There's an icon in your upper right buff icon bar that shows your percent progress toward invisibility. When you don't time your crouch correctly according to your melody, it does reduce your progress back down. The moving circles on the ground (orange in your case) give you another indicator when to crouch. You need to be finishing a crouch as a circle closes on your feet. There is also a circular flash at your feet (looks blueish for you) when you get your timing right and make progress. Based on your overall melody, you needed 3 successful crouches (with no failures) to go invisible. A common strategy is to put a note on every beat of the melody so you can just spam the crouch button and go invisible after 6-7 fast crouches.

Very good to know. So you have to press crouch slightly in advance of the best in order to get the animation to sync feet/crouch at ground level w/ ring closure. That's definitely the part I was missing. I can do the crouch in time all day long, but was wondering why sometime it worked, and sometimes it didn't. Very good info. Thank you!

To make Octavia easier you should mute her song in options and make a song that has notes on every slot, that way you always get her buffs without thinking.

I just copied a Rush E-ish song. Close enough.

I hate the fact that you have to press crouch twice to crouch the second time, I would love to follow the beat of my song if I had to press ones

I will definitely try that next time I give her a go. I played around with the song set-up several different ways. Just consistency issue on my part. Guess notes everywhere may be the best way to approach it so I can just spam crouch.
