The biggest dedicated Air Ambulance I know is the Dornier Do 328. The bigger Aircrafts like the military MedEvac are simple modular constructions fit in MRTTs and therefore are not medical aircraft per say.

Because other than the DC-10 eye hospital, they ARE the largest dedicated medical aircraft.

every single narration in this video is wrong bcos it doesn't correspond to the aircraft being shown in the video . . . the Bombardier Global 6000 & Dassault Falcon 8X private jets can be a true long haul airborne paramedic aircraft . . . the longer fuselage of both airframes should accommodate upto 3 litter plus paramedics crew . . . Orbit could do with a fleet of 2 second hand refurbished B747-400 ER wide body quad jet or alternatively second hand B777-200 LR wide body jet . . . replacing the aging DC-10 wide body trijet that has been in service since early 1990s . . . conversion to a fully equipped airborne paramedics aircraft will cost anywhere between €246 Mn - €294 Mn (approx) . . .

Each country and each rescue organisation must find the right combination of range, speed, take-off and landing capabilities including ground conditions, loading options (cargo door), number of patients that can be transported with/without ICU equipment and of course the cabin size such as height and width for the medical personnel and also the communication options or the emergency power generator. The RFDS in Australia therefore relies on aircraft that can land and take off on any surface and have sufficient range. In the case of the boat accident in Horizontal Falls at the end of May 2022, it was possible to fly the casualties from Koolan Airport to Perth without a stopover using the 3 medi-jets of RFDS Western Operations. REGA in Switzerland relies on a wide and high cabin with a long range for repatriations. This means that the majority of patients can be transported to their home country without a stopover. In 2009, for example, a patient was flown across the Atlantic at an altitude of more than 30,000 feet for the first time using a special mobile ECMO. In February 2019, REGA flew a woman from Londen to Kaohsiung (Taiwan) with a team of eight and an ECMO machine. With a pure flight time of 14.5 hours and only 1 refuelling stop, the patient arrived in her home country of Taiwan as expected. The Bombardier Challenger 650's range and cabin size are particularly important factors here. Many other aircraft are more suitable for single transports and short distances. Although the ADAC Dornier 328 is equipped with a large cabin, it has a very short range.

The Learjet us a terrible ambulance. Patients on stretchers can't fit through the front door. Patients must be flipped on their side, strapped to the stretcher and contorted around a right corner. My family was a victim of the Learjet. Never again!
