. I often cried until my pillow soaked, tossing and turning, as the window for sleeping was closing up. And this was rare because I see myself as a strong person and I barely ever cried. To make matters worse, I couldn't work. That hurt me the most since I had always been a high-performer, independent, successful professional woman. I didn't know what was wrong with me. Autoimmune disease? High fat in my blood? Why when I always eat and live so healthily? Listening to people like Bikman, Lustig, Perlmutter, Fung, etc, makes me understand what was wrong. Now I'm fasting the right way. I try to reduce carb and eat more fat, the latter which I was afraid of because of the conventional wisdom that fat is bad for health. Your channel, Casey, among a few others, has changed my perspective about healthy living, changed my behaviour and most importantly set me free. Now I understand! Suffering and not knowing what's wrong and what you can do makes one hopeless. But this is empowering. It makes me feel that I am in control and not have to depend on doctors whose appointments so hard to make and who will spend not more than 10 mins with me anyway and not explaining anything. So I just want to say thank you sincerely. I appreciate your work and Dr Bikman's work greatly. Thank you.

Dear Decana, Yes his advice on eating meat with its resident fat and cutting the carbs is spot on. I’m similar to you in many respects. I have increased my strength since starting a meat heavy diet along with its resident fat and honestly I feel 10 to 20 years younger now. I jokingly tell my husband is “stand back!” I wish you the very best. Buy his book. Read it carefully find a friend or online keto carnivore group to join you in your progress! Low insulin is the way to a quality life!

Hey @deekana Did you finally understand the underlying issues causing the chronic inflammation? Some of it could have been carbs, however from your description of whole foods consumption I would think it could be something else. On hindsight, aging does accumulate those fatty cells in the liver, pancreas and other parts (mostly as visceral fats not quite visible. The fats could have grown to their limits over the years and result in the inflammation mentioned. Therefore it would be a combination of food type AND food quantity that would need to be addressed. Love to hear your journey.

A year ago I developed tinnitus and was also officially diagnosed with severe sleep apnea. My weight was also going up. I finally got back on ketogenic diet and am now full carnivore (thanks to Dr Berry and Dr Chaffee) and have lost 30lbs back to 181lbs and dropping working out again. My tinnitus has lessened considerably and my sleep apnea has lessened as well (lost weight in my tongue…fatty tongue). Thanks for continuing to bring the knowledge so knuckleheads like me become more informed. 🙏🙏🙏❤

Heavy processed carbo addiction, yes it's an addiction. I'm 64 and I became very interested in foods and health when I was age 9. I had been home for 2 weeks curled up the the couch when my neighbor came to check on me. This lady was 85 and I was a latch key kid. She was horrified to see me travel to the bathroom crawling on my hands and knees. A doctor was called who made a house call and examined me. He called my mother and told her, in an angry tone, he was taking me to the hospital for an emergency appendectomy. Immediately after the doctor hung up the phone, he stormed into the kitchen. I could hear him opening the fridge, cupboards and drawers. He was angry, slamming things around and grumbling. After I woke from surgery, my mother came in and the doctor cornered her at one side of my bed. He pointed his finger at her telling her he was going to call Social Services and have me removed because my DIET of bread, cereals, pasta, hotdogs was killing me. To this day I CANNOT have frosted miniwheats, Life, Cheerios in the house. My kids said when they were young the neighbors all portrayed me as a nut because it was common knowledge I would not allow boxed cereal, sugar, sodas, white bread, candy, etc in the house. If I eat a bowl of cereal in the evening a couple of nights a week, the cravings will start up and the cereal will call to me at 2am screaming to me to eat it. I can't have it in the house.

Love Dr Bikman! I read his book "Why We Get Sick" and put it into action 7 months ago. I'm down 25 pounds and I am already using 70% less insulin now. I'm working on reversing my type2 diabetes with low carb and time restricted eating. So easy.

