You're wrong lady! A reasonable accommodation request or a reasonable modification request can be made in any form of communication from oral/verbal, written, text, email, you can even have someone else request this for you! Asking our telling any disabled individual that there is a form they must feel out of trekking people it needs to be in writing is misinformation! It DOES NOT need to be in writing. Landlords want things in writing so they can have a record of the requests, so that's their problem and their responsibility to make a record! I know this because I'm currently suing the low income management company for discrimination. Landlords and lawyers that represent then need to stop trying to have power over disabled people! You think we don't know are rights. You only study them, we live them! So if you want our need to make a request, do so in any form you wish! It is illegal to ask anyone to complete a form!
