It's a leftover from back when you'd meet the fishermen trying to free the mindflayer under the rubble. They project some form into your head to manipulate you, one fisherman thought the mindflayer was his long dead daughter, another thoght he was a beautiful woman.

I just came to a realization, would commoners in Dnd know what a “Brain” is? As in what it looks like? You’d imagine books illustrating the subject wouldn’t ever cross paths with people who don’t work in fields relating to it, unless this suggests DnD schooling is like modern levels of advanced which if you take magic into account maybe?

Its a reference to content that was in the Early Access version of the game. You know that trapped injured mind flayer you encounter? Well in Early Access he had enthralled three commoners who are very easy to spook into fighting you i think there was maybe one or two things you could say to them that would keep them from going hostile. So you usually ended up defending yourself from them unless you can kill the mind flayer first to break them out of it. Afterwards, if you ended up killing them it would spark some interesting dialogue from any companions you have managed to pickup about their feelings of the inevitable act you were forced into doing... Because you effectively just killed three innocents jt wasn't their fault they were mind controlled...