This interview clarified several perspectives for me, and I've listened to Bikman and many others. My family tends toward diabetes in older age as well as hearing loss. I never supposed they might be connected by causation. On the other hand it put my mind at ease about a morning bg of 110. My goal is to be under 100 each morning, and have struggled to achieve that. I have compromised to a goal/practice of fasting until bg drops below 100. UPDATE:. (Age 71) Fasting until glucose dropped below 100 didn't work, because it didn't allow me to finish my eating time early enough, (I need to be done eating by 3 pm to maintain a long enough fasting time, have an empty stomach at bed time, and stay on a 24-hour cycle). Self-imposed parameters: *24-hour fasting eating cycle *18 hour or longer fasting period *Eat enough low carb calories to maintain energy yet discourage snacking. *Restrict calories and carbs enough to be under 100 fasting glucose. Gratitude to Bikman, Fung and others for educating me to be able to lose 50+ pounds and maintain for several years.

about 4 years ago, i listened to dr lustig's lecture (sugar: the bitter truth) on a recommendation from my mom. i took the advice, to some extent, without any realistic expectations whatsoever. but my frequency of heartburn occurrence/tums consumption dropped from 4 -5 days/week down to a frequency of 1 - 3 days per month. listening to drs bickman and means discuss this issue strikes me right in the heart (burn), so to speak. thank you for discussing this with your audience. thank you, thank you.

Dr Bikman, God bless you for availing yourself to research metabolic disorders. I had all the symptoms you talk about 40 years ego and never once did any doctor mention insulin resistance. After years of getting tired of being sick l took it upon myself to go to the library to read books on nutrition. Started uncovering information on insulin and inflation. Listening to you has helped me to control my symptoms with health modifications by eating low carb. Thank for all you do.

Say what? I had horrific issues with my ears as a young girl and terrible earaches! Diagnosed later I was diagnosed as a type 1 diabetic by age 9! I also had fungus on my feet and the doctors could not figure out why I had that now I get it! Yet it's been years since I've been diagnosed I'm a nutritionist as well... It's interesting that things are still coming together for me! I love Professor Bickman's work on insulin resistance! As it appears that most of the diseases that we fight in today's medical field are related to metabolical issues!

I can attest to the carb/heartburn connection! When I was vegetarian then vegan, I ate fruits and veggies and oatmeal and high fiber toast...(trying to get rid of my diabetes).. and I ate tums almost a roll a day. once I went back to omnivore, the heartburn started easing, up... now as a primarily carnivore (still working on curing the diabetes, but a lot closer now than I ever was before)... I haven't had heartburn other than maybe once or twice (on a carb binge) in the last 2 years. TRUE relief!

@@levelsCasey & Ben: I beg you to read these research articles on B1. They match perfectly with what you're saying about the dangers of high carb/ high sugar/ frequent booze. I agree we're getting more insulin resistant, BUT those "manmade carbs" & sugars or frequency of alcohol are actually depleting our B1 thiamin during krebbs cycle! Actually very common mimicking other diseases as the first article says. -"Hiding in Plain Sight: Modern Thiamine Deficiency" -"Sugar intake and thiamine" BY DERRICK LONSDALE MD, FACN, CNS -"Thiamin Deficiency (Beriberi; Vitamin B1 Deficiency)" By Larry E. Re tinnitus. A couple of months into a ketogenic diet, I noticed that the sound of my tinnitus began to change. It went from a constant sound to a broken sound. It actually sounded like crickets. Eventually that changed too and became very quite, almost non existent. I also used to have these incidents where I'd get a sudden loud tinnitus in one ear which would actually affect my balance momentarily. That no longer happens.

What's needed is an easy way to measure insulin. Seems like everyone is moving towards the opinion that insulin and its effects are more important than glucose, but doctors still don't measure insulin except for fasting insulin. Dr. Kraft measured both for some 14,000 patients back in the 1970's and was sounding the alarm about insulin then. Here we are 50 years later and still not doing as much as he did back then. I really like what Dr. Bikman has to say and how well he explains it. Maybe he can wake up our medical community.

Gregory! This life change has been phenomenal for you! Delighted to hear it. Wanted to share one thing though. Check out dr. Ken BERRY’s video on strong bones. It indicates, and I say this lovingly, that vegans, overtime have the most bone breaks. Pescatarians and carnivores or meat eaters have the fewest. There may be a time and what you may want to integrate these other items minus the carbs but with all of the delicious fat. I just wanted to share that information because with information we can make choices in our life.🎉

Well done! I would suggest caution about sarcopenia at your/our age and a reassessment of sufficient complete protein intake. Using cronometer app and plugging in the TRUE daily intake revealed my protein was not as adequate as I believed. Anyone over 40 should be getting at least 1.5g per KG of LEAN body weight. Sufficient complete protein especially via plant only routes is tricky, most especially at our age. Be well!

