Would the companies come to me and ask me to design the jewelry with the render or will they give me the design and I'll have to re-create them for the render? Also, can I call myself a "3D Jewelry Visualizer"?

Don't mistake a design for a 3D model. A design is not necessarily a 3D model. People ask design, 3d model and render. Sometimes only the 3D model. Sometimes only the render. And many other variations. The problem is not much how you call yourself, but how others are willing to call you.
