Great info but for those of us with low cortisol AND low cortisone, with that flat daily graph and whom aren't overweight/diabetic/leptin resistant - what do we do?

so if your serum cortisol and cortisone at at top of IN ref pt but saliva is lo am and noon, ok evening and hi night? Seems like circadian needs rebalance. Wonder how to get the "inactive cortisol" to increase in the morning. Same with hormones are low but Cholesterol in hi , if missing a cofactor? Very interesting talk ! Can low free cortisol be related to low blood glucose w symptoms??

What about ACTH, aldosterone, and renin? Do you check these to ID an adrenal or cortisol issue?

A "what if" question so apologies in advance: In post menopause "if" you indeed have a higher cortisol profile (genetic pre-disposition to skinny diabetes/IR - Mom survived Hongerwinter starvation in Holland during WW2) is this the profile where progesterone hormone replacement will cause issues and/or worsen IR? Put another way will progesterone "unmask" undiagnosed or highly sensitive IR because until re-adding progesterone you've kept it at bay by eating less, no longer had the extra cortisol factory from progesterone, but you are still glucose intolerant? Trying to crack the code on this. Have posted elsewhere and I wear a Libre. My A1C was tracking at 5.5 and jumped to 5.9 when I started HRT with progesterone with no other changes or same exercise if not a bit more, great diet (hand on bible), etc. My glucose is a mess - not PPG just FG - during 12 days of progesterone. I wear a CGM as a pre-diabetic and thus have the data/evidence. Interestingly when I looked at past non-menopausal labs when I weighed 15 pounds less my A1C was higher at 6.0. Was doing endurance sports then so that played a role. Have not done the Dutch test and will explore. Awesome topic and would love your thoughts here. Happy to serve as an N=1.

Thanks. I have adrenal deficiency from immunothearapy where my body doesnt make any cortisol and ACTH. After different doses, I take 10-15 mg a day and that feels ok. Anything else I should worry about? Thanks Tom

I also have Addisons. I take fludrocortisone as well. Do you think our adrenals can heal themselves?

I have taken prednisone 20mg for 5 days, and suddenly my sleep schedule is messy, went from 9-10 hours of sleep to 6 max if I am lucky. These meds should be banned tbh

Funnily enough, the "Adrenal System" isn't a system. You might be attempting to refer to the Endocrine system, which includes, the adrenal glands, the thyroid gland, the parathyroid gland, the pancreas, the hypothalamus, and other organs. Given that you're an MD who's engaging in science communication with the public, you have an obligation (ethically and to some extent, legally) to communicate the science transparently and not warp the information you're providing so that it will fit more nicely into your personal (financial) agenda. I guess this all makes sense given that you haven't completed a residency or fellowship of any type, nor are you board-certified in any jurisdiction in the United States, or any other country with a functioning health system.

@@josephpchajek2685 No, I won't. It's people like Peter Attia who jeopardise the work of practitioners who understand the responsibility that comes with the title "Doctor". As people in medical science/medicine/science, we have a duty to the public to present the whole, unbiased, and complete truth to the best of our current scientific understanding. A quick glimpse a most medical journals can, will, and does disprove much of what he says. It's disgraceful, and it's behaviour like this that leads to major public health concerns due to the intentional dissemination of misinformation by educated people for their own personal gain.
