I owe these two fine gentlemen my life! It was 12 midnight, January 7, 2022, I woke up to a very painful stomach. Went to the toilet, was writhing in pain on the toilet seat. I literally wanted to die from the pain. What's worse was nothing was coming out! After about 30mins of the most excruciating pain in my life, I started defecating. I breathed a sigh of relief and I started FBing on my phone, and I happened upon this video. I've never engaged in health videos before. Never heard of either doctors before. How FB's algorithm decided to show me this video is nothing short of God's divine intervention. This video jump started my journey to eating healthy (no processed food, no sugar, low carb, lots of veggies and whole food). I am obese (272lbs, 5' 8", 44yo, male) and overweight by 110lbs last Jan 12. Now, 3 months and 1 week later, I am only 220lbs! I lost 52lbs! And I won't stop losing weight until I reach my ideal body weight of 160lbs. Currently in day 4 in what I hope would be a 7 day water fast. Previously, my longest water fast was 5 days. I hope Dr. Hyman won't mind, but I would also like to mention the other doctors that helped me in my journey: Dr. Pradip Jamnadas, Dr. Jason Fung, and Dr. Eric Berg. You, guys, rock!!!!

Tests: 1. Fasting Insulin should be below 5 2. Oral Glucose tolerance test ( at 30 mins, and at 2 hour) 3. Triglyceride to HDL ratio should be less than 1.5 - no insulin resistance. If above 1.5 , there is insulin resistance 4 . Waist to height ratio - if waist circumference ( around navel) times 2 is higher than height , there is insulin resistance. If less, there is none. 5. Skin signs Skin tags, acanthosis nigricans Lipid test NMR- LDL particle no and size CRP, uric acid , liver function test. Management : (small fat cells just multiply and dont become insulin resistant , not inflammatory ; big fat cells, hypertrophy, insulin resistant , inflammatory). 1. Exercise: the best is the one you do, particularly after the most starchy, spiking-insulin meal. This makes cells insulin sensitive, 2. Supplements: berberine, cinnamon, vitamin D, alfa-lipoic acid, magnesium, omega 3 fats, biotin, chromium 3. Medications: metformin ( is only half as effective as lifestyle changes) 4. Nutrition: control insulin by control carbohydrates, prioritize protein , and dont fear fats. Eat when hungry, dont Control carbs, eat them, dont drink them , dont eat them out of boxes and barcodes, Eat them from vegetables and fruits. Get high quality protein, lots of it. Focus on ancestral fat, from fruit fats and animal fats. - dairy fat, avocado , olives/olive oil, coconut oil, nuts ( seed oils cause fat cells to go into hypertrophy-inflammatory route; also saturated fats are stable but seed oils ( polyunsatirayed oils) are unstable and become easily oxidized which is phagocytosed by macrophages ( start of atherosclerosis).

In Australia, fasting insulin tests are commonly ordered. In fact, it picked up my high A1C and put me on the path to serious diet reassessment. A low-carb diet saw me drop from 84kgs to 56 kgs. So funny everytime I feel the host interrupts the guest talking consistently and it’s so hard for us to listen to, I always check the comments to see if others make comments about it 😅 HERE YOU GO!!!!!! Maybe talk to the guest first on how you two want to handle this, one person talking all the time or it’s more Q&A session style

I have just come across this conversation. It’s fascinating and all aspects, in particular the fundamental aspects of the metabolic health has been well discussed. I am also surprised that only 200,000 or so viewing so far where I would’ve expected in millions. I realise some people are disturbed with the interruptions by Mark but he is such an enthusiastic podcaster so it did not really bother me personally.

The elephant in the room as to why the proper testing isn't done for fasting insulin, our practioners only do fasting glucose, as a rule is our insurance companies. It is outrageous - I have been asking our primary doctors to do fasting insulin abd the A1C test for over a year and they refuse to do so. I finally got one primary to admit they only do the A1C test once you have a fasting blood glucose of 130 two times, otherwise insurance won't pay for it. And that is just to get the A1C test - still won't approve the fasting insulin test at that level. Have to climb a mountain to get decent health care in this country. I know I can pay for tests out of my own pocket, but why should I - I already have high premiums. Tragic.

I live in England and we have free healthcare, and they do the same thing here. I half most of the 12 signs of insulin resistance and they will only do an A1C and and a no fasting glucose or insulin. It’s madness. Surely they know that your fasting insulin will show insulin problems long before anything else

It works, on OMAD 5 days a week to start with then x2 meals a perday @ weekends..came off my insulin (dose X2 a day 48 units) I know one thing with better effects am feeling great! lost in my first week 7lbs.. sugars still higher than when eating excessive carbs on insulin, but am advised it will reduce over 12 weeks, on 20 carbs a day, weekend as well, two meals a day...same time 9am & 5pm..

Interesting, thank you. I did keto diet 10 years ago and lost huge amount of weight after 2 years I put it back on and more. I changed to plant based diet since then and I defiantly eat more vegetables and nuts than fruit. I keep my weight stable and perfect for my body. If I eat fruit in empty stomach I do put weight on. I try to keep my glucose down with eating first salads, nuts and protein then at the end I have couple fruit

You put the weight back on because you abandoned the keto. It doesn't work if you don't follow it. Now you're struggling to keep your glucose down when keto eliminates it.

I have CFS and have used keto and fasting to lose the extra weight I put on years ago. Dr. Hyman I hope someday you can address CFS the way you address blood sugar and indulin today.

do you take any supplements? have you looked into vit b1, vit B12? magnesium, zinc for example? you may be deficient in certain minerals, virits, etc. have you had a CBC blood test? do you have enough electrolytes in your diet? sodium, potassium, magnesium? sorry for all the questions, just curious as to your symptoms and what you have done and looked into. take care.

This type of talk could be refined and summarised and bulleted to educate national diabetes establishents and to facelift globally medicine tuition. This would prempt and eradicate most T2 before it could take hold

Fruits/ veggies are also full of lectins and oxolates. How does that work with the diet dr. Hyman recommends?

Experimenting with a CGM i have been eating mostly meat and when i add low levels of low glycemic carbs it keeps my fasting and average blood sugar low, 70s and 80s. When i eat just meat, my fasting/average glucose stays in the 90s to low 100s. I believe that when the liver converts protein to glucose it tends to keep the average blood glucose higher by adding low level good carbs it keeps the liver from having to convert protein to glucose and then the pancreas can manage the blood glucose. My fasting insulin levels last tested 4.6 with my fasting glucose of 96 two weeks into a keto/carnivore diet. Seems like their is a disconnect between the liver and pancreas when the body is utilizing just protein without any carbs. Seems very similar to how slow starchy carbs do roughly the same thing as protein except protein does it at a lower level where sliow high glycemic carbs keep blood sugar in the safe range of around 130-140 but for 6 hours.