This was a great dialogue. My question for both of you is, “is there a MAST Cell disorder related to all this also? And “is insulinemia the root cause to metabolic disorders or histamine from a Mast Cell Activation overload or is this a dynamic disease progression?” The disease list for MCAS seems so similar to hyoerinsulinemia issues, that I can’t help but think there is something going on there.

Wow. From tinnitus to GERD right "off the bat"! I went keto 5 months ago and was almost immediately no longer bothered by GERD. And, one of the things that has snuck up on me (because it gets to the point that it's just _there,_ and it has to be ignored) has been the slow, but steady, reduction in my tinnitus, and the return of my musician's ear acuity. I hadn't tied those health improvements to my change of food (it's not just a _diet,_ by any means), but their explanation of the biochemistry behind the improvements makes great sense, since (for the first time in many years) my A1C was finally within bounds two months ago.

The question in the beginning, I have Hashimoto and have had enormous amounts of coughs, flu symptoms and also some degree of hearing loss with time. I also had a visit at an infection clinic with pneumonia with ground glass patches on both lungs and they couldn't find any reason, no detectable virus or bacteria. When my latest TSH test was up to 7.3 despite I had made an effort to eat "a lot of fruits and greens, choose wholegrains, lean protein and cautious with fat" I started to look around and decided to try keto for at least the next three months to next regular test. I'm just one week in and I have less itchy eyes, they're not as dry. And it seems as my nose does have been running (what I'm referring to is that the inside of the nose becomes too dry and you get these watery drippings). So already after one week I notice changes I see as improvement in less issues that I think is related to the thyroid dysfunction.

I used to have GERD. It first happened when I eat a lot of donuts in the morning at work. I then often went to buffet in week end , this contribute to my over weight. I was pre-diabetes for a long time since I was around 40 yo. Yes, 40 YO, I got joints problems, sleep apnea problem, high blood pressure problem and skin allergy... BUT, I was never know It was all from condition of pre-diabetes. Until 12 years later I got a badly car accident , doctor found out and I got treatment with CPAP. After 3 to 6 months, I am off all high blood pressure med, and my GERD is gone. I notice my problem of short term memory has some improved but sugar may have been hurt my brain. It all started from SUGAR. You will get over weight, OSA sleep apnea... then GERD. From there, all problems will show up. You got hooked on medicine. But I found out the video of Dr. Lustig about the sugar. "Sugar: The bitter truth". I started searching youtube and got on low carb diet. I started walking to exercise in the local park . I lost 30-35LB. My cholesterol little high at 235 but I did not take any Lipitor. I have all the symptoms that Dr Bikmen talked in this video. I got feet pain, hearing noise in my ears, like tinnitus problem but low carb diet helps little every thing in control. Now any time I eat just little rice, I will get hurt in the feet, hearing very crazy sensitive even just little noise from normal talking from people around me. Sugar killed me.

