Hi Brent, currently a 3rd year medical student, (so not a PT student, but partner is working on getting in) and he found the cause of a lot of my low back and knee pain was actually my TFL and IT band. I’m EXTREMELY tender when he helps to static stretch like what you’re doing in this video(to where he can only put about 20% pressure down on my hip before my knee feels like it’s going to break off) . However I’ve found that If I lay on my back with my knees bent and relaxed and push into the more lateral part of my leg right under where the inguinal ligament would run that it illicits the same type of pain that radiates down my IT band. Was wondering if that’s just another access point to the TFL trigger point, or if I’m hitting a different muscle that is contributing to my pain as well? Also this video was super helpful and informative. Definitely subscribing to your channel!

Ive been told thst hip bursitis is mow called in greater trochanteric psin, and usually involves over stressed tensions, which links up with TFL, Glute Med be and Min, and IT band ..etc etc. ( Yes I have this trochanteric pain from muscular inbalances over three years). Would your trigger point release help as part of a treatment regimen? Id willingly look for a physio to do this if it would help. Thank you.

Diagnoses rarely help determine effective treatment in a physical rehabilitation environment. Our treatment is based on movement impairment. What you are actually describing is an inability to accurately differentiate between two diagnoses with similar symptom clusters. However, yes, this treatment may be effective for either, or both diagnoses depending on the results of a movement assessment. Dr. B2

I really try to cover as much as possible in each video. Overall, use your thumbs as little as possible, use other fingers, pisiform/hamate and even forearms/elbows when you can. You will have to use your thumbs fair amount of the time, so sparing them when you can is important.

Hi Brent, I kindly ask you a favour: When I squat (flex my hip), than I extend to stand up, why I have pain in my TFL after I stand up? I’ve tried to modify my squat in different ways but still have that pain... Also at night time when I lay down I feel my entire skin area of external quad and TFL burning. I would really appreciate your advise! Thanks!

Stop trying to modify your squat and start looking for a solution to the issues. You could see a physical therapist or research corrective interventions. Chances are you have lower extremity dysfunction or lumbo pelvic hip complex dysfunction resulting the symptoms you are describing. Dr. B2

Hey Gary, 1. Excessive femoral internal rotation during functional movements and or restricted ER or IR during 90/90 hip goniometry. 2. Bursitis is a diagnosis of dull pain around the greater trochanter... not a very specific diagnosis. Dr. B2

Thank you for your reply Dr. B2. This is what I am trying to figure out... I run, bike, hike and ski with no hip issues, however when I sleep on my side my hip starts throbbing at some point of each night. It also will throb if I go to my other side to sleep (maybe more from lack of support or leg angle when the bad hip is in the air). If this is a muscle issue then I can work on it. If it's bursitis then I'm probably looking at a cortisone shot. Am I looking at this correctly? Thank you again.

so i understand that u would want to engage the vmo to help keep that patella in line , but what other muscles besides the glute med need strengthening?

Hey Ryguyiskindofaflyguy, It would be better to consider the patella as staying in place, while the lower extremity falls into a functional valgus. The glute max, glute med, tibial internal rotators, tibialis anterior and tibialis posterior are all likely weak/under-active. Dr. B2

You never know where someone is in the educational process, and you also never know what gaps someone has in their knowledge.
