Well, The Main Character didn't die, The main character got revived into a other body, I mean the dead cave body.

The dead body part is not from Blind Eye, it was a reference to Clean Out. In a hallway near the Ventilation Control System room, the game would show a cutscene(vision of the operative) where he(operative) was in a cave and he dies

@@doublegg4089 Edit: and Too confused to i don't know what the hell is going on in the lore but I know there is a lore but still I dont know what the f- is going on. There not a straight line lore story it have past and present timeline it was just like a jigsaw that you have ton of ton of the story that you must put all of them to know the lore so far but in present some of the jigsaw missing must wait for new update and I can't tell is it a past or present timeline it make my little brain hurt so I don't give a chesse on dis lore. (It will be very helpful that some people tell or explain what is going on in the lore/story so far in the straigth line so my brain won't hurt).

@@Unknown-vk2rq literally me being casual and reading the dialogue, I realized that the foundation created copies of a soldier who is your protagonist, so the same soldier works for each team of the MTF, you saved the O5 but another you went to contain the 096 which was found a body that looked like you that turns out to be a copy of you who died on that mission. So there is lore and an easy one.