Here's a ChatGPT summary: - Part of the hope of the book and Dr. Ben Bickman's message is that when we acknowledge a metabolic underpinning to chronic disorders, there is a ray of hope because there can be a metabolic solution. - Dr. Ben Bickman is a professor at Brigham Young University and the director of its diabetes research lab. - He is the author of the book, Why We Get Sick, which focuses on insulin resistance as a foundational pillar of much of the chronic illnesses we see today in the Western world. - Dr. Bickman discusses the relationship between insulin resistance and ear, nose, and throat conditions, such as hearing loss, ear ringing, and vertigo. - He explains that insulin resistance can lead to higher levels of reactive oxygen species, which can activate apoptosis and the death of relevant cells of hearing. - It can also lead to compromised blood flow, which can cause damage to the fine capillaries in the ear tissue. - Dr. Bickman also discusses the correlation between insulin resistance and tinnitus, which is ear ringing, and Meniere's disease. - He explains that 92% of people with tinnitus and 76% of Meniere's disease patients are insulin resistant. - Lastly, Dr. Bickman discusses the relationship between insulin resistance and gastroesophageal reflex disease, otherwise known as GERD. - There may be a relationship between insulin resistance and GERD and reflexesophagitis. - Chronic GERD can lead to Barrett esophagus, which is one step away from esophageal cancer. - Insulin resistance can increase the risk of GERD by 15%, even independent of visceral adiposity. - Cholecystectomies are becoming more common due to hyperinsulinemia, which increases the production of cholesterol from the liver, making the bile thicker and promoting the formation of stones. - Low-fat, low-calorie diets slow down the emptying of the gallbladder, increasing the risk of stones. - Bile salts stimulate mitochondrial uncoupling in fat cells, increasing metabolic rate. - More than 30% of liver transplants are caused by the downstream effects of fatty liver disease. - There are a lack of effective therapies for non-alcoholic fatty liver disease. - Fatty liver disease is a newer problem, with the primary problem being too much fat in the liver. - Most of the fat in the liver is leaking from fat cells that have become insulin resistant. - Fructose is a major contributor to fatty liver disease, especially in children. - Fat cells can grow through hypertrophy or hyperplasia. - Hypertrophic fat cells become insulin resistant and start to leak free fatty acids, contributing to lipotoxicity. - Inflammation and elevated insulin promote the accumulation of ceramide 1-phosphate, which promotes hypertrophy. - High levels of linoleic acid from seed oils can also alter fat cells. - Oxidation is the normal burning of fat, which linoleic acid can do very readily. - Eating more linoleic acid than the body knows what to do with can push it down a peroxidation pathway, which can turn into numerous harmful molecules. - One of these molecules is HNE, which can push the fat cell down the path of hypertrophy, making it insulin resistant and pro-inflammatory. - Eating whole foods and avoiding processed foods, refined seed and vegetable oils, and fructose can help avoid this path. - Ben is working on a research project to understand the paradoxical rise in blood glucose in the context of a low carbohydrate diet. - Glucagon is a hormone that increases glucose, and it is important to understand its role in metabolism. - Glucose levels were once considered normal up to 150, but now the threshold is much lower. - Insulin resistance is the primary problem, not glucose, and Ben is more worried about insulin than glucose. - Glycation is a biochemical problem when glucose is chronically elevated, but it is unclear how much of a difference there is between 80 and 110 mg/dL. - Glucose intolerance can be caused by insulin resistance, but not always - Eating a carbohydrate heavy snack can cause glucose levels to spike and take a long time to clear - Fasting static blood glucose tests are not always accurate - Continuous glucose monitors can provide more accurate results - Research is needed to understand different metabolic phenotypes - Funds are needed to support research on this topic

I really get a lot out of these conversations. However, It is difficult to get a total understanding fluently, when the levels are quoted in the US units of practice in, g/ dl as opposed to mmol/ l, as in Canada. I really wish our two countries would get together on this. It becomes as confusing as ⁰F and ⁰C. Never an issue of understanding hot and cold until the conversation expects you to transition simultaneusly in both units of measurement. When you are quoting 70, 80 and 90 etc. g/ dl, I'm stuck in conversion, trying to relate it to my Glucose levels in the range of 4.5- 5.5 mmol/ l. Lol, just trying to wrap my head around the units of measurement with out missing the context. It would be nice to see it charted, like looking at a tape measure that is providing both imperial inches and feet, and metric mm and cm units all showen on the same tape. Great conversation. Shout out from Alberta to professor Bikman.

For me, so far, on a low carb mostly meat diet i have figured out two issues. My fasting and average blood glucose was tending to run from mid 90s to mid 100s. One of two things or both seem to be causing this. Taking a multi vitamin and the Vitamin C level, 500 mg and or not eating enough low glycemic carbs. When i quit taking the Multi vitamin and eat more salad leafy greens, onion, peppers, olives. My theory is when the liver converts protein to glucose, the conversion level is higher than what we want for a basal blood glucose level. If you eat a modest amount of good low glycemic vegetables then the liver doesn’t have to convert the protein to glucose and then the pancreas can maintain a proper blood glucose level. Its like there is a liver pancreas disconnect when relying on gluconeogenisis. We shall see as in about a week i will take the vitamin again and see if the basal blood glucose goes up again. My blood sugar is now running mid 80s during the day right now and even down into the 70s during sleep. One thing is certain, Free Style Libre 2 reads around 10 higher than actual blood results pretty consistently.
